The effect of post canal preparation on the apical seal of endodontically obturated teeth with plastic Thermafil obturators was studied in vitro. Fifty-four extracted human maxillary anterior teeth were divided into four experimental and two control groups. Two groups were obturated with lateral condensation or plastic Thermafil obturator techniques; this was followed by post space preparation. Two groups were obturated as described above except that post space was not prepared. Apical leakage was determined with the use of 2% methylene blue dye solution at an absolute pressure of 75 mm Hg. There were no statistically significant differences between any of the groups. The clinical implication of these findings is discussed.  相似文献

OBJECTIVE: Marginal leakage of amalgam restorations may lead to secondary caries and pulpal damage. The purpose of this study was to determine the effect various cavity liners might have on microleakage. METHOD AND MATERIALS: Mesio-occlusodistal amalgam restorations with margins on enamel and dentin were treated with different liner materials (an adhesive system, a topical fluoride gel, a cavity varnish, and a glass-ionomer cement) in vitro. Following restoration, the teeth were submitted to thermocycling in a stained solution and sectioned to allow assessment of microleakage. RESULTS: On enamel, the control group (no liner) and the glass-ionomer-lined group had equivalent leakage scores and were superior to every other group. On dentin, only the glass-ionomer specimens had superior performance. The cavity varnish and fluoride-lined specimens exhibited the highest leakage scores. CONCLUSION: The use of liners does not reduce microleakage on amalgam restorations when the cavity margins remain on enamel. On dentin margins, a glass-ionomer liner can reduce microleakage.  相似文献

An in vitro dye leakage study was performed to compare the sealing ability of glass ionomer, composite resin and glass ionomer/resin cement when used as restorative materials for G.V. Black class V cavities. In this research, standard Class V cavities were prepared in sound premolar teeth extracted for orthodontic reasons. The cavities were randomly divided into 3 groups. After filling the cavities with glass ionomer cement, composite resin and light curing glass ionomer/resin cement, the specimens were immersed in silver nitrate solution. Marginal microleakage at the interface between the cavity wall and restoration was evaluated. The results were analyzed by using Kruskal-Wallis and Mann Whitney U tests. Result of this in vitro study indicate that composite resin and glass ionomer/resin cement provide a better seal than glass ionomer cement.  相似文献

PURPOSE: To evaluate a dental adhesive system that uses a single conditioning/primer agent. MATERIALS AND METHODS: Twenty-five flat enamel and dentin bonding sites were prepared to 600 grit on human molar teeth. The Clearfil Liner Bond 2 adhesive system was used to bond Clearfil AP-X composite to both enamel and dentin. After 24 hours of water storage, shear bond strengths were determined using an Instron testing machine. Fifty V-shaped cavity preparations were prepared in human molar teeth with an enamel and cementum margin. Composite restorations were placed using the new adhesive system. The teeth were stored for 24 hours, thermocycled, stained with AgNO3 , sectioned and examined for microleakage. SEM examinations were also completed to evaluate the effects of the treatment steps on enamel and dentin surfaces. RESULTS: Mean shear bond strengths for the experimental adhesive to enamel and dentin were 28.2 +/- 4.9 and 19.4 +/- 3.1 MPa. A t-test revealed that the enamel bond strength was significantly greater (P<0.05) than the dentin strength. No marginal leakage was observed from the enamel margins of the restorations. Three restorations showed minimal leakage from the cementum margins. SEM examinations showed resin penetration into both the conditioned enamel and dentin surfaces. The adhesive system produced high bond strengths to both enamel and dentin, exhibited very minimal microleakage and was easy to use.  相似文献

The objective of this study was to compare the effects of a flame-heated endodontic plugger, an electrically-heated endodontic spreader, Peeso reamers, and GPX burs on the apical seal when gutta-percha was removed from the root canal for post space preparation. Eighty-nine human canines were instrumented and obturated with gutta-percha and sealer to a standardized working length of 15 mm. An 11-mm segment of gutta-percha was removed from the roots in each of the four experimental groups of 20 teeth to simulate post space preparation. The teeth were immersed in India ink for 7 days and rendered transparent. The extent of linear dye leakage was measured with a stereomicroscope. There was no statistically significant difference in linear dye leakage among any of the experimental groups when 4 mm of gutta-percha remained.  相似文献

The objective of this laboratory study was to compare root-end cavities prepared with sonic Retro-prep tips in a MM1500 Sonic Air handpiece with those created by burs in a conventional handpiece. A total of 80 single-rooted extracted human teeth with mature apices and straight canals were included in the study. Four groups of 20 extracted teeth were prepared as follows: I, a 3-4 mm root-end resection perpendicular to the long axis of the root, with a size 40 sonic Retro-prep tip creating an apical cavity 3 mm into root canal system; II, a 45 degrees bevel of the root-face removing a 3-4 mm root segment and root-end preparation as per group I; III, root-end resection as per group I, with an apical cavity prepared using a size 010 inverted cone bur 3 mm down the long axis of the root; IV, resection as per group II, followed by an apical cavity preparation with a size 010 inverted cone bur 3 mm into the root canal system. The apical root portion and root-end cavities were replicated and prepared for SEM analysis at x 20 and x 80 magnification. The degree of chipping associated with the margin of the root-end cavities, as evaluated with a standard grading system, and the incidence of root-face cracks were noted. Marginal chipping of root-end cavities prepared using sonic instrumentation was significantly worse than that produced by burs (P < 0.001). Perpendicular root-end resections showed significantly better scores than bevelled root-end resections (P < 0.005). The incidence of root-face cracking was low with no significant difference between the experimental groups.  相似文献

In this study a new epoxy-resin-based endodontic sealer, AH Plus, was tested in vitro for apical leakage. The conventional sealer AH26, for which information is already available, was used as the control. The root canals of 72 single-rooted teeth were prepared biomechanically using a stepback technique before lateral condensation of gutta-percha with one of the two sealers. Teeth were immersed in 5% methylene blue dye for 2, 4 or 10 days. The roots were split longitudinally so that the extent of dye penetration could be measured with a stereomicroscope. The mean extent at 2 days was 0.4 mm for AH26 and 1.4 mm for AH Plus. Neither material produced a complete apical seal and leakage increased with the duration of immersion in dye. The differences between observation periods as well as between materials with respect to dye penetration were statistically significant ANOVA, (P < 0.005).  相似文献

The purpose of this in vitro study was to compare both apical and coronal dye penetration when Ketac-Endo and AH-26 sealers were used with laterally condensed gutta percha. Crowns were removed from 28 teeth and the root canals were biomechanically prepared. The teeth were divided into two groups of 12-teeth each and a control group of 4 teeth. Root canals in the two experimental groups were filled with laterally condensed gutta percha and either Ketac-Endo or AH-26 sealer. The Ketac-Endo group had the coronal 3 mm of gutta percha and sealer removed and the resultant cavity was filled with Ketac-Endo alone. After the sealers had set, the root surfaces were coated with nail varnish except at the apex and at the coronal end. Positive controls had no root fillings and were coated with nail varnish in the same manner while the negative controls were sealed apically and coronally with Cavit prior to sealing the entire external root surface with nail varnish. Specimens were placed in 2% methylene blue dye in a vacuum of 660 mm of mercury for five minutes and then left immersed for a further two days. The roots were vertically sectioned to determine the following mean levels of dye penetration: Ketac-Endo, 1.08 mm apically and 6.29 mm coronally; AH-26, 0.75 mm apically and 6.67 mm coronally. Positive controls had total leakage and negative controls had no leakage. This study demonstrated that the apical and coronal seals obtained with Ketac-Endo and AH-26 were not significantly different although the apical seal obtained with each material was significantly better than the corresponding coronal seal.  相似文献

This study analyzed the relationship in orthodontically treated adults between upper central incisor displacement measured on lateral cephalograms and apical root resorption measured on anterior periapical x-ray films. A multiple linear regression examined incisor displacements in four directions (retraction, advancement, intrusion, and extrusion) as independent variables, attempting to account for observed differences in the dependent variable, resorption. Mean apical resorption was 1.36 mm (sd +/- 1.46, n = 73). Mean horizontal displacement of the apex was -0.83 mm (sd +/- 1.74, n = 67); mean vertical displacement was 0.19 mm (sd +/- 1.48, n = 67). The regression coefficients for the intercept and for retraction were highly significant; those for extrusion, intrusion, and advancement were not. At the 95% confidence level, an average of 0.99 mm (se = +/- 0.34) of resorption was implied in the absence of root displacement and an average of 0.49 mm (se = +/- 0.14) of resorption was implied per millimeter of retraction. R2 for all four directional displacement variables (DDVs) taken together was only 0.20, which implied that only a relatively small portion of the observed apical resorption could be accounted for by tooth displacement alone. In a secondary set of univariate analyses, the associations between apical resorption and each of 14 additional treatment-related variables were examined. Only Gender, Elapsed Time, and Total Apical Displacement displayed statistically significant associations with apical resorption. Additional multiple regressions were then performed in which the data for each of these three statistically significant variables were considered separately, with the data for the four directional displacement variables. The addition of information on Elapsed Time or Total Apical Displacement did not explain a significant additional portion of the variability in apical resorption. On the other hand, the addition of information on Gender to the information on the four directional displacement variables yielded an R2 value of 0.35, which indicated that these variables taken together could account for approximately a third of the observed variability in apical resorption in this sample.  相似文献

Previous studies of the bonding capabilities of glass-ionomer cements have concentrated on the use of in vitro testing conditions. Since early moisture contamination appears to have adverse effects on the physical properties of glass-ionomer cements, and with the probability of pulpally derived dentinal fluid being present under in vivo conditions, the objective of this study was to compare in vivo tensile bond strength with in vitro tensile bond strength of a glass-ionomer cement to dentin utilizing the same teeth under similar test conditions. A glass-ionomer lining cement was placed on freshly exposed labial dentin of the maxillary incisor on 10 Rhesus monkeys. Immediately following placement, an orthodontic button was placed over the cement and left undisturbed for 1 hour. The teeth were then extracted and stored in 100% relative humidity for 23 hours. An Instron testing machine was used to register in kilograms the force required to cause tensile bond failure of the cement. Identical methodology was then used on the same teeth for in vitro testing. The concluding results indicate that a statistically significant difference (P < or = 0.05) exists between in vivo and in vitro tensile bond strengths of the glass-ionomer lining cement and that the bond failure was cohesive in character for all cases both in vivo and in vitro. These findings suggest that clinically, tensile bond strengths of glass-ionomer cements to cut dentin can be expected to be weaker in vital teeth than in devital teeth.  相似文献

PURPOSE: To investigate the effects of acetone or water based Gluma primers on bonding efficacy when applied to acid-etched wet, dry or rewetted dentin. MATERIALS AND METHODS: Shear bond strength (SBS, 24 hours) was measured on human dentin etched with 20% phosphoric acid gel and rinsed with water in the wet (blot dried), the conventional (2 seconds air blast), the dry (10 seconds air drying), and the rewetted stage (10 seconds air drying, remoistening and blot drying). The priming solution was either the original water based or an acetone based experimental Gluma primer. Cavity sealing performance and depth of resin impregnation were determined by solvent and by technique along the margins of 4.5 mm wide cylindrical dentin cavities with 90 degree cavosurface angle. RESULTS: The SBSs of the wet technique groups were approximately 18 MPa, irrespective of the primer solvent. Conventional drying gave an SBS almost as high (16.5 MPa). Rewetting was highly effective with acetone as the primer solvent. The dry techniques resulted in moderate (water) and poor (acetone) bond strengths. The cavity margins of acetone based primer treated specimens were consistently gap free with the wet technique. The other seven groups showed between two and six gaps in each group of six specimens. The resin impregnated layer (RIL) thicknesses were 12.5 microns approximately in the wet groups, and thus identical with the total depth of demineralization resulting from the 30-second etching. The alternative techniques showed significantly thinner RILs particularly in the dry groups (7.5 microns).  相似文献

Amalgambond Plus with a high-performance additive was evaluated for its ability to bond a resin composite or an amalgam alloy to deep dentin in primary teeth with nonretentive cavity preparations. The clinical performance of amalgam and resin composite mesio-occlusodistal restorations bonded with Amalgambond Plus was evaluated at 15 months and 2 and 3 years. There was no statistically significant difference in the retention, marginal adaptation, secondary caries, and post-operative sensitivity over the times of the evaluation or between amalgam and resin composite restorations. After 3 years, most of the teeth were extracted because it was their exfoliation time, and 29 restorations limited within buccal and lingual cusps were evaluated for marginal leakage. There were no significant differences in microleakage between amalgam and resin composite restorations lined with Amalgambond Plus. Amalgambond Plus has the potential for serving successfully as a cavity liner with either amalgam or resin composite restorations.  相似文献

STUDY DESIGN: This retrospective study evaluated the progression of deformity after posterior fusion by reviewing 63 consecutive patients with idiopathic scoliosis who were all in Risser sign 0 at the time of surgery. All patients were observed beyond the time of skeletal maturity. Average follow-up time was 9 years and 8 months (range, 5-16 years). OBJECTIVES: To investigate the risk factors for the crankshaft phenomenon after posterior fusion and to build a model for predicting the probability of curve progression until maturation of growth. SUMMARY OF BACKGROUND DATA: There remains considerable controversy concerning the incidence, risk factors, and necessity of combined anterior fusion to prevent the crankshaft phenomenon in patients who are skeletally immature. METHODS: Serial radiographs were measured for Cobb angle, apical rotation according to Perdriolle, and apical rib-vertebra angle of Mehta. Multivariate and univariate logistic regression analysis was performed using seven potential predictors as independent variables and Cobb angle progression and rotational progression as dependent variables. RESULTS: Average progression of deformity was 3 degrees Cobb angle (range, -8-16 degrees) and 3 degrees Perdriolle rotation (range, -9-17 degrees). Progression of deformity more than 5 degrees of either Cobb angle or rotation was observed in 22 (35%) of 63 curves with 7 (11%) of 63 curves greater than 10 degrees. Chronologic age and skeletal age were found to be significantly associated with progression of deformity in univariate analysis. In multivariate analysis, only skeletal age seemed to be independently prognostic. The authors tried to build the logistic model using the three factors of chronologic age, skeletal age, and apical rib-vertebra angle. This model correctly classified 81% of all patients as progressive or nonprogressive. The positive predictive value was 90%. CONCLUSIONS: The results showed that patients with chronologic age of 11 years of younger, especially those with a skeletal age of 10 years or younger, had a high estimated probability of progression of deformity. The progression was fairly moderate, however, with an average Cobb angle of 9 degrees and average rotation of 7 degrees, which neither the patients nor the surgeon believed was of such magnitude as to warrant routine combined anterior fusion.  相似文献

The purpose of this study was to compare the ultrasonic and hand methods of sealer placement using three types of sealers: Sultan "Grossman's formula," AH-26, and CRCS. The effect of these methods on radiographic density, sealer distribution, and on the apical seal of the different sealers was evaluated. The in vitro experiment was carried out using human extracted mandibular incisors and mesio-buccal roots of maxillary first and second molars. Following standardized canal instrumentation, 120 single canal mandibular incisors were randomly assigned to six groups, depending on the type of sealer and method of sealer placement (hand versus ultrasonic). An additional ten teeth were used as controls. In the molar groups 40 canals were randomly assigned to two groups, with the method of sealer placement as the variable. Eight canals were used as controls. Postsealer analyses using radiographic density scores transformed to mm of sealer indicated that ultrasonics was superior to manual placement only for CRCS. Taken together, our investigation suggests that ultrasonics may enhance the placement of some sealers (i.e. CRCS) but not of others (i.e. AH-26 or Sultan). Method of sealer placement had no effect on apical leakage and therefore apparently on apical seal.  相似文献

Linear dye penetration was evaluated in teeth with open apices in which calcium phosphate cement was used as an apical barrier to facilitate obturation. The apical foramens of 42 extracted single-rooted human teeth were opened to a size 90 file. Half the teeth received apical barriers consisting of calcium phosphate cement (CPC) followed by obturation using a customized gutta-percha cone/ lateral condensation technique. The other half were obturated without benefit of apical barriers. Linear dye penetration was measured after 48 h exposure to India ink. The teeth receiving apical CPC barriers before obturation had significantly less dye penetration than teeth without apical barriers. Based on its proven biocompatibility and osteconductive potential, calcium phosphate cement may serve well as a replacement for calcium hydroxide in a single-visit immediate apical barrier apexification technique.  相似文献

The objective of this study was twofold: to evaluate the microleakage behavior of three dentin bonding systems and to determine if bovine teeth are comparable substrates to human teeth when studying the microleakage of various materials. The materials evaluated were Scotchbond Multi-Purpose Adhesive, Prisma Universal Bond 3, and All-Bond 2. All three bonding systems were used in combination with Prisma APH hybrid composite for comparison of microleakage behavior. Sixty class 5 preparations were cut at the cementoenamel junction for groups containing 30 human and 30 bovine teeth. A 1 mm 45 degree bevel was placed at the enamel margin. Teeth were grouped according to the dentin bonding system used and then restored according to the manufacturer's directions. After restoration, the teeth from each group were stored in distilled water at 37 degrees C for 3 days. The teeth were then thermocycled between 4 degrees C and 58 degrees C for 100 cycles and returned to distilled water at 37 degrees C for an additional 4 days. The teeth were then sealed with nail polish up to 1 mm from the margins of the restoration and placed in 45Ca isotope for 2 hours. The teeth were then sectioned and placed on x-ray film to produce autoradiographs. Microleakage was evaluated for the enamel and dentin margins separately using the following scale: 0 = no leakage, 1 = penetration of isotope to less than 1/2 the distance to the axial wall, 2 = penetration of isotope greater than 1/2 of the distance to the axial wall but short of the axial wall, and 3 = penetration of isotope to the axial wall or beyond. The materials were compared to each other using the Mann-Whitney U and Kruskal-Wallis tests. The gingival margins were compared to the incisal margins for all materials. No statistically significant differences in microleakage were revealed between the incisal and gingival location for human substrates, but there was statistically significant greater gingival microleakage for bovine substrates. All-Bond 2 leaked significantly more than Scotchbond Multi-Purpose for human substrates at the incisal margin. All-Bond 2 had significantly more microleakage than Prisma Universal Bond 3 at both dentin and enamel margins for the bovine substrate. There were no statistically significant differences in microleakage among the bonding systems for the human substrate. No statistically significant differences between the microleakage behavior of human and bovine substrates were found. These results support the use of bovine teeth for in vitro microleakage studies.  相似文献

An in vitro study was done using a fluid filtration device to compare microleakage of Temporary Endodontic Restorative Material (TERM) at various thicknesses. Forty-four extracted mandibular molars had their roots removed at the cementoenamel junction, occlusal table flattened, and a standardized endodontic access made in the crown. The teeth were restored with TERM in thicknesses of 1, 2, 3, and 4 mm, and quantitatively measured for leakage (microliters/min) at intervals of 1 and 24 h and 1, 3, and 5 wk. Thermocycling was accomplished at the 5-wk interval. Positive controls received 4-mm-thick IRM or gutta-percha restorations and were measured at 24 h only. Repeated measures of analysis of variance found no significant difference (p < 0.05) in leakage values in any group over time or between any of the different thicknesses. Positive controls showed significantly more leakage than all other groups. TERM maintained as tight a seal at 1-, 2-, and 3-mm thicknesses as it did at 4 mm and may be suited for clinical situations where less than 4 mm of restorative space exists.  相似文献

OBJECTIVE: This study evaluated the biocompatibility of a resin-modified glass-ionomer material on monkey pulps. METHOD AND MATERIALS: Standardized Class V cavities were prepared in 112 teeth distributed in six healthy adult monkeys. The resin-modified glass-ionomer cement was placed in 24 nonexposed and 36 exposed pulps according to manufacturer's instructions. ZOE was used as a control in nonexposed pulps, while calcium hydroxide was used as a control for exposed pulps. Tissues were collected at 6 to 7, 21 to 27, and 90 to 97 days. After demineralization, the teeth were serially sectioned, stained, and observed by light microscopy. RESULTS: Except for one resin-modified glass-ionomer pulp at 6 days, there were no differences between the responses of nonexposed pulps to resin-modified glass-ionomer specimens and ZOE controls. In exposed pulps, eight of 36 resin-modified glass-ionomer pulps showed various grades of inflammatory response, all associated with stained bacteria. Pulpal healing was similar in both resin-modified glass-ionomer and calcium hydroxide direct-capped exposures. Twenty-two of 26 exposed pulps restored with the resin-modified glass-ionomer cement showed dentin bridge formation at 21 and 97 days. CONCLUSION: The resin-modified glass-ionomer material exhibited acceptable biologic compatibility in exposed and nonexposed cavities.  相似文献
