Discusses the relationship of scientific psychology to the practice of psychotherapy. The use of research to change psychotherapy techniques is examined in each of 3 major orientations of psychodynamic, experiential, and behavioral psychotherapy. Common factors that may be responsible for the success of all therapies are described, and the controversy between those who see psychotherapy as a cultural ritual and those who see it as a scientifically-based procedure is reviewed. The consequences of a split between researcher and clinician roles in clinical psychology are predicted. It is concluded that scientific psychology and psychotherapy can together contribute meaningfully to society and a divorce would not be in the best interest of either discipline. The philosophy, goals, objectives, methodology, and results of a family practice faculty development program are described. Developing family practice educators who will create an education system based on patient care outcomes in family practice settings is the central philosophical purpose of this faculty development program. On completion of the program all participants recognized the essential nature of this philosophical goal and were more comfortable and confident in their ability to: (1) determine resident learning needs; (2) organize curriculum units; (3) use different teaching techniques; and (4) understand their own personal teaching needs and interests. Malignant mesenchymal tumours of the breast are rare neoplasms. Their incidence varies from 1%-3% of all malignant breast tumours (1). Stromal sarcoma of the breast is an extremely uncommon tumour (2). We present a rare case of advanced stromal sarcoma of the breast successfully managed with radical surgery and radiotherapy.  相似文献

Positive practice, a brief integrative approach to consultation with families, is described in this paper. A clear distinction is made between the stages of planning, assessment, therapy, and disengagement. Guidelines for progression from one stage to the next are provided. Frameworks for deciding who to invite to preliminary sessions and methods for planning and organizing lines of inquiry are incorporated into this approach to practice. A three-column model is used to construct formulations. The model allows therapists and clients to map information about the pattern of interaction around the presenting problem, beliefs that constrain family members from altering their roles in these problem-maintaining patterns, and factors that have predisposed family members to hold these beliefs. Positive practice offers methods for evolving new behavioral patterns and belief systems within sessions and for arranging homework tasks for clients between sessions. It also incorporates methods for dealing with resistance, for managing therapeutic crises, for convening individual sessions and broader network meetings, for disengaging from the consultation process, and for recontracting for further episodes of therapy. This evolving approach to practice draws on ideas from many traditions within the family therapy field and takes account of recent research relevant to the practice of family therapy.  相似文献

A study of 100 selected Wisconsin family physicians demonstrated that younger physicians generally had more contact with younger patients, middle-aged physicians had an even distribution of patient age contacts, and older physicians had more contact with older patients. The increase in older patients became pronounced for 56 to 60-year-old physicians. Generally a ten-year increase in physician age was accompanied by a five-year increase in patient age. The rising average age of the American population, combined with the direct physician-patient age relationship demonstrated here suggest that geriatrics will become increasingly relevant not only in the continuum of medical education but also for the individual practicing physician.  相似文献

Sleep was actigraphically investigated in 27 Kibbutz children while sleeping in communal sleeping houses, and 1 year after changing to familial sleeping arrangements. Three independent control groups of city-living children were also recorded. Two of them were age-comparable, and the third control group was included in order to examine possible effects of the Gulf War on the communal sleep group. The results showed that the sleep quality of Kibbutz children improved significantly after moving to familial sleep. Comparison with the data from the two control groups revealed a greater resemblance between sleep of the Kibbutz children after moving to live with their families and that of the city-living children. Comparing the sleep of the children in communal sleep to that of the additional group of children examined during the Gulf War strengthened the above results, i.e. the communal sleep group that was investigated before the war slept worse than the control children that were investigated during the war. After discarding developmental and physical condition-related changes, it was concluded that the improvement in sleep quality was due to the children's increased sense of security when sleeping with their families.  相似文献
