An improved inductive transducer is achieved by placing the coil and the majority of the transducer body into a recessed alumina basecoat. Process advantages of a recessed over planar structure includes lower top pole topography, improved pole trim capability, and improved top pole composition uniformity. Coil and photoresist processing in a recessed cavity allows for multiple layers with lower resistance without increasing transducer Permalloy path length. Recessed trimmed design performance has increased efficiency with higher amplitude and a narrower pulse width along with improved overwrite compared to planar untrimmed transducers. OBJECTIVES: (1) To determine whether and how outcome measurements in the ECASS trial are influenced by a shorter time window (0-3 vs. 3-6 h) between onset of symptoms and start of thrombolytic therapy using recombinant tissue plasminogen activator (rt-PA) in acute ischemic stroke. (2) To discuss the results of the ECASS 0- to 3-hour cohort with the results of the National Institute of Neurological Disorders and Stroke Study (NINDSS). DESIGN AND ANALYSIS: Analysis of the 0- to 3-hour and the 3- to 6-hour cohort in accordance with the ECASS protocol. Comparative analysis of the ECASS and NINDSS results following the NINDSS protocol using dichotomized endpoints. MAIN OUTCOME MEASURES: Primary endpoints: modified Rankin Scale, Barthel Index; secondary endpoints: combined Barthel/Rankin, long-term Scandinavian Stroke Scale, National Institutes of Health Stroke Scale, mortality at 30 and 90 days, occurrence of intracranial hemorrhage. NINDS trial endpoint: favorable outcome as defined in the NINDS trial. RESULTS: In ECASS, 87 patients were randomized within 3 h of stroke onset. Differences in favor of rt-PA treatment can be found for all primary and secondary outcome measures in the ECASS 0- to 3-hour cohort, except for mortality at day 30, which is somewhat higher in the rt-pA-treated group. However, due to the small sample size, the differences do not reach statistical significance. Early infarct signs (as defined by the ECASS protocol) are found as early as 2 h after stroke onset. Parenchymal hemorrhages are found significantly more often among rt-PA-treated patients. The results in the ECASS 0- to 3-hour cohort fit well with the results in NINDSS. CONCLUSION: Data from the 3-hour ECASS cohort support the efficacy of early thrombolytic therapy in acute hemispheric stroke patients. Comparing bleeding complications between the ECASS and NINDSS is difficult because of differences in the definition and occurrence of hemorrhagic events. 相似文献

Hyperglycaemia is a common complication of stress and prematurity in extremely low-birth-weight infants. Model-based insulin therapy protocols have the ability to safely improve glycaemic control for this group. Estimating non-insulin-mediated brain glucose uptake by the central nervous system in these models is typically done using population-based body weight models, which may not be ideal.
Method
A head circumference-based model that separately treats small-for-gestational-age (SGA) and appropriate-for-gestational-age (AGA) infants is compared to a body weight model in a retrospective analysis of 48 patients with a median birth weight of 750 g and median gestational age of 25 weeks. Estimated brain mass, model-based insulin sensitivity (SI) profiles, and projected glycaemic control outcomes are investigated. SGA infants (5) are also analyzed as a separate cohort.
Results
Across the entire cohort, estimated brain mass deviated by a median 10% between models, with a per-patient median difference in SI of 3.5%. For the SGA group, brain mass deviation was 42%, and per-patient SI deviation 13.7%. In virtual trials, 87–93% of recommended insulin rates were equal or slightly reduced (Δ < 0.16 mU/h) under the head circumference method, while glycaemic control outcomes showed little change.
Conclusion
The results suggest that body weight methods are not as accurate as head circumference methods. Head circumference-based estimates may offer improved modelling accuracy and a small reduction in insulin administration, particularly for SGA infants. 相似文献
Traditionally, standard Lagrangian-type finite elements, such as linear quads and triangles, have been the elements of choice
in the field of topology optimization. However, finite element meshes with these conventional elements exhibit the well-known
“checkerboard” pathology in the iterative solution of topology optimization problems. A feasible alternative to eliminate
such long-standing problem consists of using hexagonal (honeycomb) elements with Wachspress-type shape functions. The features
of the hexagonal mesh include two-node connections (i.e. two elements are either not connected or connected by two nodes),
and three edge-based symmetry lines per element. In contrast, quads can display one-node connections, which can lead to checkerboard;
and only have two edge-based symmetry lines. In addition, Wachspress rational shape functions satisfy the partition of unity
condition and lead to conforming finite element approximations. We explore the Wachspress-type hexagonal elements and present
their implementation using three approaches for topology optimization: element-based, continuous approximation of material
distribution, and minimum length-scale through projection functions. Examples are presented that demonstrate the advantages
of the proposed element in achieving checkerboard-free solutions and avoiding spurious fine-scale patterns from the design

Labour force participation of adolescents in Australia is growing at an unprecedented rate. This increased participation is coupled with a growing realisation of the vulnerability of adolescents in the labour market in terms of occupational injury. Despite recent evidence that time of day may be an important determinant of adolescent injuries, the impact of non-standard and night work on adolescent injury rates has received scant attention to date. The current study addresses this shortcoming by examining injury patterns of 3201 working adolescents in Queensland. Results revealed that female adolescents are 2.5 times more likely to sustain an injury on day shift and 4.71 times more likely to sustain an injury on night shift than their adult counterparts when total work hours are taken into consideration. Similar results were found for male adolescents with an injury to work hours ratio of 2.19 on day shift and 3.05 on night shift. These findings point to the value of considering the temporal pattern of adolescent work in future research aimed at minimising injuries at work and improving the work experience of tomorrow's workforce. 相似文献
If a trademark (“mark”) becomes a generic term, it may be cancelled under trademark law, a process known as genericide. Typically, in genericide cases, consumer surveys are brought into evidence to establish a mark’s semantic status as generic or distinctive. Some drawbacks of surveys are cost, delay, small sample size, lack of reproducibility, and observer bias. Today, however, much discourse involving marks is online. As a potential complement to consumer surveys, therefore, we explore an artificial intelligence approach based chiefly on word embeddings: mathematical models of meaning based on distributional semantics that can be trained on texts selected for jurisdictional and temporal relevance. After identifying two main factors in mark genericness, we first offer a simple screening metric based on the ngram frequency of uncapitalized variants of a mark. We then add two word embedding metrics: one addressing contextual similarity of uncapitalized variants, and one comparing the neighborhood density of marks and known generic terms in a category. For clarity and validation, we illustrate our metrics with examples of genericized, somewhat generic, and distinctive marks such as, respectively, DUMPSTER, DOBRO, and ROLEX.
