False recognition of semantic associates can be reduced when older adults also study pictures representing each associate. D. L. Schacter, L. Israel, and C. Racine (1999) attributed this reduction to the operation of a distinctiveness heuristic: a response mode in which participants demand access to detailed recollections to support a positive recognition decision. The authors examined patients with probable Alzheimer's disease (AD) and older adults with this paradigm. Half of the participants studied pictures and auditory words; the other half studied visual and auditory words. Older adults who studied pictures were able to reduce their false alarms compared with those who studied words only. AD patients who studied pictures were unable to reduce their false alarms compared with those who studied words only and, in fact, exhibited trends toward greater false recognition. Recognition can be guided by familiarity, a restricted form of retrieval devoid of contextual recall, or by recollection, which occurs when retrieval is sufficient to support the full experience of remembering an episode. Recollection and familiarity were disentangled by testing recognition memory using silhouette object drawings, high target-foil resemblance, and both yes-no and forced-choice procedures. Theoretically, forced-choice recognition could be mediated by familiarity alone. Alzheimer's disease and its preclinical stage, mild cognitive impairment (MCI), were associated with memory impairments that were greater on the yes-no test. Remarkably, forced-choice recognition was unequivocally normal in patients with MCI compared with age-matched controls. Neuropathology in hippocampus and entorhinal cortex, known to be present in MCI, presumably disrupted recollection while leaving familiarity-based recognition intact. L. Markson and P. Bloom (1997) concluded that there was evidence against a dedicated system for word learning on the basis of their finding that children remembered a novel word and a novel fact equally well. However, a word-learning system involves more than recognition memory; it must also provide a means to guide the extension of words to additional exemplars, and words and facts may differ with regard to extendibility. Two studies are reported in which 2–4-year-old children learned novel words and novel facts for unfamiliar objects and then were asked to extend the words and facts to additional exemplars of the training objects. In both studies, children extended the novel word to significantly more category members than they extended the novel fact. The results show that by 2 years of age, children honor the necessary extendibility of novel count nouns but are uncertain about the extendibility of arbitrary facts. This study explored the relationship between episodic memory and anosognosia (a lack of deficit awareness) among patients with mild Alzheimer's disease (AD). Participants studied words and pictures for subsequent memory tests. Healthy older adults made fewer false recognition errors when trying to remember pictures compared with words, suggesting that the perceptual distinctiveness of picture memories enhanced retrieval monitoring (the distinctiveness heuristic). In contrast, although participants with AD could discriminate between studied and nonstudied items, they had difficulty recollecting the specific presentation formats (words or pictures), and they had limited use of the distinctiveness heuristic. Critically, the demands of the memory test modulated the relationship between memory accuracy and anosognosia. Greater anosognosia was associated with impaired memory accuracy when participants with AD tried to remember words but not when they tried to remember pictures. These data further delineate the retrieval monitoring difficulties among individuals with AD and suggest that anosognosia measures are most likely to correlate with memory tests that require the effortful retrieval of nondistinctive information. There are a wide variety of neuropsychological deficits in Alzheimer's disease (AD), among which are disorders of visual perception and spatial cognition. The present study investigated the ability of 20 mildly to moderately deteriorated patients with AD (and 174 age- and education-matched controls) on tasks that required them to visually identify, provide the canonical orientation of, and mentally rotate common objects. Some 85% of the AD patients performed poorly on all tasks. The authors were able to identify a small number of individual patients whose pattern of performance represented double dissociations between recognizing objects and knowing their canonical orientation. These findings are interpreted in the context of previous findings, especially as to whether information relating to an object's orientation and identity is independently coded. Recall is typically better for emotional than for neutral stimuli. This enhancement is believed to rely on limbic regions. Memory is also better for neutral stimuli embedded in an emotional context. The neural substrate supporting this effect has not been thoroughly investigated but may include frontal lobe, as well as limbic circuits. Alzheimer's disease (AD) results in atrophy of limbic structures, whereas normal aging relatively spares limbic regions but affects prefrontal areas. The authors hypothesized that AD would reduce all enhancement effects, whereas aging would disproportionately affect enhancement based on emotional context. The results confirmed the authors' hypotheses: Young and older adults, but not AD patients, showed better memory for emotional versus neutral pictures and words. Older adults and AD patients showed no benefit from emotional context, whereas young adults remembered more items embedded in an emotional versus neutral context. Two experiments examined different forms of gist and detail memory in people with Alzheimer's disease (AD) and those with amnestic mild cognitive impairment (MCI). In Experiment 1, 14 AD, 14 MCI, and 22 control participants were assessed with the Deese-Roediger-McDermott paradigm. Results indicated that false recognition of nonstudied critical lures (gist memory) was diminished in the AD compared with the MCI and control groups; the two latter cohorts performed similarly. In Experiment 2, 14 AD, 20 MCI, and 26 control participants were tested on a text memory task. Results revealed that recall of both macropropositions (gist information) and micropropositions (detail information) decreased significantly in AD and in MCI as compared with control participants. This experiment also revealed that the impairment was comparable between gist and detail memory. In summary, the results were consistent across experiments in the AD but not in the MCI participants. The discrepancy in MCI participants might be explained by differences in the degree of sensitivity of the experimental procedures and/or by the differences in the cognitive processes these procedures assessed. Awareness of deficit was examined in 24 patients with Alzheimer's disease (AD) and their spouses (for a total of 48 participants) using performance prediction-postdiction and questionnaire discrepancy (QD) paradigms. Participants estimated their own memory performances as well the performances of spouses and of a fictional, memory-disordered patient observed on videotape. Patients overpredicted self-performances, but the extent of overestimation decreased for postdictions. Patients and caregivers accurately estimated caregiver performances but overestimated performances of the fictional patient. QD data revealed that patients underestimated their difficulties performing daily functioning tasks as compared with caregiver reports. Awareness of deficit is a complex ability, involving dissociable cognitive processes. AD patients may display intact immediate awareness of memory dysfunction but fail to incorporate incidents of memory failure into generalized self-belief systems. Memory encoding and retrieval strategies were assessed in patients with behavior-executive variant frontotemporal dementia (FTD), language variant FTD, and Alzheimer's disease (AD) using verbal and visuospatial supraspan learning tests. FTD patients obtained higher free recall, cued recall, and recognition scores than AD patients. Comparison of free recall scores with cued recall and recognition scores was similar in the 3 dementia groups. Groups did not differ in semantic clustering strategies during learning, but serial-order recall was more common in FTD patients. These data do not support the idea that FTD patients' poor memory is due to a selective retrieval disorder, though FTD patients may fail to implement sophisticated organizational strategies during learning. FTD patients' retained capacity for encoding new information into long-term declarative memory is likely due to relatively spared medial temporal lobe involvement. Forty-six preclinical Alzheimer's disease (AD) participants and 188 nondemented control persons from the Kungsholmen Project (L. B?ckman et al., 2004) were compared on prospective memory (ProM) and retrospective memory (RetM) tasks 3 years before dementia diagnosis. The preclinical AD participants showed deficits in both ProM and RetM. Most interestingly, logistic regression analyses revealed that ProM made an independent contribution to the prediction of AD over and above that of RetM. This finding suggests that ProM and RetM tap partly different cognitive operations. Furthermore, within the ProM task, both the retrospective and prospective components were similarly impaired in preclinical AD. Within RetM, the preclinical AD participants were impaired on indices of encoding, storage (forgetting), and retrieval of information. Hence, the findings indicate a rather global episodic memory impairment in preclinical AD that cuts across type of memory assessed (ProM and RetM) as well as across different components of both the ProM and RetM tasks. Rats with excitotoxic hippocampal lesions were trained on delayed nonmatching-to-sample (DNMS) with small goal boxes, containing complex objects, presented on a pseudo trial-unique schedule. A series of experiments then tested performance on repeated presentation of either the small object or large empty goal boxes. All rats acquired the nonmatching rule, but hippocampal-lesioned rats performed less well than controls on choice accuracy for the final 2 blocks of acquisition. In the study's main phase, the lesions impaired choice accuracy when the large empty boxes were used as stimuli. This deficit was ameliorated when the rats were tested with the small object boxes, although the performance of the hippocampal-lesioned rats was still below that of controls. These results extend previous reports of box size-dependent effects of hippocampal aspiration lesions on DNMS and suggest that selective damage to the hippocampus, not neuronal loss in adjacent structures or fiber tracts, is critical for the effect. The authors investigated the recognizability of recently studied word and nonword stimuli in relation to both experimentally controlled prior frequency of occurrence and, for words, normative frequency (assessed by counts of occurrences in printed English). The interaction between these variables was small and nonsignificant across all conditions of 2 experiments. Patterns of recognition measures in relation to controlled prior frequency, but not normative frequency, appeared interpretable in terms of response biases generated by long-term priming. Application of a global memory model and analyses of correlations among item categories yielded evidence for a lexicality dimension underlying normative-frequency effects and an implication that "word-frequency effects" on recognition are better termed lexicality effects. A test measuring knowledge of unusual meanings of familiar words was constructed, and was hypothesized to be more sensitive than traditional vocabulary tests to language impairment in schizophrenic and brain-damaged patients. In addition, the oft-noted inferiority of these patients on tests of abstract reasoning, as compared to vocabulary tests, was hypothesized to be a function of insensitivity of usual vocabulary tests. The new test provides evidence supporting both hypotheses in schizophrenic but not in brain-damaged patients. The results suggest that the similar outcome of impaired abstract reasoning in brain damage and schizophrenia may results from different processes in each disorder. (16 ref.) We taught a novel animal category by rule-based and similarity-based processes to participants with Alzheimer's disease (AD), corticobasal degeneration (CBD), and healthy age-matched participants. Healthy participants successfully categorized by either process. AD patients' rule-based categorization was impaired, while their similarity-based categorization resembled that of healthy participants. Correlations of AD patients' performance with measures of executive functioning suggested a deficit in the cognitive resources necessary for engaging rule-based categorization. The contribution of limited executive resources to categorization difficulty in AD was further demonstrated in a second experiment in which features determining category membership were of lower salience. CBD patients were relatively impaired at similarity-based processing, suggesting that qualitatively distinct categorization processes can be selectively compromised in patients with focal neurodegenerative diseases. Moreover, AD patients' impaired categorization correlated with performance on a measure of semantic memory, implicating this categorization deficit in AD patients' semantic memory difficulty. The distinctiveness heuristic is a response mode in which participants expect to remember vivid details of an experience and make recognition decisions on the basis of this metacognitive expectation. The authors examined whether the distinctiveness heuristic could be engaged to reduce false recognition in a repetition-lag paradigm in patients with Alzheimer's disease (AD). Patients with AD were able to use the distinctiveness heuristic--though not selectively--and thus they showed reduction of both true and false recognition. The authors suggest that patients with AD can engage in decision strategies on the basis of the metacognitive expectation associated with use of the distinctiveness heuristic, but the patients' episodic memory impairment limits both the scope and effectiveness of such strategies. Earlier studies in monkeys have reported mild impairment in recognition memory after nonselective neonatal hippocampal lesions. To assess whether the memory impairment could have resulted from damage to cortical areas adjacent to the hippocampus, we tested adult monkeys with neonatal focal hippocampal lesions and sham-operated controls in three recognition tasks: delayed nonmatching-to-sample, object memory span, and spatial memory span. Further, to rule out that normal performance on these tasks may relate to functional sparing following neonatal hippocampal lesions, we tested adult monkeys that had received the same focal hippocampal lesions in adulthood and their controls in the same three memory tasks. Both early and late onset focal hippocampal damage did not alter performance on any of the three tasks, suggesting that damage to cortical areas adjacent to the hippocampus was likely responsible for the recognition impairment reported by the earlier studies. In addition, given that animals with early and late onset hippocampal lesions showed object and spatial recognition impairment when tested in a visual paired comparison task, the data suggest that not all object and spatial recognition tasks are solved by hippocampal-dependent memory processes. The current data may not only help explain the neural substrate for the partial recognition memory impairment reported in cases of developmental amnesia, but they are also clinically relevant given that the object and spatial memory tasks used in monkeys are often translated to investigate memory functions in several populations of human infants and children in which dysfunction of the hippocampus is suspected. The present study examines 2 factors that might moderate the object-recognition deficit seen after perirhinal cortex damage. Object recognition by normal rats was improved by extending (from 4 to 8 min) the sample period during which an object was first explored. Furthermore, there was a significant positive correlation between time spent in close exploration of the sample object and degree of successful novelty discrimination. In contrast, rats with perirhinal cortex lesions failed to benefit from increased close exploration and did not discriminate the novel object after even the longest sample period. Nevertheless, the lesions did not disrupt habituation across repeated exposure to the same object. The second factor was extent of perirhinal cortex damage. A significant correlation was found between total perirhinal cortex loss and degree of recognition impairment. Within the perirhinal cortex, only damage to the caudal perirhinal cortex correlated significantly with recognition memory deficits. This study highlights the critical importance of the perirhinal cortex within the temporal lobe for recognition memory and shows that the lesion-induced deficit occurs despite seemingly normal levels of close object exploration. Previous research suggests that patients with Alzheimer's disease exhibit cognitive impairment in the years preceding a clinical diagnosis. Memory impairments are particularly pronounced, but the relative degree to which other cognitive functions are impaired and the speed with which they decline during the preclinical years remains unclear. The authors report a detailed neuropsychological evaluation of 11 patients over the course of 3 years up to and including the 1st year of nonnormal diagnosis. The results suggest that performance falls off rapidly in all areas of cognitive functioning but that abilities thought to be subserved by the medial and lateral temporal lobes (episodic and semantic memory, respectively) appear to be substantially more impaired than those abilities thought to be subserved by the frontal lobes. The authors present a computational neural-network model of how the hippocampus and medial temporal lobe cortex (MTLC) contribute to recognition memory. The hippocampal component contributes by recalling studied details. The MTLC component cannot support recall, but one can extract a scalar familiarity signal from MTLC that tracks how well a test item matches studied items. The authors present simulations that establish key differences in the operating characteristics of the hippocampal-recall and MTLC-familiarity signals and identify several manipulations (e.g., target-lure similarity, interference) that differentially affect the 2 signals. Objective: This study examines working memory (WM) in mild cognitive impairment (MCI) and Alzheimer's disease (AD).
