Internet and healthcare in Brazil: the role of the Working Group for Healthcare (GT Saúde)

The development of a disposable amperometric immunomigration sensor for the detection of triazine pesticides in real samples is presented. Thick film electrodes printed on PVC were used as strip-type transducers. Monoclonal antibodies against atrazine and terbutylazine served as biorecognition element. For the generation and amplification of the signal, hapten-tagged liposomes entrapping ascorbic acid as a marker molecule were chosen. A capillary gap and a wicking filter membrane strip served as migration zone. For signal detection on a graphite electrode, liposomes were lysed by Triton X-100 and the released ascorbic acid was quantified at a potential of + 300 mV vs printed Ag/AgCl. Signal response time was 1-3 min, and sensitivity of measurements in tap water was below 1 microgram l-1 of atrazine and terbutylazine. Atrazine determinations in soil extracts correlated well with standard procedures based on ELISA and HPLC.     

Healthcare rationing: issues and implications

The purpose of this study is to trace a parallel between the medical discourse content and the expected and stereotyped nurses behavior at the beginning of the twentieth century. It is a historic-social approach in which authors analysed the speeches used by nurses and physicians at nursing and medicine schools. The analysis of the physicians discourse compared to nurses discourse revealed the formation of a mentality shaped by the medical power that wanted them "holy" and servile.

During exercise skeletal muscle glucose utilization is higher than at rest. This is due to the combined effect of an increase in glucose supply, increased surface membrane glucose transport capacity and increased muscle glucose metabolism during exercise. The kinetics of glucose utilization in skeletal muscle during exercise in humans show an apparent Km of approximately 10 mM, indicating that changes in the blood glucose concentration around the physiological level of approximately 5 mM almost linearly translate into changes in muscle glucose utilization. The signalling events responsible for increased glucose transport in contracting muscle are not well understood, although calcium seems to be involved. Contractions do not utilize the proximal part of the insulin signalling cascade to activate glucose transport, because contractions do not cause phosphorylation of insulin receptor substrate 1 or activation of phosphatidylinositol 3-kinase. Endurance training leads to a decrease in glucose utilization during submaximal exercise of a given absolute submaximal power output in spite of a large increase in the total muscle GLUT4 content. The molecular mechanism behind this decrease in glucose utilization seems to be blunted exercise-induced translocation of GLUT4 protein to the sarcolemma, in turn blunting the exercise-induced increase in sarcolemmal glucose transport capacity.

The virtual cutting edge: The Internet and adolescent self-injury.

The 2 studies reported here use observational data from message boards to investigate how adolescents solicit and share information related to self-injurious behavior. Study 1 examines the prevalence and nature of these message boards, their users, and most commonly discussed topics. Study 2 was intended to explore the correlations between content areas raised for discussion. Both studies were intended to shed light on the role of message boards in spreading information about self-injurious practices and influencing help-seeking behavior. More than 400 self-injury message boards were identified. Most are populated by females who describe themselves as between 12 and 20 years of age. Findings show that online interactions clearly provide essential social support for otherwise isolated adolescents, but they may also normalize and encourage self-injurious behavior and add potentially lethal behaviors to the repertoire of established adolescent self-injurers and those exploring identity options. Analyzing the main indices of tuberculosis epidemiology among the convicts showed that there is 1 steady-state upward trend of its morbidity, mortality, bacillary forms. The latters are characterized by the extent of tuberculosis and its severity, which is suggestive of a large infection reservoir among the convicts. This is due to deteriorating macro- and microsocial factors, so this situation should be considered to be extremely explosive both for the convicts and for the general population of the country.     

Online counseling: The Internet and mental health treatment.

The author asserts that the mental health profession is unprepared for the likely growth and related implications of mental health therapy delivered over the Internet. Clinical, ethical, and economic issues are addressed. Conclusions are that therapy can be done online, that it can be done ethically, and that online services might not be a serious threat to face-to-face therapy. Regulatory and professional organizations are strongly advised to cooperate with each other. Suggestions include how practitioners and educators might develop skills, how ethics and legislation might be coordinated, how research might be facilitated, and how certain support systems might be established. Internet: the basics

This three-part series on health legislation describes the policy shift toward regulating the private healthcare system to ensure adequate consumer protection and access to health insurance. Still burning from the failed 1993-1994 healthcare reform effort, Congress and the White House are looking only for incremental policies necessary to reduce the federal deficit and protect the public while the private healthcare system continues to undergo major transformation. In previous articles, the author discussed Medicare, Medicaid, and other federal program revisions and consumer protection legislation aimed at unacceptable managed cure practices. This last article discusses incremental health insurance proposals aimed toward segments of the uninsured.
