Restenosis after coronary angioplasty occurs in at least 30% of patients in the first six months and, as yet, there is no known treatment to decrease this event. We tested a monoclonal antibody Fab fragment (c7E3) directed against the platelet glycoprotein IIb/IIIa integrin, the receptor mediating the final common pathway of platelet aggregation, to see whether it reduced the frequency of clinical restenosis. Patients who had unstable angina, recent or evolving myocardial infarction, or high-risk angiographic morphology, were randomised to receive c7E3 bolus and a 12 hour infusion of c7E3 (708 patients), c7E3 bolus and placebo infusion (695 patients), or placebo bolus and placebo infusion (696 patients). With maintenance of the double-blind state, patients were followed-up for at least 6 months to determine the need for repeat angioplasty or surgical coronary revascularisation and the occurrence of ischaemic events. By 30 days, 12.8% of placebo bolus/placebo infusion patients had had a major ischaemic event (death, myocardial infarction, urgent revascularisation), compared with 8.3% of c7E3 bolus/c7E3 infusion patients, yielding a 4.5% difference (35% reduction, p = 0.008). At 6 months, the absolute difference in patients with major ischaemic event or elective revascularisation was 8.1% between placebo bolus/placebo infusion and c7E3 bolus/c7E3 infusion patients (35.1% vs 27.0%; 23% reduction p = 0.001). The favourable long-term effect was mainly due to less need for bypass surgery or repeat angioplasty in patients with an initial successful procedure, since need for repeat target vessel revascularisation was 26% less for c7E3 bolus/c7E3 infusion than for placebo treatment (16.5% vs 22.3%; p = 0.007). The c7E3 bolus/placebo infusion group had an intermediate outcome which was not significantly better than that of the placebo bolus/placebo infusion group. These results extend the benefit of c7E3 bolus/c7E3 infusion from reducing abrupt closure and acute-phase adverse outcomes to a diminished need for subsequent coronary revascularisation procedures. Because this therapy carries a risk of bleeding complications and has been studied only in high-risk angioplasty patients, further evaluation is needed before it can be applied to other patient groups.  相似文献

In arteries, muscarinic agonists such as acetylcholine release an unidentified, endothelium-derived hyperpolarizing factor (EDHF) which is neither prostacyclin nor nitric oxide. Here we show that EDHF-induced hyperpolarization of smooth muscle and relaxation of small resistance arteries are inhibited by ouabain plus Ba2+; ouabain is a blocker of Na+/K+ ATPase and Ba2+ blocks inwardly rectifying K+ channels. Small increases in the amount of extracellular K+ mimic these effects of EDHF in a ouabain- and Ba2+-sensitive, but endothelium-independent, manner. Acetylcholine hyperpolarizes endothelial cells and increases the K+ concentration in the myoendothelial space; these effects are abolished by charbdotoxin plus apamin. Hyperpolarization of smooth muscle by EDHF is also abolished by this toxin combination, but these toxins do not affect the hyperpolarizaiton of smooth muscle by added K+. These data show that EDHF is K+ that effluxes through charybdotoxin- and apamin-sensitive K+ channels on endothelial cells. The resulting increase in myoendothelial K+ concentration hyperpolarizes and relaxes adjacent smooth-muscle cells by activating Ba2+-sensitive K+ channels and Na+/K+ ATPase. These results show that fluctuations in K+ levels originating within the blood vessel itself are important in regulating mammalian blood pressure and flow.  相似文献

The study was undertaken to determine if the internal carotid artery peak systolic velocities (ICA PSVs) measured by two colour Doppler imaging systems (Acuson 128 and Siemens Quantum) agree sufficiently for the two systems to be interchangeable in evaluating carotid artery disease. One operator obtained blinded measurements of ICA PSV in 63 prospective nonrandomized patients at risk of stroke. The operator examined 20 patients in the first cohort to assess the intraobserver variation, and 43 patients in the second cohort to assess the limits of agreement between the systems. In vitro comparisons of the systems were also undertaken, using both string and flow phantoms. Excluding one outlier, the intraobserver reproducibility coefficient for both machines was 0.48 m s-1. The limits of agreement (within which 95% of differences lie) between systems were -0.47 to 0.45 m s-1. This reduced to -0.39 to 0.33 m s-1 when the one outlier was excluded. This is within the intraobserver reproducibility range. In vitro data show little intersystem variation with phantom velocity. Intratransducer differences increase when the Doppler angle is increased using the string phantom; maximum differences: Acuson 0.30 m s-1 (42%) and Siemens 0.32 m s-1 (32%). These are within the in vivo reproducibility range. Intratransducer difference decreases when the Doppler angle is increased using the flow phantom, maximum differences: Acuson 0.05 m s-1 and Siemens 0.07 m s-1. The results show the systems agree sufficiently to be interchangeable in evaluating carotid artery disease; however, errors in maximum PSVs, caused by operator or system variation, may lead to errors in percent stenosis grading of the carotid arteries.  相似文献

The mechanical structure and control system architecture for pneumomechanical single-axis modular units are described. The units were designed as subsystems to allow the construction of robots with kinematics suitable for a wide range of workhandling tasks. The controls also demonstrate modularity with regard to both software and hardware. Chronic nicotine administration in animal models evokes a dose-dependent increase in brain nicotinic receptor numbers. Genetically determined variability in nicotinic receptor number in different mouse strains has also been reported, which is thought to affect sensitivity to nicotine, as well as the development of tolerance. Humans self-administer nicotine principally in the form of cigarettes and other tobacco products. The present study compared [3H]nicotine binding in human postmortem brain from thalamus and hippocampus of nonsmoking subjects, subjects who had variable life-long smoking histories and subjects who had quit smoking. A significant increase was seen in [3H]nicotine binding in both hippocampus and thalamus of subjects with life-long smoking histories. In the hippocampus, this change resulted from a change in total receptor number (Bmax), with no change in receptor affinity (Kd). There was also a positive correlation between the degree of smoking, as measured by the average reported packs smoked per day, and the number of nicotine binding sites found in both the hippocampus and thalamus, showing that humans exhibit a dose-dependent increase in brain nicotinic receptor binding. Receptor levels in these brain regions after smoking cessation were at or below those found in the control population, which indicated that smoking-induced changes are reversible after cessation of nicotine treatment. These results suggest that increases in nicotinic receptor levels in the human brain may underlie nicotine tolerance and addiction in smokers.  相似文献

Under the auspices of its Dioxin Characterization Program, members of The Vinyl Institute (VI), have analyzed for potential polychlorinated dibenzodioxin/furan (PCDD/F) concentrations in polyvinylchloride (PVC) resins, treated wastewater effluent and ethylene dichloride (EDC) product at EDC, vinyl chloride monomer (VCM) and PVC manufacturing facilities in the U.S. and Canada. No 2,3,7,8-tetrachlorodibenzodioxin (TCDD) was detected in any sample analyzed under the program to date. Trace concentrations (low pg/g) of PCDD/F were detected in only a few samples of PVC resins and EDC product. Treated wastewater contained low ppq concentrations of PCDD/F. All concentrations are expressed as Toxic Equivalents (TEQ). Extrapolation of these data shows that the contribution of EDC/VCM/PVC manufacturing via these media constitutes substantially less than 1 percent of the estimated annual U.S. dioxin releases to the environment.  相似文献

The development of a low-power 12-channel multiplexer-demultiplexer pair that is clocked at the standard synchronous optical network (SONET) rate of 622.08 MHz is discussed. Each device has been integrated in silicon using a 0.75- mu m NMOS VLSI technology that provides high fabrication yield at relatively low cost. Highlighted are the analog interface circuits of the two chips. These include a phase splitter and amplifier for the maser clock input, a precision 50- Omega output driver for high-speed synchronous-transport-signal-12 (STS-12) data, as well as input amplifier and an output stage for low-speed differential STS-1 data.<>  相似文献

Sphagnum mosses mediate long-term carbon accumulation in peatlands. Given their functional role as keystone species, it is important to consider their responses to ecological gradients and environmental changes through the production of phenolics. We compared the extent to which Sphagnum phenolic production was dependent on species, microhabitats and season, and how surrounding dwarf shrubs responded to Sphagnum phenolics. We evaluated the phenolic profiles of aqueous extracts of Sphagnum fallax and Sphagnum magellanicum over a 6-month period in two microhabitats (wet lawns versus dry hummocks) in a French peatland. Phenolic profiles of water-soluble extracts were measured by UHPLC-QTOF-MS. Andromeda polifolia mycorrhizal colonization was quantified by assessing the intensity of global root cortex colonization. Phenolic profiles of both Sphagnum mosses were species-, season- and microhabitat- dependant. Sphagnum-derived acids were the phenolics mostly recovered; relative quantities were 2.5-fold higher in S. fallax than in S. magellanicum. Microtopography and vascular plant cover strongly influenced phenolic profiles, especially for minor metabolites present in low abundance. Higher mycorrhizal colonization of A. polifolia was found in lawns as compared to hummocks. Mycorrhizal abundance, in contrast to environmental parameters, was correlated with production of minor phenolics in S. fallax. Our results highlight the close interaction between mycorrhizae such as those colonizing A. polifolia and the release of Sphagnum phenolic metabolites and suggest that Sphagnum-derived acids and minor phenolics play different roles in this interaction. This work provides new insight into the ecological role of Sphagnum phenolics by proposing a strong association with mycorrhizal colonization of shrubs.  相似文献

Large-scale relational learning becomes crucial for handling the huge amounts of structured data generated daily in many application domains ranging from computational biology or information retrieval, to natural language processing. In this paper, we present a new neural network architecture designed to embed multi-relational graphs into a flexible continuous vector space in which the original data is kept and enhanced. The network is trained to encode the semantics of these graphs in order to assign high probabilities to plausible components. We empirically show that it reaches competitive performance in link prediction on standard datasets from the literature as well as on data from a real-world knowledge base (WordNet). In addition, we present how our method can be applied to perform word-sense disambiguation in a context of open-text semantic parsing, where the goal is to learn to assign a structured meaning representation to almost any sentence of free text, demonstrating that it can scale up to tens of thousands of nodes and thousands of types of relation.  相似文献

Experiments were conducted under controlled conditions to investigate the apparent allelopathic effects of sudex [Sorghum bicolor (L.) Moench ×Sorghum Sudanese (P.) Stapf, cv. FFR 201] on weed and vegetable species. Allelopathic potential, as measured by radicle elongation of herbaceous indicator species, decreased with increasing sudex age. Greatest potential allelopathic activity of sudex shoot tissue was observed when sudex was collected at 7 days of age. Small-seeded broadleaf species were more inhibited in the presence of sudex shoot tissue than were grass species. Two major phytoinhibitors were isolated from aqueous extracts of sudex shoot material by partitioning with diethyl ether, followed by thin-layer and liquid column chromatography. Phytoinhibitors were identified asp-hydroxybenzoic acid andp-hydroxybenzaldehyde, potentially the enzymatic breakdown products of the cyanogenic glycoside dhurrin. The I50 values of these compounds using a cress (Lepidium sativum L.) seed bioassay were 140 and 113 g/ml for the acid and aldehyde, respectively. Sudex tissue collected at 7 days of age possessed a greater percentage of these phytoinhibitors on a per gram basis than did older sudex tissue.
