In order to compare the effects of using helium–oxygen and air in assisted breathing and inhalation therapies, flow and particle deposition results were obtained in a realistic model of human oral extrathoracic (ET) airways using computational fluid dynamics (CFD) and pressure loss measurements. As the main deposition mechanism for pharmaceutical aerosols in the ET is inertial impaction, the ET model was reconstructed from medical images to take into account the complexity of realistic morphological features. Calculations were performed with the CFD software Fluent®, and pressure losses were measured on a cast based on a stereolithographic fabrication of the model. Results show that ET pressure loss and particle deposition are lower with helium–oxygen as compared to air. Moreover, further simulations were performed with various particle sizes and inspiratory flow rates, which indicate that particle deposition in the ET depends on both the Stokes and Reynolds number.  相似文献

Nickel was introduced in ZSM-5 zeolite by two different methods: dry impregnation and ionic exchange. Different loadings of metal, ranging from 0.4 to 6 wt% were explored. These materials were thoroughly characterized by EXAFS, TPR, acidity measurements by H/D isotope exchange and ethane hydrogenolysis. Regardless of the metal introduction method, at 0.4 wt% Ni, the Ni-ZSM-5 catalysts present only nickel located inside the zeolite channels as compensation cations. In contrast, an increase to 1 wt% nickel (via impregnation) led to its presence both inside and outside the channels.The catalytic activity of these Ni-ZSM-5 zeolites was tested in n-hexane cracking. Depending on the way the metal was introduced, it was possible to modify the n-hexane cracking activity and the selectivity toward light alkenes. Hence, a proper design of metal and acid sites could be achieved.  相似文献

The ability of dendrimer 2G‐[Si{O(CH2)2N(Me)2+(CH2)2NMe3+(I?)2}]8 (NN16) to transfect a wide range of cell types, as well as the possible biomedical application in direct or indirect inhibition of HIV replication, was investigated. Cells implicated in HIV infection such as primary peripheral blood mononuclear cells (PBMC) and immortalized suspension cells (lymphocytes), primary macrophages and dendritic cells, and immortalized adherent cells (astrocytes and trophoblasts) were analyzed. Dendrimer toxicity was evaluated by mitochondrial activity, cell membrane rupture, release of lactate dehydrogenase, erythrocyte hemolysis, and the effect on global gene expression profiles using whole‐genome human microarrays. Cellular uptake of genetic material was determined using flow cytometry and confocal microscopy. Transfection efficiency and gene knockdown was investigated using dendrimer‐delivered antisense oligonucleotides and small interfering RNA (siRNA). Very little cytotoxicity was detected in a variety of cells relevant to HIV infection and erythrocytes after NN16 dendrimer treatment. Imaging of cellular uptake showed high transfection efficiency of genetic material in all cells tested. Interestingly, NN16 further enhanced the reduction of HIV protein 24 antigen release by antisense oligonucleotides due to improved transfection efficiency. Finally, the dendrimer complexed with siRNA exhibited therapeutic potential by specifically inhibiting cyclooxygenase‐2 gene expression in HIV‐infected nervous system cells. NN16 dendrimers demonstrated the ability to transfect genetic material into a vast array of cells relevant to HIV pathology, combining high efficacy with low toxicity. These results suggest that NN16 dendrimers have the potential to be used as a versatile non‐viral vector for gene therapy against HIV infection.  相似文献

The Driver-Pressures-State-Impact-Response approach is applied to heavy metals in the Seine River catchment (65,000 km(2); 14 million people of which 10 million are aggregated within Paris megacity; 30% of French industrial and agricultural production). The contamination pattern at river mouth is established on the particulate material at different time scales: 1930-2000 for floodplain cores, 1980-2003 for suspended particulate matter (SPM) and bed-sediments, 1994-2003 for atmospheric fallout and annual flood deposits. The Seine has been among the most contaminated catchments with maximum contents recorded at 130 mg kg(-1) for Cd, 24 for Hg, 558 for Pb, 1620 for Zn, 347 for Cu, 275 for Cr and 150 for Ni. Today, the average levels for Cd (1.8 mg kg(-1)), Hg (1.08), Pb (108), Zn (370), Cu (99), Cr (123) and Ni (31) are much lower but still in the upper 90% of the global scale distribution (Cr and Ni excepted) and well above the natural background values determined on pre-historical deposits. All metal contents have decreased at least since 1955/65, well before metal emission regulations that started in the mid 1970's and the metal monitoring in the catchment that started in the early 1980's. In the last 20 y, major criteria changes for the management of contaminated particulates (treated urban sludge, agricultural soils, dredged sediments) have occurred. In the mid 1990's, there was a complete shift in the contamination assessment scales, from sediment management and water usage criteria to the good ecological state, now required by the 2000 European Directive. When comparing excess metal outputs, associated to river SPM, to the average metal demand within the catchment from 1950 to 2000, the leakage ratios decrease exponentially from 1950 to 2000 for Cd, Cr, Cu, Pb and Zn, meanwhile, a general increase of the demand is observed: the rate of recycling and/or treatment of metals within the anthroposphere has been improved ten-fold. Hg environmental trajectory is very specific: there is a marked decontamination from 1970 to 2000, but the leakage ratio remains very high (10 to 20%) during this period. Drivers and Pressures are poorly known prior to 1985; State evolution since 1935 has been reconstructed from flood plain cores analysis; Impacts were maximum between 1950 and 1970 but remained unknown due to analytical limitation and lack of awareness. Natural organic matter (NOM) in drinking water supplies can provide precursors for disinfectant byproducts, molecules that impact taste and odors, compounds that influence the efficacy of treatment, and other compounds that are a source of energy and carbon for the regrowth of microorganisms during distribution. NOM, measured as dissolved organic carbon (DOC), was monitored daily in the White River and the Indiana-American water treatment plant over 22 months. Other parameters were either measured daily (UV-absorbance, alkalinity, color, temperature) or continuously (turbidity, pH, and discharge) and used with stepwise linear regressions to predict DOC concentrations. The predictive models were validated with monthly samples of the river water and treatment plant effluent taken over a 2-year period after the daily monitoring had ended. Biodegradable DOC (BDOC) concentrations were measured in the river water and plant effluent twice monthly for 18 months. The BDOC measurements, along with measurements of humic and carbohydrate constituents within the DOC and BDOC pools, revealed that carbohydrates were the organic fraction with the highest percent removal during treatment, followed by BDOC, humic substances, and refractory DOC.  相似文献

BACKGROUND: Telemetry monitoring is widely used in hospitals; the importance of being able to monitor and examine dysrhythmias has been universally accepted. Yet it is often used for patients who do not actually require this technology. A model to improve the efficiency of telemetry use entailed the use of an advanced practice nurse (APN; identical to a nurse practitioner) to provide concurrent review and intervention of floating telemetry, which is available for patients independently of the floor location and who do not need an intensive care unit bed. ADDRESSING OVERUSE: The demand for floating telemetry at Hackensack University Medical Center had equaled or exceeded the telemetry availability virtually 100% of the time, even after local guidelines had been disseminated in 1998. The APN carried out concurrent monitoring and intervened with the attending physician when patients were on telemetry for longer than 48 hours and did not meet the local telemetry guidelines. RESULTS: The mean number (standard error [SE]) of hours per patient declined from 65.2 +/- 0.7 hours (95% confidence interval, 63.8 to 66.6 hours) for the 11 months before the intervention to a mean of 49.6 +/- 0.4 hours (95% confidence interval, 48.7 to 50.2 hours) for the 29 months after intervention--representing a decrease of 34% (p < 0.0001). This decrease led to an increase in the number of patients per month put on telemetry. DISCUSSION: The APN model, an aggressive approach that induced change almost immediately, was then applied to other quality improvement projects.  相似文献

Examined reviewer recommendations and subsequent editorial decisions for 153 papers submitted for publication. In an earlier article (D. W. Fiske and L. Fogg; see record 1990-23769-001), the authors demonstrated the diversity of reviewer comments and the relative uniqueness of each review. In the current study, overall reviewer recommendation to accept or reject an article predicted 93% of the 153 editorial decisions correctly. Reviewers evaluated papers on the basis of planning and execution flaws or general negative comments, with little weight given to interpretations or results.
