Dual-income parents (N = 122 couples) with an oldest child 18-60 months of age completed the Child-Rearing Disagreements Scale (E. N. Jouriles et al., 1991), the Child Behavior Checklist (T. M. Achenbach & L. A. Rescorla, 2000), and the Verbal Aggression subscale of the Conflicts and Problem-Solving Scale (P. K. Kerig, 1996). Replicating the results of E. N. Jouriles et al. (1991) and extending these findings to daughters and fathers, the authors found links between child-related disagreements and parental ratings of child behavior problems in this low-risk sample. There were no links between fathers' reports of verbal aggression and child behavior problems. Among mothers, however, use of verbal aggression mediated the link between child-related disagreements and ratings of sons' internalizing problems. Verbal aggression did not moderate the link between child-related disagreements and child behavior problems for either mothers or for fathers. This article provides a review of various psychotropic agents employed for children and adolescents with externalizing disorders. With the exception of the stimulants, clinical use of psychotropic medications for children with externalizing disorders far exceeds the available empirical literature. Further, there are insufficient data pertaining to the safety and efficacy of most psychotropics. Studies that examine the combination of psychotherapy and pharmacotherapy are also reviewed, with particular attention to the National Institutes of Health Multimodal Treatment Study for Children with Attention-Deficit/Hyperactivity Disorder. Recommendations are made for future multimodal studies all externalizing disorders and with all psychotropics in addition to the stimulants. Future goals for research over the next decade include studies that examine treatment acceptability, adherence and satisfaction, adherence with various psychotropic agents, and ecologically valid investigations of pharmacotherapies for both symptom reduction and enhancement of functional behaviors among children and adolescents with externalizing behavioral problems. The Psychological Screening Inventory (PSI) was designed for use as a rapid mental health screening device to identify persons who should receive more extensive evaluation. This study evaluated the accuracy of the three screening scales using receiver operating characteristic (ROC), positive predictive power-negative predictive power, and sensitivity-specificity analyses. Persons with major psychiatric disorder (N = 536) were distinguished from control participants by the Alienation scale with an area underneath the ROC curve (AUC) of .905; persons with significant antisocial behavior (N = 410), by the Social Nonconformity (Sn) scale with an AUC of .811; and persons with general psychological distress (N = 620), by the Discomfort (Di) scale with an AUC of .773. There were comparable findings for the other comparison procedures, and some results were better for men and women separately. An empirically developed index (the Overall Screening Index--Revised) showed significant improvement over the Di scale in identifying persons with general psychological distress (AUC = .842). In this study, 18 teachers and 32 of their students who had reading difficulties were randomly assigned to 1 of 2 assessment conditions. In the functional assessment condition, consultants identified functional relationships among environmental events and targeted reading behaviors to develop an intervention plan. In the empiric condition, consultants selected an intervention plan based on interventions matched to various reading problems (omitting individualized problem analysis). Consultants facilitated interventions consistent with the 2 conditions through a problem-solving consultation model and collected multiple outcome measures. Both functional assessment and empiric conditions led to improved outcomes in targeted reading behaviors, with most improvements falling in the moderate to large range of single-participant effect sizes. No significant differences were found between functional assessment and empiric conditions on the outcome variables of intervention effects, curriculum-based assessment, consumer satisfaction, or material costs, although empiric assessment took less time than functional. Implications regarding the generalizability of these results and directions for further study regarding the treatment utility of assessment for consultation are discussed. The author examined relations among demographic risk (income, maternal education, single-parent status), growth in temperament (fear, irritability, effortful control), and parenting (rejection, inconsistent discipline) across 3 years and the prediction of children's adjustment problems in a community sample (N=190; ages 8-12 years at Time 1). Family income was related to higher initial levels of fear, irritability, rejection, and inconsistency and lower effortful control but was not related to changes in these variables. Higher initial rejection predicted increases in child fear and irritability. Higher initial fear predicted decreases in rejection and inconsistency. Higher initial irritability predicted increases in inconsistency, and higher initial effortful control predicted decreases in rejection. When growth of parenting and temperament were considered simultaneously, increases in effortful control and decreases in fear and irritability predicted lower Time 3 internalizing and externalizing problems. Increases in rejection and inconsistent discipline predicted higher Time 3 externalizing, although sometimes the effect appeared to be indirect through temperament. The findings suggest that temperament and parenting predict changes in each other and predict adjustment during the transition to adolescence. This study investigated the interaction of child temperament and maternal discipline in the prediction of externalizing problems in early childhood. Interaction effects were evaluated in a sample of 227 one- to three-year-old children with relatively high externalizing problems scores on the Child Behavior Checklist/1 1/2-5. Child temperament was reported by the mothers, maternal discipline was observed in a laboratory session, and child outcome measures included both mother-reported externalizing problems and observed physical aggression. Results indicate that children with difficult temperaments are more susceptible to negative discipline (i.e., they showed more externalizing problems) as well as more susceptible to positive discipline (i.e., they showed fewer externalizing problems and less physical aggression), as compared with children with relatively easy temperaments. These findings provide empirical evidence for the differential susceptibility hypothesis and suggest directions for enhancing the effectiveness of interventions aimed at reducing early childhood externalizing problems. This study investigated the classification validity of the Dynamic Indicators of Basic Early Literacy Skills (DIBELS) using a sample of kindergarteners (N = 177). Results indicated the cutoff scores for determining at-risk status on the DIBELS produced substantial false negative rates. Cutoff scores identifying students as at some risk produced substantial false positive rates. At both levels of risk status, the DIBELS showed low positive predictive power, but high negative predictive power, indicating it was far better at identifying students with adequate reading skills than those with inadequate reading skills. The authors examined the longitudinal association between externalizing and depressive symptoms using a sample of 185 young adolescents whose mothers had histories of depression. The relation between externalizing behaviors in 6th grade and depressive symptoms a year later was partially mediated by dependent social stressors. Moreover, consistent with the personality-event congruence hypothesis, this mediation model was particularly true for children with high levels of interpersonal orientation (Neediness and Connectedness). In contrast, social stressors that were judged to be independent of the children's behavior, as well as both dependent and independent nonsocial stressors, did not mediate the longitudinal relation between externalizing and depressive symptoms. Despite recent research indicating that 1 of the pivotal times for identifying pathways to early conduct problems is the toddler period, few family-based preventive interventions have been specifically designed to modify child disruptive behavior during this age period. This randomized trial tested the effectiveness of the Family Check-Up in sustaining maternal involvement and preventing the exacerbation of child conduct problems among 120 at-risk toddler-age boys, half of whom were randomly assigned to a treatment condition. The intervention was associated with reductions in disruptive behavior and greater maternal involvement and was particularly effective for children at greater risk for a persistent trajectory of conduct problems. The current study evaluated the associations between externalizing psychopathology and marital adjustment in a combined sample of 1,805 married couples. We further considered the role of personality in these associations, as personality has been found to predict both the development of externalizing psychopathology as well as marital distress and instability. Diagnostic interviews assessed conduct disorder, adult symptoms of antisocial personality disorder, and alcohol dependence. Personality was assessed using the Multidimensional Personality Questionnaire. The Dyadic Adjustment Scale was used to measure marital adjustment. Results indicate that more externalizing psychopathology, greater negative emotionality, and lower communal positive emotionality were associated with reduced marital adjustment in both individuals and their spouses. Low constraint was associated with reduced marital adjustment for individuals but not for their spouses. Multivariate analyses indicated externalizing psychopathology continued to predict marital adjustment even when accounting for overlap with personality. These results highlight the importance of examining the presence of externalizing psychopathology and the personality attributes of both members of a dyad when considering psychological predictors of marital adjustment. This commentary emphasizes the implications for a mental health research agenda that stem from the papers in this special section on dimensional models of psychopathology. These include the need to extend dimensional models to a wider range of psychopathology; the relationship of the dimensions described in these papers, largely based on symptom and self-report measures, to findings from current research in genetics, neuroimaging, and other domains of neuroscience; the need for new scales that can assess the entire range of relevant dimensions with modern psychometric techniques; and ways to employ these dimensions in applied clinical situations. It is concluded that hierarchical dimensional models offer powerful ways of organizing our thinking about psychopathology and will serve to guide many promising avenues of future research. Research indicates that peer victimization contributes to poor school functioning in childhood and adolescence, yet the processes by which victimization interferes with school functioning are unclear. This study examined internalizing and externalizing problems as domain-specific mediators of the association between subtypes of peer victimization (relational, physical) and school functioning (engagement, achievement) with a cross-sectional sample of 337 early adolescents. School engagement was examined further as a proximal process that intervenes in the associations between internalizing and externalizing problems and achievement. Gender differences in these associations were assessed. As expected, internalizing problems showed stronger links with relational than with physical victimization and partially mediated the influence of both on engagement for girls but not boys. Externalizing problems partially mediated the influence of both subtypes of victimization on school functioning for girls and physical victimization for boys. Notably, engagement was a robust mediator of the contributions of internalizing problems and physical victimization to achievement for girls and externalizing problems to achievement for girls and boys. Findings also suggest that physical (but not relational) victimization partially mediates the link between internalizing and externalizing problems and school functioning. A developmental cascade model linking competence and symptoms was tested in a study of a normative, urban school sample of 205 children (initially 8 to 12 years old). Internalizing and externalizing symptoms and academic competence were assessed by multiple methods at the study outset and after 7, 10, and 20 years. A series of nested cascade models was tested through structural equation modeling. The final model indicated 2 hypothesized cascade effects: Externalizing problems evident in childhood appeared to undermine academic competence by adolescence, which subsequently showed a negative effect on internalizing problems in young adulthood. A significant exploratory effect was consistent with internalizing symptoms containing or lowering the net risk for externalizing problems under some conditions. These 3 cascade effects did not differ by gender and were not attributable to effects of IQ, parenting quality, or socioeconomic differences. The authors examined heterogeneity in risk for externalizing symptoms in children of alcoholic parents, as it may inform the search for entry points into an antisocial pathway to alcoholism. That is, they tested whether the number of alcoholic parents in a family, the comorbid subtype of parental alcoholism, and the gender of the child predicted trajectories of externalizing symptoms over the early life course, as assessed in high-risk samples of children of alcoholic parents and matched controls. Through integrative analyses of 2 independent, longitudinal studies, they showed that children with either an antisocial alcoholic parent or 2 alcoholic parents were at greatest risk for externalizing symptoms. Moreover, children with a depressed alcoholic parent did not differ from those with an antisocial alcoholic parent in reported symptoms. These findings were generally consistent across mother, father, and adolescent reports of symptoms; child gender and child age (ages 2 through 17); and the 2 independent studies examined. Multialcoholic and comorbid-alcoholic families may thus convey a genetic susceptibility to dysregulation along with environments that both exacerbate this susceptibility and provide few supports to offset it. The comorbidity of various externalizing behaviors stems from a broad predisposition that is strongly genetically determined (R. F. Krueger, B. M. Hicks, C. J. Patrick, S. R. Carlson, W. G. Iacono, & M. McGue, 2002). This finding raises the question of how externalizing behavior is related to broad personality traits that have been identified in normal populations and that also have a genetic component. Using structural equation modeling, the authors applied a hierarchical personality model based on the Big Five and their two higher order factors, Stability (Neuroticism reversed, Agreeableness, and Conscientiousness) and Plasticity (Extraversion and Openness). Cognitive ability was included to separate variance in Openness associated with Extraversion (hypothesized to be positively related to externalizing behavior) from variance in Openness associated with cognitive ability (negatively related to externalizing behavior). This model was used to predict a latent externalizing behavior variable in an adolescent male sample (N = 140) assessed through self- and teacher reports. This study used data from the National Institute of Child Health and Human Development Study of Early Child Care to examine relations between parenting, self-control, and externalizing behavior among 1st graders. Of special concern was the relation between opportunities for productive activity and behavior problems and whether the relation was mediated by self-control. Evidence in favor of the hypothesis was observed for both mother-reported and teacher-reported externalizing behavior even with substantial controls on the models examined. Self-control also mediated relations for maternal harshness and maternal sensitivity. Early Head Start, a federal program begun in 1995 for low-income pregnant women and families with infants and toddlers, was evaluated through a randomized trial of 3,001 families in 17 programs. Interviews with primary caregivers, child assessments, and observations of parent-child interactions were completed when children were 3 years old. Caregivers were diverse in race-ethnicity, language, and other characteristics. Regression-adjusted impact analyses showed that 3-year-old program children performed better than did control children in cognitive and language development, displayed higher emotional engagement of the parent and sustained attention with play objects, and were lower in aggressive behavior. Compared with controls, Early Head Start parents were more emotionally supportive, provided more language and learning stimulation, read to their children more, and spanked less. The strongest and most numerous impacts were for programs that offered a mix of home-visiting and center-based services and that fully implemented the performance standards early. This study investigated a broad array of putative risk factors for the onset of major depression and examined their screening properties in a longitudinal study of 479 adolescent girls. Results indicated that the most potent predictors of major depression onset included subthreshold depressive symptoms, poor school and family functioning, low parental support, bulimic symptoms, and delinquency. Classification tree analysis revealed interactions between 4 of these predictors, suggesting qualitatively different pathways to major depression. Girls with the combination of elevated depressive symptoms and poor school functioning represented the highest risk group, with a 40% incidence of major depression during the ensuing 4-year period. Results suggest that selected and indicated prevention programs should target these high-risk populations and seek to reduce these risk factors. Children's strategies for coping with parental marital conflict were examined as predictors, mediators, and moderators of the relations between marital conflict and 8- to 11-year-olds' internalizing, externalizing, and physical health problems. In the context of marital conflict, a higher level of active coping and support coping combined was a protective factor against girls' depression symptoms and self-esteem problems and both boys' and girls' health problems. Further, avoidance coping was a vulnerability factor for externalizing, internalizing, and physical health problems in boys, and distraction coping was protective against children's depression and health problems. These findings extend the literature by delineating coping strategies that either protected children against, or heightened their vulnerability to, adjustment and health problems associated with exposure to parental marital conflict. Reviews the book, Pediatric and Adolescent Behavioral Medicine: Issues in Treatment edited by Patrick J. McGrath and Philip Firestone (1983). With recent advances in behavioural medicine more generally, paediatric behavioural medicine has been expanding rapidly during the last decade. At this time, it seems most opportune to integrate notions from psychological and developmental models of treatment with medical treatment models, practice, and services. This book covers a number of childhood and adolescent disorders, including abdominal pain syndrome, asthma, elevated blood pressure, cigarette smoking, and obesity. The issues raised in this edited volume cover a number of important difficulties in the behavioural paediatrics area. Psychologists and other professionals working with difficulties associated with treating paediatric problems in well-child health care facilities will find it very useful.
