Selective antagonists to the Type 3 serotonin receptor (5HT3) in combination with corticosteroids are now considered the standard of care for the prevention of emesis from moderately to highly emetogenic chemotherapy. Here we address issues of optimal dose, schedule and route of administration of four currently available selectable 5HT3 antagonists. This paper utilizes an evidence based medicine approach to the literature regarding this class of drugs, emphasizing the results large, randomized, controlled trials to make formal recommendations concerning optimal use of this important new class of anti-emetic agents. We conclude that for each drug there is a plateau in therapeutic efficacy at a definable dose level above which further dose escalation does not improve outcome. Furthermore, a single dose is as effective as multiple doses or continuous infusion, and finally, emerging data demonstrate that the oral route is equally efficacious as the intravenous route of administration, even with highly emetogenic chemotherapy. 相似文献

The reactivity of AlN powder with water in supernatants obtained from centrifuged Si3 N4 and SiC slurries was studied by monitoring the pH versus time. Various Si3 N4 and SiC powders were used, which were fabricated by different production routes and had surfaces oxidized to different degrees. The reactivity of the AlN powder in the supernatants was found to depend strongly on the concentration of dissolved silica in these slurries relative to the surface area of the AlN powder in the slurry. The hydrolysis of AlN did not occur if the concentration of dissolved silica, with respect to the AlN powder surface, was high enough (1 mg SiO2 /(m2 AlN powder)) to form a layer of aluminosilicates on the AlN powder surface. This assumption was verified by measuring the pH of more concentrated (31 vol%) Si3 N4 and SiC suspensions also including 5 wt% of AlN powder (with respect to the solids). 相似文献

The N-oxide group of the cellulose solvent N-methylmorpholine-N-oxide showed a strong preference for the axial position compared with N-methyl as determined by NMR experiments and computational studies. In solvents with negligible solvent-solute interaction, about 95% of the NMMO molecules obtained a typical chair conformation with an axial N-O (1) while 5% had an equatorial N-O (2) at room temperature (25 °C). Other conformations (boat and twist) are energetically largely disfavored. N-Benzylmorpholine-N-oxide was prepared as reference compound possessing 100% axial N-O. Aprotic solvents of increasing polarity slightly shifted the conformation equilibrium towards the more polar conformer 2. The effect of protic solvents, capable of forming H-bonds, was more pronounced, with water increasing the percentage of 2 to 25% of the total population. Addition of sugar model compounds reversed this effect, so that formation of 2 was suppressed and exclusively 1 was found, indicating a strong interaction between the latter conformer and the carbohydrate. 相似文献

The A peptide is a major beta-amyloid species in the human brain, forming toxic aggregates related to Alzheimer’s Disease. It also strongly chelates Cu(II) at the N-terminal Phe-Arg-His ATCUN motif, as demonstrated in A and A model peptides. The resulting complex resists ROS generation and exchange processes and may help protect synapses from copper-related oxidative damage. Structural characterization of Cu(II)A complexes by NMR would help elucidate their biological function, but is precluded by Cu(II) paramagneticism. Instead we used an isostructural diamagnetic Pd(II)-A complex as a model. To avoid a kinetic trapping of Pd(II) in an inappropriate transient structure, we designed an appropriate pH-dependent synthetic procedure for ATCUN Pd(II)A, controlled by CD, fluorescence and ESI-MS. Its assignments and structure at pH 6.5 were obtained by TOCSY, NOESY, ROESY, H-C HSQC and H-N HSQC NMR experiments, for natural abundance C and N isotopes, aided by corresponding experiments for Pd(II)-Phe-Arg-His. The square-planar Pd(II)-ATCUN coordination was confirmed, with the rest of the peptide mostly unstructured. The diffusion rates of A, Pd(II)-A and their mixture determined using PGSE-NMR experiment suggested that the Pd(II) complex forms a supramolecular assembly with the apopeptide. These results confirm that Pd(II) substitution enables NMR studies of structural aspects of Cu(II)-A complexes. 相似文献

We studied the morphological properties of precipitated, nanostructured, boehmite coatings on a polished alumina surface by exploiting the aluminum nitride (AlN) powder hydrolysis at elevated temperatures. The hydrolysis tests of the 3 wt% AlN powder suspensions in the temperature range 50–90 °C were performed in order to estimate the time needed for the synthesis of the coatings. They consisted of interlocked boehmite lamellas, positioned perpendicularly to the ceramic surface, and they exhibited a strong temperature-dependent size and surface density. The aim of this research was a quantitative assessment of the as-formed boehmite coatings. Based on electron microscopy micrographs, a stereometric analysis of the as-prepared coatings was performed in order to estimate the relevant geometric parameters of the lamellas. This work deals with the preparation of ceramic phases similar to those encountered in natural biocomposites through relatively fast and low-cost aqueous routes and various simple reactants and additives such as urea, commercial gelatin and hexamethyldiamine. In addition to the crystallographic (or amorphous) character of targeted phases (calcite, vaterite, aragonite, silica and silicates) particle morphology is also of interest and among others, we have obtained fractions of particles in the form of nanofibrilar calcite networks, calcium silicate doughnuts and Gordian knots, and diatom-like perforated silica cylinders. 相似文献

The purpose of this study was to evaluate the effect of Y-TZP ceramic surface functionalization with a nano-structured alumina coating on bond strength of the resin modified glass ionomer dental cement. A total of 160 disc-shaped specimens were produced and randomly divided into two groups of 80. Half of the discs in each group received an alumina coating which was fabricated by exploiting the hydrolysis of aluminum nitride (AlN) powder. The shear bond strengths of the resin-modified glass ionomer cement FUJI+ (GC Japan) and the composite resin luting agent RelyX Unicem (3M ESPE, USA) were then studied for the coated and uncoated surfaces The SEM analyses revealed that the application of an alumina coating to the Y-TZP ceramics created a highly retentive surface for bonding. The bond strengths for the coated groups in both cements were significantly higher than the uncoated groups. 相似文献

We studied the influence of magnetic fields (MFs) and simulated solar radiation (SSR) on ornithine decarboxylase (ODC) and polyamines in mouse epidermis. Chronic exposure to combined MF and SSR did not cause persistent effects on ODC activity or polyamines compared to the animals exposed only to UV, although the same MF treatment was previously found to accelerate skin tumor development. In an acute 24-h experiment, an elevation of putrescine and down-regulation of ODC activity was observed in the animals exposed to a 100-microT MF. No effect was seen 24 h after a single 2-MED (minimal erythemal dose) exposure to SSR. The results indicate that acute exposure to 50 Hz MF does exert distinctive biological effects on epidermal polyamine synthesis. 相似文献

The eukaryotic 20 S proteasome is a barrel-shaped protease complex, made up of four seven-membered rings. The outer and inner rings contain seven different alpha and beta-type subunits, respectively, each subunit located at a defined position. Recently, we have reported that the recombinant human alpha-type subunit C8 (HsC8) assembles into a heptameric ring-like structure by itself. In the present study we show that the two naturally neighboring alpha-type subunits of HsC8, HsPROS30 and HsPROS27, do not form ring-like complexes by themselves, but only dimers. This indicates that the propensity to form homo-oligomeric rings is not a general feature among human alpha-type subunits. However, coexpression of HsC8 and either of these neighbor alpha-type subunits results in the formation of hetero-oligomeric ring complexes, resembling the HsC8 ring-like structure. The ratio between the two types of subunits in the mixed complexes is surprisingly heterogeneous, varying from very high to very low HsC8 content. The three tested alpha-type subunits thus apparently lack binding sites that selectively interact with a specific neighboring subunit. This suggests that the correct positioning of the different alpha-type subunits in the eukaryotic 20 S proteasome is not dictated by the alpha-type subunits themselves, but rather by the interaction with specific beta-type subunits. 相似文献

Thyroid hemiagenesis (THA) is an inborn absence of one thyroid lobe of largely unknown etiopathogenesis. The aim of the study was to reveal genetic factors responsible for thyroid maldevelopment in two siblings with THA. None of the family members presented with congenital heart defect. The samples were subjected to whole-exome sequencing (WES) (Illumina, TruSeq Exome Enrichment Kit, San Diego, CA 92121, USA). An ultra-rare variant c.839C>T (p.Pro280Leu) in NKX2-5 gene () was identified in both affected children and an unaffected father. In the mother, the variant was not present. This variant is reported in population databases with 0.0000655 MAF (GnomAD v3, dbSNP rs761596254). The affected amino acid position is moderately conserved (positive scores in PhyloP: 1.364 and phastCons: 0.398). Functional prediction algorithms showed deleterious impact (dbNSFP v4.1, FATHMM, SIFT) or benign (CADD, PolyPhen-2, Mutation Assessor). According to ACMG criteria, variant is classified as having uncertain clinical significance. For the first time, NKX2-5 gene variants were found in two siblings with THA, providing evidence for its potential contribution to the pathogenesis of this type of thyroid dysgenesis. The presence of the variant in an unaffected parent, carrier of p.Pro280Leu variant, suggests potential contribution of yet unidentified additional factors determining the final penetrance and expression.
