Recurrence is a common sequela of Clostridium difficile-associated diarrhea (CDD) and may increase morbidity, costs, and treatment-related antimicrobial resistance. Because recurrent CDD (RCDD) frequently occurs very soon after an initial episode, our goal was to determine the risk factors for early RCDD (occurring < or = 45 days after the initial episode). We conducted a case-control study, comparing 13 patients with early RCDD (case patients) with 46 patients who had only one CDD episode (control patients) at Centre Hospitalier Angrignon (Québec) during January 1993 through November 1994. Risk factors for early RCDD included a history of chronic renal insufficiency, a white blood cell count of > or = 15 x 10(3)/mm3, and community-acquired diarrhea with the first CDD episode. For seven of eight case patients, C. difficile strains from the first and second CDD episodes were identical, suggesting that relapse is more common than reinfection. These results suggest that treatments should be directed at preventing relapses in patients at high risk for early RCDD. 相似文献
Artificial joint replacement is becoming increasingly important in orthopedics. Several hundreds of thousands of operations, especially of the hip joint, have already been performed. The design of such joints depends largely on how accurately they can be modeled analytically such that their load transmission characteristics can be determined. One of the most critical areas of investigation is the interface existing between bone and implant in the orthopedic prostheses applications. This investigation examines the effect of varying modulus between bone and cement (PMMA) on the failure of the joint. An energy criterion is used rather than the stress criteria normally applied in the open literature.
The results of the present study show that the modulus variation within the interface can have a significant influence on the stress and energy fields in the region near a material interface. It is found that if the interface modulus has a gradual variation, the predicted stress fields resemble those with a single material interface model which has a higher (stiffer) average modulus. The interface modulus variation and average modulus are shown to have a significant effect on the predicted location and onset of failure which is of primary interest. The present modeling scheme is intended to demonstrate some of the effects which might locally be found near the bone and cement interface in a prosthetic joint. 相似文献
Many security and surveillance tasks involve either finding an object in a cluttered scene or discriminating between like objects. For example, an observer might look for a person of known height and weight in a crowd, or he might want to positively identify a specific face. The paper "Modeling target acquisition tasks associated with security and surveillance" [Appl. Opt. 46, 4209 (2007)] describes a specific-object model used to predict the probability of accomplishing this type of task. The U.S. Army Night Vision and Electronic Sensors Directorate (NVESD) and the U.S. Army Research Laboratory have developed a terahertz (THz) -band imaging system performance model for detection and identification of concealed weaponry. The MATLAB-based model accounts for the effects of all critical sensor and display components and for the effects of atmospheric attenuation, concealment material attenuation, and active illumination. The model is based on recent U.S. Army NVESD sensor performance modeling technology that couples system design parameters to observer-sensor field performance by using the acquire methodology for weapon identification performance predictions. This THz model has been developed in support of the Defense Advanced Research Project Agencies' Terahertz Imaging Focal-Plane Technology (TIFT) program and is currently being used to guide the design and development of a 0.650 THz active-passive imaging system. This paper will describe the THz model in detail, provide and discuss initial modeling results for a prototype THz imaging system, and outline plans to calibrate and validate the model through human perception testing. 相似文献
Thin-layer chromatography (TLC), which is widely used for separation of glycolipids, oligosaccharides, lipids, and compounds of environmental and pharmaceutical interest, can be readily coupled to matrix-assisted laser desorption/ionization (MALDI) time-of-flight mass spectrometers, but this arrangement usually compromises mass spectral resolution due to the irregularity of the TLC surface. However, TLC can be coupled to an external ion source MALDI-Fourier transform (FT) MS instrument without compromising mass accuracy and resolution of the spectra. Furthermore, when the FTMS has a vibrationally cooled MALDI ion source, fragile glycolipids can be desorbed from TLC plates without fragmentation, even to the point that desorption of intact molecules from "hot"matrixes such as alpha-cyano-4-hydroxycinnamic acid is possible. In this work, whole brain gangliosides are separated using TLC; the TLC plates are attached directly to the MALDI target, where the gangliosides are desorbed, ionized, and detected in the FTMS with >70 000 resolving power. 相似文献
Common variants of the angiotensin-converting enzyme (ACE) gene (ACE ie humans, Ace in mice) associated with changes in circulating ACE activities have been suggested to confer differential risks for atherosclerosis. Using a mouse model of atherosclerosis induced by heterozygosity for apolipoprotein E gene disruption and an atherogenic diet, we have studied the impact on atherogenesis of a mutation that changes the level of function of Ace. We find that this genetically determined change does not influence the size or complexity of atherosclerotic lesions. Ace genotype was not a significant determinant of lesion size in female (+/+ = 12.9 +/- 1.5 and +/- = 11.7 +/- 1.6 microns2 x 10(4)) or male (+/+ = 0.95 +/- 0.25 and +/- = 1.83 +/- 0.59 microns2 x 10(4)) mice; however, lesions were significantly larger (P < .001) in female than male mice. Ace genotype also did not affect lesion complexity; however, lesions in females showed significantly increased frequency of cholesterol clefts, acellular cores, fibrous caps, and calcifications compared with those in males. The hypothesis that genetic variation in the level of ACE gene expression affects the development of atherosclerosis is not supported by these findings. 相似文献
Accumulative damage model based on the hysteresis strain energy density is proposed for predicting fatigue crack growth. Investigated is the application of sinusoidal loading on an edge crack whose growth rates are obtained by specifying the number of cycles, Δ, for each growth step. The corresponding increment of crack growth, Δ, is calculated by having the accumulated local strain energy density to reach certain critical value, . As it is to be expected, each growth increment Δ increases up to the point of unstable rapid fracture. The growth rate versus a data are generated from the nonlinear incremental theory of plasticity. Because of the complexities involved in the stress and subcritical crack growth analysis, the finite element procedure is adopted such that the grid pattern is readjusted for each step of crack growth. Results for the edge crack specimen are displayed graphically and compared with those for the center cracked specimen made of the same material. The different growth characteristics are discussed and expected because material damage by fatigue is sensitive to changes in load history, specimen geometry and crack configuration. Insight into these nonlinear effects provides a means for establishing the range of applicability of the linear fatigue growth models. Discussed in particular are the vs δ and AS relations where the linear theory of elasticity is used to calculate Δ and Δ. 相似文献
The Airborne Laser Infrared Absorption Spectrometer II (ALIAS-II) is a lightweight, high-resolution (0.0003-cm(-1)), scanning, mid-infrared absorption spectrometer based on cooled (80 K) lead-salt tunable diode laser sources. It is designed to make in situ measurements in the lower and middle stratosphere on either a balloon platform or high-altitude remotely piloted aircraft. Chemical species that can be measured precisely include long-lived tracers N(2)O and CH(4), the shorter-lived tracer CO, and chemically active species HCl and NO(2). Advances in electronic instrumentation developed for ALIAS-I, with the experience of more than 250 flights on board NASA's ER-2 aircraft, have been implemented in ALIAS-II. The two-channel spectrometer features an open cradle, multipass absorption cell to ensure minimal contamination from inlet and surfaces. Time resolution of the instrument is =3 s, allowing rapid in situ measurements with excellent spatial resolution. ALIAS-II has completed successful balloon flights from New Mexico, Alaska, and Brazil providing CH(4) and N(2)O vertical profiles in the tropics, mid-latitudes, and high northern latitudes up to altitudes of 32 km. 相似文献
Many factors govern conditioning effectiveness, including the intertrial interval (ITI) used during training. The present study systematically varied the training ITI during both trace and long-delay fear conditioning. Rats were trained using one of six different ITIs and subsequently tested for conditioning to the white noise conditioned stimulus (CS) and the training context. After trace conditioning, percent freezing to the CS was positively correlated with training ITI, whereas percent freezing to the context was negatively correlated with training ITI. In contrast, when rats were trained using a long-delay paradigm, freezing during the CS test session did not vary as a function of training ITI; rats exhibited robust freezing at all ITIs. The long-delay conditioned rats exhibited relatively low levels of freezing during the context test. Thus, trace is more sensitive than long-delay fear conditioning to variations in the training ITI. These data suggest that training ITI is an important variable to consider when evaluating age or treatment effects, where the optimal ITI may vary with advancing age or pharmacological treatment.
