The current standard of care for advanced non-small-cell lung cancer is based on detecting actionable mutations that can benefit from targeted therapy. Comprehensive genetic tests can have long turn-around times, and because EGFR mutations are the most prevalent actionable mutation, a quick detection would enable a prompt initiation of targeted therapy. Furthermore, the scarcity of diagnostic material means that sometimes only cytologic material is available. The Idylla™ EGFR assay is a real-time PCR–based method able to detect 51 EGFR mutations in 2.5 h. Idylla is validated for use only on FFPE sections, but some researchers described their experiences with cytological material. We reviewed the relevant literature, finding four articles describing 471 cases and many types of cytological input material: smears, cell-block sections, suspensions, and extracted DNA. The sensitivity, specificity, and limit of detection appear comparable to those obtained with histological input material, with one exception: the usage of scraped stained smears as input may reduce the accuracy of the test. In conclusion, usage of cytological material as input to the Idylla EGFR test is possible. A workflow where common mutations are tested first and fast, leaving rarer mutations for subsequent comprehensive profiling, seems the most effective approach.  相似文献

Abstract

This study considers the decay heat removal through passive systems (i.e., natural convection) in the pool-type experimental reactor MYRRHA (SCK?CEN). The low velocities associated with natural convection may cause a degradation of the thermal mixing in the upper plenum of the reactor: buoyancy dominated phenomena, such as stratification, buoyant jets or convective cells may occur. The large temperature gradients associated with these phenomena may induce thermal loads on the structure, compromising its integrity. The objectives of this work are, in order, (1) achieving an understanding of the fluid mechanics leading to the formation of thermal gradients in the upper plenum and (2) mitigating the temperature gradients the upper plenum’s geometry. The methodology chosen to address the problem is experimental. A ''two-dimensional'' water facility modeling a slab of the reactor's primary loop was designed. Simultaneous measurements of velocity and temperature were performed in the model's buffer tank using thermocouples and innovative optical imaging techniques. It was observed that vertical thermal gradients develop in the tank because of the combined effect of stratification and advection. Important reductions of local gradients were observed when changing porosity and position of upper plenum’s buffer plates.  相似文献

This study deals with the ex-post evaluation of ten major transport projects which were co-financed by the European Regional Development Fund and the Cohesion Fund in the period 2000–2013 and located across nine EU Member States. The original contribution of this study is represented by the combination of the traditional ex-post Cost–Benefit approach with a qualitative analysis. Specifically, a retrospective Cost–Benefit analysis is used to quantify the benefits and costs of the projects, then interviews to relevant stakeholders were carried out. Ex-post CBA, when appropriately implemented and integrated with qualitative evidence, represents a powerful tool for supporting decision-making processes and for policy lessons.  相似文献

Caffeine is the most widely studied psychoactive molecule in history due to its many pharmacological activities and a high number of biological and physiological effects. In literature, there is a great number of applications that describe extraction, identification, and quantification of caffeine in foods and beverages. For this purpose, an extraction step is followed by an analytical technique for the identification and quantification of caffeine. This work proposes an innovative method in which sample preparation, separation, and detection steps are unified in a single step. To the best of our knowledge, this is the first report on the determination of caffeine in coffee, tea, and cocoa by means of an online extraction coupled to a liquid chromatographic system equipped with a photodiode array detector. The developed methodology was validated in terms of sensitivity, detection limits, accuracy, and precision. The advantages of this technique are (i) a significant reduction of analysis time (more than 70%) and of solvents used (the extraction step is integrated in the chromatographic analysis), (ii) the whole procedure is thus completely automated drastically reducing possible operator errors to occur, and (iii) easily realized by using a conventional monodimensional liquid chromatography system.  相似文献

Herein, we present poly(butylene 1,4-cyclohexanedicarboxylate) (PBCE) films characterized by an unpatterned microstructure and a specific hydrophobicity, capable of boosting a drastic cytoskeleton architecture remodeling, culminating with the neuronal-like differentiation of human bone marrow-mesenchymal stem cells (hBM-MSCs). We have used two different filming procedures to prepare the films, solvent casting (PBCE) and compression-moulding (PBCE*). PBCE film had a rough and porous surface with spherulite-like aggregations (Ø = 10–20 μm) and was characterized by a water contact angle = 100°. PBCE* showed a smooth and continuous surface without voids and visible spherulite-like aggregations and was more hydrophobic (WCA = 110°). Both surface characteristics were modulated through the copolymerization of different amounts of ether-oxygen-containing co-units into PBCE chemical structure. We showed that only the surface characteristics of PBCE-solvent-casted films steered hBM-MSCs toward a neuronal-like differentiation. hBM-MSCs lost their canonical mesenchymal morphology, acquired a neuronal polarized shape with a long cell protrusion (≥150 μm), expressed neuron-specific class III β-tubulin and microtubule-associated protein 2 neuronal markers, while nestin, a marker of uncommitted stem cells, was drastically silenced. These events were observed as early as 2-days after cell seeding. Of note, the phenomenon was totally absent on PBCE* film, as hBM-MSCs maintained the mesenchymal shape and behavior and did not express neuronal/glial markers.  相似文献

In this article we explore the role of innovation champions and the ways innovation is championed and promoted in five selected UK megaprojects. To date, this research area is surprisingly under-explored in the megaproject management literature. The study is based on thirty interviews with innovation champions to capture their own perceptions and narratives on the ways innovation is stimulated and promoted in megaprojects, combined with relevant textual materials. Innovation was commonly defined as a step change or best practice that creates value that could be financial, environmental, societal etc. The data demonstrate the role of CEOs and their collaboration with innovation champions and academic partners in introducing innovation into the vocabulary of megaprojects, developing and formalising innovation strategy, and transferring experiences across megaprojects. The data show the ways different communication channels are used to promote innovations by champions such as forums, portals, platforms, campaigns, events etc.  相似文献
