Purpose

To describe the scanning slit topography in two children with presumed unilateral Terrien's marginal degeneration.

Methods

Two girls aged 9 and 12 years presented with redness and reduced vision in their right eyes. The younger patient had scarring adjacent to the superior limbus, with vessels coursing across, and lipid deposition. Visual acuity improved to 20/20 from 20/400 with +1.0 −3.5 × 90°. The older patient had similar features inferiorly. Her visual acuity improved to 20/20 from 20/40 with −3.5 × 65°. Their left eyes were normal. A diagnosis of unilateral Terrien's Marginal Degeneration (TMD) was made.

Results

Scanning slit topography (Orbscan II™, Orbscan Inc., Salt Lake City, UT) of the right eyes revealed high against-the-rule astigmatism. The left eyes showed elevations of anterior and posterior corneal surface above the best-fit-sphere, skewing of principal meridians and increased astigmatism from 3.0 to 5.0 mm zones. Based on these findings bilateral asymmetric TMD was diagnosed.

Conclusion

Though the diagnosis of TMD is mainly clinical, scanning slit topography findings in TMD have not been described earlier and are useful in establishing the diagnosis of bilateral asymmetric TMD.  相似文献

1 Scope

We investigate the effects of extracellular vesicles (EVs) obtained from in vitro adipocyte cell models and from obese subjects on glucose transport and insulin responsiveness.

2 Methods and results

EVs are isolated from the culture supernatant of adipocytes cultured under normoxia, hypoxia (1% oxygen), or exposed to macrophage conditioned media (15% v/v). EVs are isolated from the plasma of lean individuals and subjects with obesity. Cultured adipocytes are incubated with EVs and activation of insulin signalling cascades and insulin‐stimulated glucose transport are measured. EVs released from hypoxic adipocytes impair insulin‐stimulated 2‐deoxyglucose uptake and reduce insulin mediated phosphorylation of AKT. Insulin‐mediated phosphorylation of extracellular regulated kinases (ERK1/2) is not affected. EVs from individuals with obesity decrease insulin stimulated 2‐deoxyglucose uptake in adipocytes (p = 0.0159).

3 Conclusion

EVs released by stressed adipocytes impair insulin action in neighboring adipocytes.  相似文献

Purpose

The aim is discussing the origins of worsening of external eye condition (EEC) and of tear film (TF) instability after wear of silicone-hydrogel contact lenses (CLs) with hydrogen-peroxide (H2O2) care system.

Methods

EEC and TF stability were evaluated before and after 15 days of wear combined with different care systems: (1) H2O2, (2) detergent solution and H2O2, (3) multipurpose solution (MPS), (4) H2O2 and artificial tears. In-vitro cell mortality tests were performed after 24 h cell incubation with CLs treated with H2O2. Photon correlation spectroscopy (PCS) was carried out on tears of non-wearers and CL wearers who used MPS or H2O2 solution.

Results

Worsening of EEC was observed only for the group using H2O2 (group 1). In-vitro, cell mortality was found higher for worn CL than for unworn CLs. Worsening of TF stability was observed regardless of care system and also PCS results on tears of CL wearers were found different compared to non-wearers regardless of care system. The only observed remedy for tear instability of CL wearers was found to be the administration of artificial tears.

Conclusions

Worsening of EEC of CL wearers using H2O2 is attributed to H2O2 scarce cleaning efficacy, which can be solved by adding a CL detergent solution. The origin of TF instability is found to be different. A remedy was found to be the administration of artificial tears, whose effect could be attributed either to the role of specific components or to rinsing and replacement of TF during wear.  相似文献
