The objective of this paper is to tackle methodological issues associated with the inclusion of cognitive and dynamic considerations into Human Reliability methods. A methodology called Human Error Risk Management for Engineering Systems is presented that offers a ‘roadmap’ for selecting and consistently applying Human Factors approaches in different areas of application and contains also a ‘body’ of possible methods and techniques of its own. Two types of possible application are discussed to demonstrate practical applications of the methodology. This paper presents an analysis of accident precursors or ‘near misses’ (e.g. the space shuttle flights with damaged O-rings prior to the Challenger disaster). The interpretation of such events is often problematic, since the ambiguous nature of the evidence in such cases makes them subject to widely divergent interpretations. Simple Bayesian analysis provides a resolution of this problem, showing that an accident precursor almost always justifies increased rather than decreased estimates of overall accident frequencies. 相似文献

The goals of this study were twofold: (1) to confirm a relationship between employee perceptions of psychological safety climate and safety behavior for a sample of workers in the rail industry and (2) to explore the relative strengths of relationships between specific facets of safety climate and safety behavior. Non-management rail maintenance workers employed by a large North American railroad completed a survey (n = 421) regarding workplace safety perceptions and behaviors. Three facets of safety climate (management safety, coworker safety, and work-safety tension) were assessed as relating to individual workers’ reported safety behavior. All three facets were significantly associated with safety behavior. Dominance analysis was used to assess the relative importance of each facet as related to the outcome, and work-safety tension evidenced the strongest relationship with safety behavior. 相似文献

The need for safety analysis has grown in the fields of nuclear industry, civil and military aviation and space technology where the potential for accidents with far-reaching consequences for employees, the public and the environment is most apparent. Later the use of safety analysis has spread widely to other industrial branches. General systems theory, accident theories and scientific management represent domains that have influenced the development of safety analysis. These relations are shortly presented and the common methods employed in safety analysis are described and structured according to the aim of the search and to the search strategy. A framework for the evaluation of the coverage of the search procedures employed in different methods of safety analysis is presented. The framework is then used in an heuristic and in an empiric evaluation of hazard and operability study (HAZOP), work safety analysis (WSA), action error analysis (AEA) and management oversight and risk tree (MORT). Finally, some recommendations on the use of safety analysis for preventing accidents are presented. 相似文献

Research has shown that inexperienced drivers underestimate the risks associated with a range of driving situations. In addition, personality factors are an important influence on both risk perceptions and driving behaviour. The study investigated the strength of relationship between personality factors, risk perceptions, and driving behaviour among young, mainly inexperienced drivers. One-hundred and fifty-nine students aged between 17 and 20 completed an online questionnaire assessing five facets of personality, four components of risk perceptions, and one measure of driving behaviour. Using structural equation modeling as a means of assessing the overall fit of each model, 39% of the variance in young drivers' speeding was accounted for by Excitement-Seeking, Altruism, their Aversion to Risk Taking, and their own likelihood of having an accident, with Altruism and Aversion to Risk Taking both showing moderate negative relationships. Road safety interventions should examine whether they strengthen young drivers' appreciation of the impact of their actions on others through positive reinforcement of altruistic norms. 相似文献

Analysis of driving exposure and accident rates will be important in the future for evaluations of highway safety countermeasures. Each should be uniquely classified according to variables of the highway transportation system, i.e. driver-vehicle-road-environment combinations, in order to allow analyses which deal with the most significant differences in exposure and accident rate. An important part of the AP600 design, as well as of the design certification review by the US Nuclear Regulatory Commission, is devoted to ensuring defense in depth through deep consideration and management of severe accidents. Going beyond the traditional Level 2 PRA, in this article we show how this defense in depth was achieved and demonstrated in a consistent manner between prevention and mitigation, through application of the Integrated ROAAM approach. This requires the up-front integration of probabilistic and deterministic thought, which leads naturally to clear and coherent safety goals as an overall guide toward closure. 相似文献

Many safety educators firmly believe that good safety attitudes are a must if people are to avoid accidents in the workplace and elsewhere. This idea has evolved mainly from the industrial safety movement and has been adopted in most fields of safety. However, this fundamental adage has never been tested in agriculture. A random sample of Pennsylvania farmers were asked their attitudes toward nationally recognized farm safety concepts. The Semantic Differential Attitude Test was the instrument used to collect the data. Four hundred and ninety-three respondents indicated they have about the same attitudes toward farm safety concepts regardless of their accident involvement and regardless of other variables studied. The results of this study suggest that the apparent high priority given to safety attitude development as a primary means of accident prevention should be re-examined. 相似文献

Sleep disorders and various common acute and chronic medical conditions directly or indirectly affect the quality and quantity of one's sleep or otherwise cause excessive daytime fatigue. This article reviews the potential contribution of several prevalent medical conditions – allergic rhinitis, asthma, chronic obstructive pulmonary disease, rheumatoid arthritis/osteoarthritis – and chronic fatigue syndrome and clinical sleep disorders – insomnia, obstructive sleep apnea, narcolepsy, periodic limb movement of sleep, and restless legs syndrome – to the risk for drowsy-driving road crashes. It also explores the literature on the cost-benefit of preventive interventions, using obstructive sleep apnea as an example. Although numerous investigations have addressed the impact of sleep and medical disorders on quality of life, few have specifically addressed their potential deleterious effect on driving performance and road incidents. Moreover, since past studies have focused on the survivors of driver crashes, they may be biased. Representative population-based prospective multidisciplinary studies are urgently required to clarify the role of the fatigue associated with common ailments and medications on traffic crash risk of both commercial and non-commercial drivers and to comprehensively assess the cost-effectiveness of intervention strategies. 相似文献

Does the use of a safety belt increase or decrease the perception of the risk of an accident? Young and older male drivers were asked to drive an urban route and rate their perceptions of the risk of an accident. On the first driving trip all subjects were unbelted, while on the second driving trip half of the subjects wore a safety belt while half did not. Results indicated that young male drivers decrease their perception of the risk of an accident as they become familiar with a driving route if they are NOT wearing a safety belt. Young male drivers asked to wear a safety belt sustained their perception of the risk of an accident as they became familiar with the test route. Older drivers' perception of the risk of an accident was not affected by familiarity or safety belt usage. 相似文献

The paper discusses the use of modern information technologies, and in particular geographic information systems (GIS), in the management and control of major accident risk. For this purpose, the regulatory framework of the recent "Seveso II" Directive is briefly described. This asks for more transparent procedures and decision-making, and requires consultation of the public in land-use and off-site emergency planning. Correspondingly, new demands are put to support tools being developed. The main features of tools dealing with hazard sources mapping, risk assessment, risk management, and emergency planning are discussed and examples are given. Moreover, it is argued that, if appropriately designed, their use can enhance the dialog between plant operators, authorities and the public to facilitate a consensus on risk issues. Finally, limitations in the use of these tools and prospects for future developments are discussed. 相似文献

Pinch Analysis is an established method for enhancing the sustainability of industrial processes via efficient use of various resources. It is based on the principle of target identification followed by subsequent system design aided by a problem decomposition strategy based on the Pinch Point. This approach has recently been extended to apply to a broad range of structurally analogous problems in various domains, such as financial management and carbon-constrained energy planning. In this work, a novel graphical methodology for industrial safety risk and environmental management is proposed. In this method, it is assumed that a set of risk or pollution reduction measures is available, and that each measure is characterized by its implementation cost and the degree of benefit that it delivers. These data are then used to generate a source composite curve. Targeting can then be achieved by shifting this curve relative to a pre-defined sink composite curve, which represents the locus of the plant management’s “willingness to pay,” or budget relative to benefits with respect to risk or pollutant reduction. The methodology is then demonstrated on two case studies. The first case is based on the well-known Bhopal incident, while the second case focuses on the reduction of airborne fluoride emissions from brick firing plant. 相似文献

A method has been developed to estimate the relative accident risk posed by different patterns of driving over a multiday period. The procedure explicitly considers whether a driver is on duty or off duty for each half hour of each day during the period of analysis. From a data set of over 1,000 drivers, nine distinct driving patterns are identified. Membership in the patterns is determined exclusively by the pattern of duty hours for seven consecutive days; for some drivers an accident occurred on the eighth day while others had no accident, therefore each pattern can be associated with a relative accident risk. Additional statistical modeling allowed the consideration, in addition to driving pattern, of driver age, experience with the firm, hours off duty prior to the last trip and hours driving on the last trip (either until the accident or successful completion of the trip). The finding of the modeling is that driving patterns over the previous seven days significantly affect accident risk on the eighth day. In general, driving during the early and late morning (e.g., midnight to 10 A.M.) has the highest accident risk while all seven other multiday patterns had indistinguishable risk. Consecutive hours driven also has a significant effect on accident risk: the first hour through the fourth hour having the lowest risk with a fluctuating increase in risk to a maximum beyond nine hours. Driver age and hours off duty immediately prior to a trip do not appear to affect accident risk significantly. These findings quantitatively assess the relative accident risk of multiday driving patterns using data from actual truck operations. Further research is recommended in the areas of refining model structures, adding explanatory variables (such as highway type), and testing more complex models. 相似文献

Although safety analysis is now being put to increased usage, it is often criticized for the quality of the analyses and for the resources needed. Quality systems devised for other industrial purposes offer means for developing methods and criteria to support the quality management of safety analyses.This study evaluated the quality of four safety analyses from the perspective of operational experience, using a new method developed to assess the quality of safety analysis. The study identified some problems that are typical of safety analysis processes and some factors which decrease the quality of analyses. This paper discusses quality assessment, quality management, and means for improving the quality of safety analyses. This paper presents the first reliable estimation of the alcohol-related accident risk in Germany by comparing a representative sample of accidents to a representative sample of trips not leading to a crash. The information about the trips was taken from the German Roadside Survey 1992-1994 (n=9087) conducted in Unterfranken, part of Bavaria. These data were weighted according to a representative study of driving in Germany (KONTIV 89). The accident study comprises a representative sample of accidents in Unterfranken in 1993 (n=1968). Relating accident risk to BAC, the global risk function indicates an exponential increase of accident risk for BACs above 0.05%. Controlling for correlating factors leads to an overall lower estimation with, however, the same structure, indicating that alcohol is consumed by drivers in circumstances which further increase the risk introduced by alcohol. Analyzing the attributable risk (AR) shows that about 12% of all accidents are attributable to alcohol. Over 96% of these happen with BACs of 0.05% and above. Thus, measures aimed at reducing the alcohol-related accident risk must focus on larger BACs, especially of 0.08% and above. 相似文献
