We report a case of sleep apnea syndrome (SAS) with nocturnal pulmonary hypertension (NPH) in a 71-year-old man suffering from dyspnea during sleep. Severe snoring at night and daytime sleepiness were noticed before admission by his wife. Nocturnal oxygen desaturation (NOD) was documented with a pulse oximeter and severe sleep apnea syndrome was diagnosed on the basis of results of respiratory inductive plethysmography, an apnea index (AI) > 20, minimum SpO2 56%. NPH was diagnosed by Swan-Ganz catheter. The levels of NPH were severe. Elevation of systolic pulmonary arterial pressure (PAP) above 40 mmHg was observed 137 episodes at night. Both NPH and NOD were improved by 1 L/min of nasal oxygen therapy. A number of episodes of systolic PAP above 40 mmHg with oxygen therapy was 55 episodes. Peak mean PAP was 36 mmHg in room air vs 33 mmHg in oxygen therapy. Minimum SpO2 with oxygen therapy was improved to 69%. Total time of SpO2 < 90% at night was 153 minutes in room air vs 37 minutes in oxygen therapy. In this case, NPH and NOD due to severe SAS were remarkably improved by oxygen therapy.  相似文献

The finite element method is used to analyze open three-dimensional transmission line structures in the quasi-TEM regime. Starting from the general solution of the Laplace equation in spherical coordinates, a set of asymptotic boundary conditions are derived for three-dimensional quasi-static problems for a spherical outer boundary. The second-order boundary condition is generalized to a box-shaped outer boundary and implemented in the finite element method to solve the potential problem of a rectangular microstrip patch. Numerical results show that the asymptotic boundary conditions yield more accurate results than those obtainable with a perfectly conducting shield placed at the same location  相似文献

We have developed a low-loss bidirectional optical coupler for high-speed optical communication with plastic optical fibers (POFs). The coupler, which is fabricated by an injection molding method that uses poly (methyl methacrylate), has an antisymmetric tapered shape. We show that the coupler has low insertion and branching losses. The tapered shape of the receiving branch reduces beam diameter and increases detection efficiency coupling to a photodetector, whose area is smaller than that of the plastic optical fiber. The possibility of more than 15-m bidirectional transmission with a signaling bit rate up to 500 Mbits/s for simplex step-index POFs is demonstrated.  相似文献

The purpose of the present study was to examine the relevancy of the high throughput solubility assay and permeability assay to the biopharmaceutics classification system (BCS). Solubility and permeability were measured by high throughput solubility assay (HTSA) and parallel artificial membrane permeation assay (PAMPA), respectively. High throughput solubility assay was performed using simulated gastric fluid (SGF, pH 1.2) and simulated intestinal fluid without bile acid (SIF, pH 6.8). We categorize 18 drugs based on the BCS using HTSA and PAMPA. Fourteen out of 18 drugs were correctly classified (78% success rate). The result of the present study showed that HTSA could predict BCS class with a high success rate, and PAMPA could also be useful to predict the permeation of drugs.  相似文献

This paper presents a nine‐switch inverter that can drive two AC motors independently. Recently, as an inverter that can drive them independently, a five‐leg inverter has been proposed. Ten switching devices are used for the five‐leg inverter, whereas only nine are used for the nine‐switch inverter. So, the nine‐switch inverter has the merit that one switching device can be reduced compared to the five‐leg inverter. Moreover, the maximum output voltage of the nine‐switch inverter equals that of the five‐leg inverter. In this work, we propose the structure of the nine‐switch inverter and a modulation method for the same, and validate the nine‐switch inverter by showing simulation results. The possibility of nanoparticle (NP) uptake to the human central nervous system is a major concern. Recent reports showed that in animal models, nanoparticles (NPs) passed through the blood–brain barrier (BBB). For the safe use of NPs, it is imperative to evaluate the permeability of NPs through the BBB. Here we used a commercially available in vitro BBB model to evaluate the permeability of NPs for a rapid, easy and reproducible assay. The model is reconstructed by culturing both primary rat brain endothelial cells and pericytes to support the tight junctions of endothelial cells. We used the permeability coefficient (Papp) to determine the permeability of NPs. The size dependency results, using fluorescent silica NPs (30, 100, and 400 nm), revealed that the Papp for the 30 nm NPs was higher than those of the larger silica. The surface charge dependency results using Qdots® (amino-, carboxyl-, and PEGylated-Qdots), showed that more amino-Qdots passed through the model than the other Qdots. Usage of serum-containing buffer in the model resulted in an overall reduction of permeability. In conclusion, although additional developments are desired to elucidate the NPs transportation, we showed that the BBB model could be useful as a tool to test the permeability of nanoparticles.  相似文献

It has been reported that hyperpnea-induced bronchoconstriction in guinea pigs is a potential model for exercise-induced asthma in humans. We hypothesized that calcitonin gene-related peptide (CGRP) could modulate leukotriene D4 (LTD4)-induced responses and be involved in the pathophysiology in this asthma model. We measured tracheal (Ptr) and alveolar pressure (PA) using alveolar capsules in open-chested, mechanically ventilated (f = 1 Hz, VT = 9 ml/kg, PEEP = 4 cm H2O) guinea pigs. Animals were intravenously pretreated with saline (SAL), CGRP(8-37) (CGRP receptor antagonist), CGRP, MK-571 (LTD4 receptor antagonist), MK-886 (5-lipoxygenase inhibitor), or CGRP(8-37) + MK-571, and then underwent dry gas hyperpnea challenge (HC, 95% 02-5% CO2, 150 breaths/min, 7 min). We calculated resistance of lung (RL), tissue (Rti), and airway (Raw). HC increased RL, Rti, and Raw in SAL controls (322 +/- 27, 430 +/- 59, 299 +/- 23% baseline, respectively). MK-571, MK-886, and CGRP significantly reduced the responses to HC, while CGRP(8-37) enhanced HC-induced responses. Pretreatment with CGRP(8-37) and MK-571 in combination attenuated HC-induced constriction. In addition, pretreatment with CGRP reduced responses induced by intravenous administration of LTD4. These observations suggest that CGRP might be involved in the pathophysiology of hyperpnea-induced constriction in guinea pigs via modulation of LTD4-elicited responses.  相似文献

This paper presents a technique based on the full-wave, two-dimensional spectral domain method and the distributed transmission line approach to analyze structures with a combination of conductor width and dielectric height variations. The technique is applied to a regular taper and the results are compared with existing models. A matched taper transition is then designed based on simultaneous variation of the conductor width and dielectric height such that their ratio is kept constant. The analysis of such tapers is carried out and compared to measured data. It is shown that significant reduction in the return loss can be achieved with such transitions over fairly wide, tunable frequency ranges  相似文献
