This study was designed to investigate the relationship among jaw movements, physical characteristics of food, and sensory perception of hardness in man. Vertical movements of the mandible were recorded with an infrared tracking device in humans during biting on two test foods, carrot and cheese. Samples of standard length (2 cm) and width (2 cm) were prepared in three different thicknesses (0.5, 1.0, and 1.5 cm). Nine subjects were asked to perform two types of bite with their incisor teeth. In the first, they cut through the food, then stopped and spat out the pieces (bite alone); in the second, biting was followed by mastication and swallowing (bite+chew). The 12 conditions (thickness x3, food x2, and bite x2) were presented in a random order within each block, and blocks were repeated five times (60 trials per subject). Subjects also estimated the hardness of the samples twice for each condition on visual analogue scales (VAS) 100 mm long. The duration, vertical amplitude, and maximum vertical velocity of the mandible during biting were calculated by computer for the three phases of the movements (opening, and fast and slow closing). Multilevel statistical models were used for data analysis. The estimated hardness scores associated with the first bite of thin carrot (59.0 VAS units) was significantly greater than for cheese (16.8 VAS units). The type of bite had no significant effect on these scores, but the estimate of hardness was significantly greater for the thickest sample (+13.3 VAS units). Food type had its strongest effect on the slow-closing phase. In particular, the peak velocity that followed the fracturing of the food sample was much greater for carrot than for cheese (thin, 34.1 mm.s-1 vs. 26.6 mm.s-1), and the difference between foods increased with thickness. The amplitude of opening was significantly greater for the thickest sample than for the other two. There were no significant relationships between VAS scores and the movement parameters. These results suggest that, when humans bite food: (1) changing the thickness of food has a greater effect on movement parameters than changing from soft to hard food, (2) the parameters of biting change little if biting is followed by mastication, (3) hardness perception is dependent on the thickness of food, (4) hardness perception is not different when food is removed from the mouth than when it is chewed and swallowed, and (5) there is no relationship between any of the parameters of movement that change with food type and the perceived hardness of food.  相似文献

This study evaluated a sieve method for measuring masticatory performance and determined the associations between masticatory performance and masticatory mandibular movements. Ten dentate adults and three complete denture-wearing subjects participated in the study. The masticatory performance indices for these subjects were determined after a 10 s masticatory sequence and another sequence that ended at the swallowing threshold. Almond was used as the test food and almond fragments were sized using a standard 0.65 mm square size brass wire sieve. Masticatory mandibular movements were simultaneously recorded by an optoelectronic device. The reproducibility of the masticatory performance tests was good (r = 0.98 and 0.82 for the 10 s and swallowing threshold test indices, respectively). The masticatory performance indices were markedly different between dentate and denture-wearing subjects. The 10 s masticatory performance index in the dentate subjects, showed moderate to rather strong correlation with all parameters of mandibular velocity (r = 0.6-0.7). The 10 s index also showed a strong negative correlation to the duration of the occlusal level phase and the total duration of the chewing cycle (r = -0.7 to -0.8) in the dentate subjects. This study confirms that masticatory performance levels are relatively stable and associated with the efficacy of specific masticatory mandibular movement parameters.  相似文献

PURPOSE: To define the risk of seizure recurrence (RSR) that families and physicians would accept before discontinuing antiepileptic drugs (AEDs) for children with controlled epilepsy. METHODS: A questionnaire was completed by families of 76 children with epilepsy > or = 3 months seizure-free and by their attending epilepsy specialist (n = 4). RESULTS: Forty-two percent of families were unwilling to discontinue AEDs with an RSR of 25%. In contrast, 20% were willing to accept a > 75% RSR. Several factors differentiated the risk acceptable to families: previous seizure frequency (risk adverse with intermediate frequency), multiple seizure types (risk taking), grade or grades repeated in school (risk adverse), and the family's strategy of playing lotteries. Although families and physicians were prepared to accept similar median RSR (35 and 40%, respectively), individual answers did not correlate (r2 = -0.07). Physicians were unable to predict the families response (r2 = 0.09). CONCLUSIONS: Our current practice is to discontinue AEDs after 2 years of seizure-free results in seizure recurrence of 30-40%. This risk may seem excessive to more than half of families, whereas other families will risk stopping AEDs at higher risks of recurrence. Physicians are poor judges of the degree of risk that is acceptable to a particular family, which may account in part for the anxiety manifested by families at AED discontinuation.  相似文献

The electromyographic (EMG) activity of the human genioglossus (GG) muscle during chewing efforts is not fully understood. In this study, the EMG activity of the human GG muscle during unilateral gum chewing was illustrated and correlated with the activities in the anterior temporalis (AT), the anterior digastric (DG), and the inferior orbicularis oris (OI) muscles. GG muscle activity was measured with customized surface electrodes, while other muscles were recorded with conventional surface electrodes. EMG activities during tongue displacement and the articulation of long vowels, recorded by the customized electrodes, were consistent with the recordings obtained by fine wire electrodes placed in the GG muscle. Jaw displacement was monitored by means of a kinesiograph with a transducer attached to the mandibular central incisors. Mean normalized GG muscle activity showed an onset in the last one-fifth of the intercuspal phase, gradually increasing during jaw-opening, and at its greatest immediately before the maximum jaw-opening position. It then decreased during jaw-closing and ceased in intercuspation but showed a small rebound in the third fifth of the intercuspal phase. The GG muscle burst showed phase lags with the DG and OI muscles and an opposite phase with the AT muscle (all P < 0.0001). All correlations were statistically significant (all P < 0.0001, r values between 0.88 and 0.97). The results suggest central coordination of the timing of the activities of the jaw, lip, and tongue muscles in chewing.  相似文献

Recently new definitions were agreed for the glucose tolerance test (GTT), for impaired glucose tolerance and for the classification of diabetes mellitus. The World Health Organization and the American Diabetes Association have been active on this point. The fasting glucose value has been lowered and been brought into line with the two hour value of the GTT. Fasting glucose values can now be used for the diagnosis of diabetes mellitus and of impaired glucose tolerance. The new classification is based on differences in cause of the diabetes. The classification includes diabetes mellitus types 1 and 2, pregnancy diabetes and 'other forms of diabetes'.  相似文献

A study of the relationships of the temporomandibular joint disc and the lateral pterygoid, temporalis, and masseter muscles during the human fetal period and in the adult was conducted. The superior head of the lateral pterygoid muscle was seen to insert into the anteromedial two thirds of the temporomandibular joint disc. The fibers of the posterior one third of the temporalis muscle and fibers of the deep bundle of the masseter muscle were attached on the anterolateral one third of the disc. The attachment of these muscles to the disc was through the anterior extension of the disc, also known as the premeniscal or prediscal lamina. The possible functional role of these muscle attachments is discussed.  相似文献

Sex differences in the clinical course of multiple sclerosis were investigated in 947 patients with multiple sclerosis collected from all over Japan through the nationwide survey undertaken by the Multiple Sclerosis Research Committee of Japan from 1972 through 1973. A course of remissions and exacerbations, more frequent involvement of optic nerves, and more severe motor and visual disability were present in a higher proportion of women than men. These features corresponded to those which have been considered to characterize Oriental multiple sclerosis patients.  相似文献

Effects of food consistency on the mode of the phase-linked modulation in the digastric reflex amplitude were examined in naturally chewing rabbits. Two test foods with different textures (bread as a soft food, pellet as a hard food) were used. The digastric reflex was elicited by electrical stimulation (10 train pulses at 2 kHz) of the inferior alveolar nerve. The amplitude of the digastric reflex measured was divided into three categories depending on the chewing phases in which the stimulus was delivered and each value was compared with the control response obtained when the animal was resting. The reflex was strongly inhibited in the jaw-opening phase and no difference was observed in the inhibitory effect between the foods. In the jaw-closing phase, larger digastric reflexes than those in the opening phase were elicited with both foods. This was the case in both the fast-closing and slow-closing phases. Reflex amplitude was significantly larger during chewing of the hard food than the soft food and, thereafter, inhibition of the reflex was observed only during chewing of the soft food in the closing phase. The results suggest the following: (1) food consistency may affect the central mechanism which regulates the digastric reflex and (2) the reflex may contribute to the regulation of masticatory force during chewing particularly hard food.  相似文献

The in vitro metabolism of pirmenol (cis-alpha-[3-(2,6-dimethyl-1-piperidinyl)propyl]-alpha-phenyl-2-pyri dinemethanol) and glucuronidation of its metabolites, a 4-hydroxylated derivative of pirmenol (M3) and 3-methylether of M3 (M5), were investigated using a hepatic 9000 x g supernatant and microsomes, respectively, of female and male rats in order to elucidate the higher urinary excretion of M3G and M5G (glucuronides of M3 and M5, respectively) in females previously observed in in vivo metabolism. Pirmenol delta1' iminium ion (M2) and M3 were formed by the oxidation of pirmenol in both sexes; however, M2 was the main metabolite in males, while M2 and M3 were formed at nearly the same level in females. On glucuronidation of M3 and M5, the Vmax values of both compounds were higher in female rats, consistent with the results in vivo. In addition, the sex difference in the urinary excretion ratio of M5G to M3G (1.1 in female, 2.5 in male) might reflect the lower availability of M3 for glucuronidation in male rats in vivo. The chromatographic separation of diastereomers of M5G was also described.  相似文献

Mortality rates among White Leghorn-Rhode Island Red pure-line and reciprocal cross adult female progeny were recorded for 8 years. The mortality rate among progeny of White Leghorn males exceeded that of the progeny of Rhode Island Red males in both pure-line and crossbreds. White Leghorn X Rhode Island Red crossbred progeny had the highest mortality rate. In a second phase of the study, a comparison was made between mortality rates of male and female progeny of White Leghorn-Rhode Island Red reciprocal crosses. Rate of loss was significantly lower in males than in females but did not differ between crosses. These results support a conclusion that sex linkage is a major factor in differential mortality rates between progeny of White Longhorn-heavy breed reciprocal crosses.  相似文献

We report on a sporadic case of Pfeiffer syndrome in a male newborn with complex craniosynostosis, broad thumbs and great toes and early demise. SSCP and direct sequencing revealed a missense mutation at position 1037 of the exon B (or IIIc) of the FGFR2 gene (codon 342) resulting in a cysteine to serine modification (TGC-TCC). Genotype-phenotype correlations between the FGFRs mutations and the different craniosynostotic syndromes are discussed.  相似文献

Chagas' disease is caused by the hemoflagellate protozoan Trypanosoma cruzi, which is predominantly found in South and Central America and Mexico. Although the parasite is present in the United States, confirmed cases of human disease are rare. The most serious manifestation of chronic Chagas' disease is a progressive inflammatory cardiomyopathy. However, T. cruzi has not been consistently demonstrated with histologic techniques in inflammatory cardiac lesions. In this study, we used both polymerase chain reaction (PCR) amplification of extracted DNA from hematoxylin and eosin-stained tissue scrapings, and in situ hybridization to detect the presence of T. cruzi in infected murine cardiac tissue sections. Three T. cruzi-specific DNA sequences were used: a 122-basepair (bp) sequence localized within the minicircle network (MCS), a 188-bp nuclear repetitive sequence (RS), and a 177-bp sequence within the open reading frame of a gene coding for a flagellar protein (FPS). We found that all three sequences are amplifiable from scrapings of murine cardiac tissue. The MCS and RS are detected at 0.167 and 0.24 amastigote DNA equivalents, while FPS is barely detected at 0.24 amastigote DNA equivalents. On the other hand, in situ hybridization with all three sequences allowed for the detection of T. cruzi amastigotes within the tissue. The MCS and FPS, however, consistently yielded a more intense signal. These results indicate that PCR and in situ hybridization may prove useful in establishing the prevalence of T. cruzi in human chagasic cardiomyopathy.  相似文献

Masticatory and bite forces, when applied to the teeth, generate tremendous compressive energy in the temporomandibular joint (TMJ). Excessive 'TMJ loading', if left untreated, deteriorates articular functions. Normally, it is controlled, to a certain extent, by stomatognathic means. In an attempt to clarify this control mechanism, we analyzed the relationship between TMJ loading and the activities of the masticatory muscles, by employing a static two-dimensional jaw model. This comprises two rigid bodies, the upper and lower jaws, including three dominant muscles, i.e. the masseter, the anterior portion of the temporalis and the lateral pterygoid. Static equilibrium analyses determined that TMJ loading can be minimized, under controlled bite conditions, by pointing the loading vector in a direction solely indicated by individual morphological factors, such as the position and orientation of the masseter and the temporalis. This theoretically optimum direction of TMJ loading was also anatomically acceptable, because the load is applied exactly to those portions of the articular disk and mandibular head that can most easily sustain it. Interestingly, this factor was absolutely independent of both the activities of the lateral pterygoid and the direction of bite force. Consequently. TMI loading can be minimized, by coordinating the activities of the masseter and the anterior portion of the temporalis.  相似文献

The principle aims of this research were, firstly, to determine if the relationship between integrated electromyography (iEMG) and exercise intensity was linear or threshold-like, and secondly, to determine if the relationship between iEMG and exercise intensity was repeatable on different test occasions. A group of 20 trained male subjects participated in the study. Each subject completed two incremental exercise tests on a Monark cycle ergometer. The tests were identical and separated from each other by a mean period of 42 (SD 12) h. The EMG signals were recorded from the vastus lateralis, rectus femoris and vastus medialis muscles at each intensity using surface electrodes. The relationship between iEMG and intensity was shown to be linear (r = 0.95 to r = 0.98) with no obvious iEMG thresholds present. The gradients of simple regression lines fitted to the iEMG compared to intensity were not significantly different on the retest occasion (CV 9%-12%). In summary, the findings of this study indicated that, during incremental exercise, the relationship between iEMG of the quadriceps musculature and exercise intensity was linear and not threshold-like. Furthermore, the linear relationship between iEMG and workload was repeatable on different test occasions.  相似文献

1. Primary and secondary resistance to the widely used antimetabolite 5-fluorouracil (5-FU) are common phenomena in cancer chemotherapy. Because 5-FU still remains the agent of choice in the treatment of, for example, colorectal cancer, circumvention of resistance is of vital importance. 2. Resistance to fluoropyrimidines is a multifactorial event, which includes transport mechanisms, metabolism, molecular mechanisms, protection from apoptosis, and resistance via cell cycle kinetics. To date, the prediction of primary resistance to 5-FU in the clinic is limited to few studies focusing mainly on the key enzyme thymidylate synthase. To gain a deeper insight into the key events responsible for 5-FU resistance in vivo, the evaluation of additional parameters such as other (fluoro)pyrimidine converting enzymes, the mutational status of regulators of apoptosis, and tumour angiogenesis is currently under investigation. 3. Most studies on the circumvention of fluoropyrimidine resistance refer to preclinical investigations and were rarely confirmed in clinical trials. Although our understanding of resistance to 5-FU leaves many open questions, the fundamental insights accomplished during the last years provide a rational understanding to exceed the bounds of the actual therapeutic schedules.  相似文献

Papaverine (0.3-100 microM) more potently inhibited phenylephrine (1 microM)-induced contraction than 65 mM K+-induced contraction of the aorta, while it equally inhibited contractions induced by 65 mM K+ and carbachol (1 microM) in ileal smooth muscle. In phenylephrine-treated aorta, papaverine (1-10 microM) increased the cAMP and cGMP content. However, in carbachol-treated ileum, 30 microM papaverine partially increased the cAMP content while it maximally relaxed the preparation. In fura2-loaded aorta, papaverine (0.3-10 microM) inhibited both the contraction and the increase in intracellular Ca2+ level ([Ca2+]i) induced by phenylephrine in parallel. However, papaverine inhibited carbachol-induced contraction with only a small decrease in [Ca2+]i. Papaverine (1-30 microM) inhibited the carbachol-induced increase in oxidized flavoproteins, an indicator of increased mitochondrial oxidative phosphorylation, in ileal smooth muscle whereas it did not change the phenylephrine-induced increase in the aorta. These results suggest that papaverine inhibits smooth muscle contraction mainly by the accumulation of cAMP and/or cGMP due to the inhibition of phosphodiesterase in the aorta whereas, in ileal smooth muscle, papaverine inhibits smooth muscle contraction mainly by the inhibition of mitochondrial respiration.  相似文献

The purpose of this review is to consider some issues in the interpretation of the electromyogram (EMG) and to discuss current areas of controversy regarding use of the EMG. We consider the underlying physiology and origin of the EMG signal and offer an abbreviated discussion of measurement issues and selected factors that affect the characteristics of the EMG signal. We discuss many of the problems affecting interpretation, including normalization, crosstalk, and issues specific to contraction. In the final section, we consider topics of current interest in electromyography, such as muscle fatigue, task specificity, multichannel representations, and muscle fiber conduction velocity. We present, in addition, alternative analysis techniques. This review should interest researchers and clinicians who seek to obtain the valuable information inherent in the EMG while respecting the potential sources of variance and misinterpretation.  相似文献

The effects of coca chewing on prolonged submaximal exercise responses were investigated in chronic coca chewers and compared with a group of nonchewers. At rest, coca chewing during a 1-h period was followed by a significant increase in blood glucose, free fatty acid, and norepinephrine concentrations and a significant reduction in insulin plasma level. During prolonged (1-h) submaximal (65-70% peak O2 uptake) exercise, chewers displayed a significantly greater adrenergic activation (as evidenced by a higher level of plasma epinephrine) and an increased use of fat (as evidenced by a lower respiratory exchange ratio). The gradual increase in oxygen uptake (O2 drift) commonly observed during prolonged exercise was blunted in coca chewers. This blunting in O2 drift is not related to coca-induced changes in ventilatory or lactate responses to exercise but could possible be related to an enhanced glucose utilization by chewers during the late phase of exercise. The present results provide experimental evidence of the physiological effects of coca chewing that could explain the better ability of coca users to sustain strenuous work for an extended period of time.  相似文献
