Advocacy oral history: a research methodology for social activism in nursing

The reinstatement of social activism as a central feature of nursing practice has been advocated by nursing scholars and is consistent with contemporary conceptualizations of primary health care and health promotion that are rooted in critical social theory's concept of empowerment. Advocacy oral history from a feminist postmodern perspective offers a method of research that has the potential and purpose to empower participants to transform their political and social realities and may, therefore, be considered social activism. A recent study of public health nurses who had experienced significant distress through the reduction and redirection of their practice is provided as an exemplar of advocacy oral history. Philosophies underpinning the research method and characteristics of feminist postmodern research are reviewed and implications for the use of this methodology for social activism in nursing are drawn.     

Constructivism: a naturalistic methodology for nursing inquiry

This article will explore the philosophical underpinnings of the constructivist research paradigm. Despite its increasing popularity in evaluative health research studies there is limited recognition of constructivism in popular research texts. Lincoln and Guba's original approach to constructivist methodology is outlined and a detailed framework for nursing research is offered. Fundamental issues and concerns surrounding this methodology are debated and differences between method and methodology are highlighted.     

University-based psychiatric nursing education: a promise for the future?

University-based nursing education was introduced in Victoria, Australia, to redress the deficiencies attributed to the system of hospital-based training. The narrow and restrictive focus, maintenance of the subservient position of nurses, the employee status of students, an inadequate relationship between theory and practice and failure to keep pace with changes in the role of the nurse were the main deficiencies identified. This paper refers to a qualitative research study which examines the differences between graduates of a university-based and hospital-based psychiatric nursing program during the year following graduation. The findings suggest that the tertiary-based course had the potential to significantly redress some inadequacies of the hospital-based course to produce different qualities within its graduates.

Psychotherapy outcome research: An application of a new method for evaluating research methodology.

Reviewed 142 psychotherapy outcome studies published in 21 journals between 1978 and 1985, focusing primarily on psychotherapy as a treatment of emotional problems, but including articles regarding the use of psychotherapy as a treatment for other problems (e.g, marital discord). Of concern was whether the methodological rigor of published psychotherapy outcome studies had improved since 1978: Results fail to indicate significant overall improvement or deterioration. Community psychiatric nursing: promoting health in a community setting

With the development of care in the community, some mental health nurses have adopted approaches focused on health. This article explores one model of this approach which is aimed at secondary and tertiary health promotion in a community setting.     

Program evaluation in the public interest: a new research methodology

For every social welfare or social control service program there are several parties, each with different interests: patients, clients, staff, management, and sponsors. Evaluation of such a program in the public interest must take the interests of these parties into account. To do so requires an untraditional methodology, that of a second-person, or communal, science, which is not above the conflict of parties and their interests in specifying the variables, staffing the research, balancing considerations of intrusion against those of bias, considering the action implications of the data, sequentially staging the research, or even publishing findings. This all makes evaluation in the public interest a highly political process often unlikely to be logically decisive about intervariable relationships, to yield generalizable results, or even to be completed.     

Giving permission to embodied knowing to inform nursing research methodology: the poetics of voice(s)

This paper originated from my experience of trying to find an authentic way to research women's experience of the premenstrum. I describe how personal change informed an evolving methodological approach. This change occurred when I felt tension between two strong voices. Conflict and insecurities originated from the pressure of my academic voice to conform to the dominant culture in what often seemed a disempowering way; a way that denied my body voice by emphasizing theory over experience. The construction of a poetic of voice(s) to illustrate how I eventually found an embodied voice is used to provide space for the reader's own interpretive process. The evolving embodied voice integrates my experience as a woman/nurse/researcher, validating my knowing bodily, practically, experientially and academically that I was able to help the participants to disclose their embodied knowing. This approach has wider implications when considering the value nurse researchers and academics give to embodied wisdom.     

Hermeneutic philosophy. Part I: Implications of its use as methodology in interpretive nursing research

Increasingly, nurses use the philosophy of hermeneutics, especially Heideggerian and Gadamerian hermeneutics, to inform interpretive research. However, application of the work of these philosophers to interpretive nursing research has proved problematic as it fails to recognise, or act upon, obligations inherent in their work. Through a review of hermeneutically informed nursing research, methodological implications regarding the use of hermeneutic philosophy are examined in relation to: (i) the need to address forestructures and pre-understandings; (ii) checking interpretations with research participants; (iii) seeking objectivity, consensus and accuracy in textual interpretation; (iv) evaluating interpretations; and (v) gaining entry into the hermeneutic circle. This review suggests that despite obligations inherent in Heideggerian and Gadamerian hermeneutic philosophy, nurses fail to actualise these obligations in their research. In following Heidegger, nurses need to make the scientific theme secure by working out their forestructures in terms of the phenomenon under investigation. Those following Gadamer need to accept that interpretation seeks to manifest textual meanings rather than individual ones. And, whether following Heidegger or Gadamer, the way to make manifest textual meanings is to engage with the text within the hermeneutic circle by continuing to address forestructures and pre-understandings and by considering their influences upon the research.

Tumor necrosis factor-alpha (TNF-alpha) is thought to be an important mediator in the pathogenesis of neonatal septicemia. To assess the role of TNF-alpha in neonatal sepsis, serum levels of TNF-alpha were measured in a group of neonates with septicemia and compared with the levels of gestational-postnatal, age-matched healthy controls. The relationships between severity of infection, the nature of causative microorganisms, and TNF-alpha levels were also investigated in this prospective study. A total of 49 infants (25 full-term, 24 preterm) with proven sepsis and 40 healthy infants (20 full-term, 20 preterm) were included. Serum TNF-alpha levels were measured using the TNF-alpha immunoradiometric assay. The median level of TNF-alpha was found to be significantly higher in infants suffering from sepsis (154 pg/mL) particularly in those with septic shock (242.5 pg/mL) as compared to healthy controls (61.5 pg/mL) (p < 0.001). No correlation was found between TNF-alpha and postnatal ages, gestational ages or birth weights of the infants. TNF-alpha levels were not different in surviving and terminal neonates. Although serum, TNF-alpha levels were found to be slightly higher in gram-negative septicemia, the difference was not significant. These findings suggest that TNF-alpha plays an important role in the pathophysiology of neonatal septicemia, but its importance as a prognostic factor is not yet clear.
