This article describes 2 important practice considerations affirmed in the U.S. Supreme Court's Sell v. United States (2002) decision: (a) the importance of providing least restrictive services prior to interventions that violate patients' liberty interests, and (b) contextual and environmental factors may be considered in clinical determinations of dangerousness. The psychological treatment of behavior disorders fall within the purview of least restrictive or intrusive interventions compared with the involuntary administration of psychoactive medications. To legitimately comply with the least restrictive criterion, the provision of psychological services is essential. This long-held criterion is rarely acknowledged today as providers use restricted service arrays and attempt to resolve complex and co-occurring behavior problems with medications and restraints. Less restrictive psychological interventions are required for effective treatment of challenging behaviors. A 2nd significant implication lies in the court's affirmation that it is legitimate to consider contextual factors such as history and current environmental conditions in determining dangerousness. The antitumor effect and mechanisms activated by murine IL-12 and IL-18, cytokines that induce IFN-gamma production, were studied using engineered SCK murine mammary carcinoma cells. In syngeneic A/J mice, SCK cells expressing mIL-12 or mIL-18 were less tumorigenic and formed tumors more slowly than control cells. Neither SCK.12 nor SCK.18 cells protected significantly against tumorigenesis by distant SCK cells. However, inoculation of the two cell types together synergistically protected 70% of mice from concurrently injected distant SCK cells and 30% of mice from SCK cells established 3 d earlier. Antibody neutralization studies revealed that the antitumor effects of secreted mIL-12 and mIL-18 required IFN-gamma. Interestingly, half the survivors of SCK.12 and/or SCK.18 cells developed protective immunity suggesting that anti-SCK immunity is unlikely to be responsible for protection. Instead, angiogenesis inhibition, assayed by Matrigel implants, appeared to be a property of both SCK.12 and SCK.18 cells and the two cell types together produced significantly greater systemic inhibition of angiogenesis. This suggests that inhibition of tumor angiogenesis is an important part of the systemic antitumor effect produced by mIL-12 and mIL-18. 相似文献

BB-10010 is a variant of the human form of macrophage inflammatory protein-1alpha (MIP-1alpha), which has been shown in mice to block the entry of hematopoietic stem cells into S-phase and to increase their self-renewal capacity during recovery from cytotoxic damage. Its use may constitute a novel approach for protecting the quality of the stem cell population and its capacity to regenerate after periods of cytotoxic treatment. Thirty patients with locally advanced or metastatic breast cancer were entered into the first randomized, parallel group controlled phase II study. This was designed to evaluate the potential myeloprotective effects of a 7-day regimen of BB-10010 administered to patients receiving six cycles of 5-fluorouracil (5-FU), adriamycin, and cyclophosphamide (FAC) chemotherapy. Patients were randomized, 10 receiving 100 microgram/kg BB-10010, 11 receiving 30 microgram/kg BB-10010, and nine control patients receiving no BB-10010. BB-10010 was well-tolerated in all patients with no severe adverse events related to the drug. Episodes of febrile neutropenia complicated only 4% of the treatment cycles and there was no difference in incidence between the treated and nontreated groups. Studies to assess the generation of progenitor cells in long-term bone marrow cultures were performed immediately preceding chemotherapy and at the end of six dosing cycles in 18 patients. Circulating neutrophils, platelets, CD 34(+) cells, and granulocyte/macrophage colony-forming cell (GM-CFC) levels were determined at serial time points in cycles 1, 3, and 6. The results showed similar hemoglobin and platelet kinetics in all three groups. On completion of the six treatment cycles, the average pretreatment neutrophil levels were reduced from 5.3 to 1.7 x 10(9)/L in the control patients and from 4.3 to 1.9 and 4.5 to 2.5 x 10(9)/L in the 30/100 microgram/kg BB-10010 groups, respectively. Relative to their pretreatment values, 50% of the patients receiving BB-10010 completed the treatment with neutrophil values significantly higher than any of the controls (P = .02). Mobilization of GM-CFC was enhanced by BB-10010 with an additional fivefold increase over that generated by chemotherapy alone, giving a maximal 25-fold increase over pretreatment values. Bone marrow progenitor assays before and after this standard regimen of chemotherapy indicated little long-term cumulative impairment to recovery from chemotherapy. Despite the limited cumulative damage to the bone marrow, which may have minimized the protective value of BB-10010 during this regimen of chemotherapy, better recovery of neutrophils in the later treatment cycles with BB-10010 was indicated in a number of patients. 相似文献

Through the use of semicustom integrated circuit technology, an implantable muscle stimulator has been developed. The unit is small, lightweight, has low power consumption, and is intended for permanent usage. The stimulator circuitry is externally controlled and powered by a single encoded radio frequency carrier. Up to eight independently controlled stimulus output channels are provided, with output channel selection, stimulus pulse width, and stimulus pulse frequency under external control. A constant current biphasic stimulus pulse is used, in which the stimulus current amplitude can be preset by a single resistor value. The stimulator circuitry has been implemented in thick film hybrid form, and has undergone laboratory evaluation for 48 months. 相似文献

An international intercomparison of horn gain and polarization measurements at X-band has previously been completed. There were seven participating laboratories with the National Institute of Standards and Technology serving as the pilot laboratory. Two X-band pyramidal standard gain horns with a nominal gain of 22 dB served as the traveling standards. Quantities measured included on-axis fixed frequency gain at 8, 10, and 12 GHz, swept frequency gain between 8-12 GHz and polarization characteristics at the three fixed frequencies. All laboratories performed the fixed frequency-gain measurements. The swept-frequency and polarization measurements were optional, with four laboratories performing swept-frequency measurements and three laboratories measuring polarization. The device degradation under ac and dc stress have been discussed and a relationship between the two has been established,. We have shown that the commonly used lifetime criteria of 10% linear current degradation for 10 years for a transistor under dc stress is overly conservative for representing the circuit operating lifetime. Using experimental and simulated data for inverter chains, we proposed that a meaningful equivalent lifetime based on 10% Idl degradation under dc stress is 1 year lifetime (for a 10 year circuit lifetime based on 54b degradation in ring oscillator frequency). We also compared this criteria to actual circuit degradation for microprocessors and a DRAM. For DSP microprocessors with 0.8 μm LDD transistors, the projected lifetime was more than 200 years at 5.5 V, with a corresponding 10% I dr lifetime of 20 years. For 1 Mb DRAMs with 1 pm LDD transistors, the 5% speed degradation lifetime at 5.5 V was more than 100 years, whereas the individual transistors had 10% Idl lifetime of 4 years. These circuit results support the 10% Idl transistor lifetime. An equivalent circuit theory design of a class of rectangular waveguide E-plane T-junction diplexer with E-plane all-metal insert filters is presented. Element values of equivalent circuit models of E-plane T-junction proposed in [1] are computed and approximated. The parameters of the filters designed by [2] are also given. By use of the network combining technique, the scattering matrix of the diplexer is obtained and optimized according to the diplexer specification with a novel objective function. Optical telemetry has long been an option for transcutaneous data transfer and has been used in various types of implanted systems. This telemetry modality and the efficiency of these optical links are becoming ever more important as higher bandwidth sources such as cortical recording arrays are being implemented in implanted systems. The design of the transmitter-skin-receiver interface (the "optical interface") is paramount to the operation of a transcutaneous optical telemetry link. This interface functions to achieve sufficient receiver signal power for data communication. This paper describes a mathematical analysis and supporting data that quantitatively describes the relationship between the primary interface design parameters. These parameters include the thickness of the skin through which the light is transmitted, the size of the integration area of the optics, the degree of transmitter-receiver misalignment, the efficiency of the optics system, and the emitter power. The particular combination of these parameters chosen for the hardware device will determine the receiver signal power and, therefore, the data quality for the link. This paper demonstrates some of the tradeoffs involved in the selection of these design parameters and provides suggestions for link design.
