New research directions on the effectiveness of mental health services for children and adolescents offer the opportunity for school psychology to apply its knowledge base to the systemic juncture between mental health and school systems. Models of service delivery to children, adolescents, and their families that integrate school, mental health, and other service sectors are being actively studied to answer questions about the outcomes of these services for children with mental health problems. The papers in this journal were first presented at the 6th Annual Research Conference of the Florida Mental Health Institute on "A System of Care for Children's Mental Health: Expanding the Research Base." The papers describe state-of-the art studies of school-based mental health interventions for children, adolescents, and their families. In each of the papers, particular attention is paid to the salient methodological issues researchers face in conducting these studies within school settings. It is hoped that these articles will foreground the healthy and creative tensions that exist between different research paradigms and multiple service communities, especially mental health and school systems, by encouraging new research on important and as yet unanswered questions about the effectiveness of school-based service delivery to children and adolescents with mental health needs. "In preparation for the 1959 meeting of chief psychologists in state mental health programs a questionnaire on research was prepared and sent to 77 psychologists who were either functioning as chief psychologists in state programs" or in similar positions. Replies were received from 39 individuals, representing 30 states. "Evaluation of treatment was listed most frequently as an area in need of research." At least 45 different types of ongoing research projects were listed. "Three factors were emphasized as the main obstacles to research in the state programs: lack of personnel… lack of funds… emphasis on service, with subsequent lack of time for research." Proposes 4 principles for community mental health programing that are consistent with an ecological thesis: (1) Assessment methods are focused on the total population rather than on those persons who presently receive a mental health service. (2) Mental health services are designed to reduce a high risk for community service. (3) Professional and research services are created as local community resources. "By initiating the informal coordination of current services, the community mental health program helps to create specific new community services as needed." (4) The program plans for change; this involves mobilizing anticipatory problem-solving resources not only for clients but for professionals as well. Critical comments regarding the terms "mental health" are indicated. The following points are elaborated: "1. Among many other things, mental health is a vast social movement which, for better or for worse, will carry us all along with it. 2. Psychologists, for a number of reasons, have not been enthusiastic participators in this social movement. 3. Psychologists have a great deal to contribute to the mental health movement, and they can contribute mightily without loss of identity." Mental health as a social movement seems due to three factors: (a) The tendency to conceive human behavior in naturalistic terms. (b) The American belief in the almost infinite improvability of almost anything. (c) The rising level of aspiration for health and well-being. The scientific humility of the psychologist "can become maladaptive diffidence, and diffidence sometimes becomes irresponsibility." 6 suggestions for dealing with diffidence are indicated. The psychologists perceptions of mental health, the orientation of the movement toward the "pathological," and the focus on the clinical may have discouraged the interests of psychologists in mental health. "Psychologists are assuming a wider range of responsibilities in community program administration and community consultation: in mental health, corrections, education, and anti-poverty." A program for training consultants was developed in New Mexico during a 4-yr demonstration project. Selection criteria include: (1) sound graduate training including the PhD for psychologists, (2) substantial clinical skills in diagnosis and therapy, (3) experience in consultation and administration, and (4) experience in community work. A 2-mo., full-time orientation program was conducted for the consultants hired. The project stimulated development of various new resources in local communities including, among others, a day school for retarded children, a family casework agency, a day center for emotionally disturbed children, a training and consultation service to an orphanage, and an alcoholism treatment and rehabilitation program. "The study reported here involves the application of opinion sampling methods to public attitudes toward mental health professions." The following titles were employed: doctor, physician, nurse, psychiatrist, psychoanalyst, psychologist, clinical psychologist, research psychologist, mental hospital attendant, social worker. The Semantic Differential was used. The sample of respondents was obtained from the Opinion Panel of the Institute of Communications Research; 207 of 239 panel members returned questionnaires on time. "A very high, positive, public attitude was found for the professions as a whole." Virtually no distinction is made between the concepts doctor and physician. "No distinction is made… among the concepts Psychiatrist, Psychoanalyst, Clinical Psychologist, and Research Psychologist on the Semantic Differential… . There are large and significant differences between the concept Doctor and Physician and the cluster of concepts whose titles start with the morpheme 'Psych-.' It is "our belief that the field of community mental health is a challenging and stimulating field for research oriented psychologists. We feel strongly that the identity of the psychologist may be best maintained through a scientist-professional model." Recommendations are made for the future training of psychologists in this field. "The American psychological profession is taking a leading role in the nation's first regional program to improve mental health and combat mental disease which the Southern Regional Education Board is conducting." "We are witnessing a conspicuous change in the climate between religion and science in recent years." In psychology this has been evidenced in seminars on religion and psychiatry, work shops on pastoral care and psychotherapy, grants to universities for developing mental health curricula for theological students, creation of an Academy of Religion and Mental Health, and the establishing of an APA committee to study relationships between religion and mental health. The extensive problems of mental illness are reviewed. "Either we must develop new kinds of mental health personnel or we must develop new skills in the personnel we already have. This is no longer a pious statement; it is an absolute necessity in the imminence of approaching events." Problems involved in training specialists are considered. Nadine Lambert, one of the 1986 recipients of the Distinguished Professional Contributions Award, is recognized for making "numerous contributions to the professional practice of psychology. In 1960, Dr. Lambert was among the first to advocate and support multilevel legislation for children with learning and mental health problems in California. In 1965, she initiated and developed the first doctoral level professional school program in California. She is also responsible for the adaptation, validation, and standardization of the AAMD-SE Adaptive Behavior Scale. Dr. Lambert's research on the social and the psychological antecedents of various childhood and adolescent mental health problems, as well as her work with hyperactive children, has contributed significantly to the improved treatment and understanding of many childhood and adolescent behavioral problems." There are many barriers to successful recruitment of professional and subprofessional personnel in the mental health professions. The companion program "recruits students from… universities to serve as volunteer companions to mentally ill patients for a full academic year. Students are invited to either select or be assigned to a chronic patient who has minimal or no contact with individuals outside the hospital, and to meet with this patient for one hour on a weekly basis throughout the school year… . The goal that is stressed throughout is that the student must build a feeling of companionship between himself and the patient." My book Pure types are rare: Myths and meanings of madness (see record 2007-08930-001) may have piqued some members of the mental health community; thus I was not entirely surprised by the caustic review that appeared in the January 1985 issue of Canadian Psychology, nor do I wish to engage the polemics or the personal asides therein. I am impelled, however, to respond to the allegation of "repeated failure of professional scholarship." The reviewer provides three substantive examples. In the first, he objects to the citation following my statement that post-mortem studies have failed to find neurological correlates of senile dementia, and implies that the study cited did find such correlates. Actually, the study was inadequate for the question inasmuch as statistics were not presented for senile dementia exclusive of other conditions, and normal controls were not used. The second maintains that I underestimated the significance of a concordance rate of 53% for the diagnosis of schizophrenia between two psychiatrists examining the same patient, and says it should actually be "somewhere beyond even the .00000001 level." My point, however, was that in vivo, where more than 50% of mental patients are diagnosed schizophrenic, a 53% concordance rate represents chance. Third, Professor Burd relates several sentences of mine to make it appear that I questioned the validity of the co-twin data on genetic factors in schizophrenia on the "curious grounds" that similar data have been obtained for a variety of behavioural and personality traits. I will comment on the statement "There are but a dozen references to articles in established professional journals." I did not attempt to deduce the criteria for "established," nor did I count journals, but I did ascertain that there were 112 citations in the text, comprising 80 separate sources.
