Proctological surgery in the one-day surgery regimen: the preliminary results with 232 patients

Simple anorectal surgery can be routinely employed on a one-day surgery (ODS) bases; however complications such as bleeding, urinary retention, and postoperative pain represent a limitation in this respect. In this paper we report preliminary results of our experience in surgery for haemorrhoids, anal fissures and fistulas, achieved in two years on 232 patients. Our protocol includes admission in the morning of the operation and preoperative evaluation by means of ECG, coagulation profile, assay of beta-HCG for female patients. The patients, prepared with a self-administered enema and perianal applications of prilocaine-lidocaine ointment, is taken in the operative room were a venous line is placed and an anaesthesiologist proceed to monitoring of ECG, blood pressure and oximetry. 211 patients were operated under locoregional anaesthesia performed by the surgeon by means of bilateral pudendal nerves blocking. Whereas the remaining underwent general or spinal anesthesia. With this approach we performed 106 haemorrhoidectomies, 96 sphincterotomies, 19 of which with posterior anoplasty and 30 fistulectomy or fistulotomy. 60 mg of ketorolac have been injected locally at the end of operation in order to improve postoperative pain control. Patients undergoing hemorrhoidectomy, anoplasty, fistulotomy or fistulectomy were discharged after 24 hours whereas those undergoing sphincterotomy went home the same day. We reported 4 early postoperative complications in the haemorrhoids group with an incidence of 1.7% (two bleedings, one urinary retention and one fever) treated conservatively. Postoperative pain resulted adequately controlled by a low dosage of NSAID (a mean of 3.7 doses of 30 mg ketorolac/patient). Our satisfactory results seem to suggest continuing the practice of one-day surgery in proctology.     

Multicultural trends and toys in play therapy.

Because culture plays an increasingly important role in play therapy, the purpose of this article is to review existing literature on multicultural issues in play therapy and to discuss current trends and toys in play therapy reported from a group of play therapists. A group of play therapists (N = 505) registered with the Association for Play Therapy were asked to respond to two open-ended questions: What trends, if any, do you see in play therapy with your culturally diverse clients? What items do you include in your playroom to specifically represent culturally diverse populations? Responses to these open-ended questions were analyzed using open and axial coping as part of the inductive process. Results of the first question were grouped into five main categories and responses to the second question were categorized based on area and type of play. Demonstration-based learning and control for automatic grasping

We present a method for automatic grasp generation based on object shape primitives in a Programming by Demonstration framework. The system first recognizes the grasp performed by a demonstrator as well as the object it is applied on and then generates a suitable grasping strategy on the robot. We start by presenting how to model and learn grasps and map them to robot hands. We continue by performing dynamic simulation of the grasp execution with a focus on grasping objects whose pose is not perfectly known.     

Characterization of macroporous ion-exchange resins by means of conventional and MAR-NMR spectra

By comparison with MAR-NMR spectra, the shape of the internal water band in conventional NMR spectra of macroporous cation-exchange resins is shown to be characteristic of resin structure. In resins composed of relatively large microparticles (order of microns) the internal water line may be broadened by kinetic process in the resin phase. In resins composed of very small microparticles (hundreds of Å), a considerable inter-particle heterogeneity is revealed and can be quantitatively characterized from NMR spectra.     

Wavelet transform based digital protection for transmission lines

This paper presents a high speed, computationally efficient scheme for protection of transmission lines. The relay logic consists of three parts: directional protection, fault classification and fault location. Wavelet transform is used for extracting information from the fault transients and only the first level high frequency details of the voltages and currents are used. Proposed protection logic compares the directional signals from both terminals to discriminate between faults inside and outside the zone of interest. Fault classification is achieved using local terminal current information. An estimate of the location of the faults is obtained utilizing single faulted phase current information from both terminals. The logic is deterministic and can work reliably in the presence of fault resistance, load variation and CT saturation. The validity of the proposed logic was exhaustively tested by simulating various types of faults on a four bus meshed system modeled in EMTP/ATP.     

Ultrafast integrable and reconfigurable XNOR, AND, NOR, and NOT photonic logic gate

A novel, simple, compact, and integrable scheme of reconfigurable and ultrafast photonic logic gate is demonstrated, based on a single semiconductor optical amplifier (SOA) and able to process ultrafast signals. XNOR function has been optically implemented exploiting four-wave mixing and cross-gain modulation in an SOA. The same scheme can be easily reconfigured to obtain AND, NOR, and NOT logic gates. Performances in terms of bit error rate for 20-ps return-to-zero signals at 10 Gb/s show a power penalty limited to 0.5 dB for all logic gates but the AND, which experiences regeneration (-2-dB power penalty) due to nonlinear SOA noise compression.     

Dynamics of MRI-Guided thermal ablation of VX2 tumor in paraspinal muscle of rabbits

This study combines fast magnetic resonance imaging (MRI) and model simulation of tissue thermal ablation for monitoring and predicting the dynamics of lesion size for tumor destruction. In vivo experiments were conducted using radiofrequency (RF) thermal ablation in paraspinal muscle of rabbit with a VX2 tumor. Before ablation, turbo-spin echo (TSE) images visualized the 3-D tumor (necrotic core and tumor periphery) and surrounding normal tissue. MR gradient-recalled echo (GRE) phase and magnitude images were acquired repeatedly in 3.3 s at 30-s intervals during and after thermal ablation to follow tissue temperature distribution dynamics and lesion development in tumor and surrounding normal tissue. Final lesion sizes estimated from GRE magnitude, post-ablation TSE, and stained histologic images were compared. Model simulations of temperature distribution and lesion development dynamics closely corresponded to the experimental data from MR images in tumor and normal tissue. The combined use of MR image monitoring and model simulation has the potential for improving pretreatment planning and real-time prediction of lesion-size dynamics for guidance of thermal ablation of tumors.

Comparison between plasma concentrations of testosterone, nitric oxide and endothelin 1-2 in penile and brachial venous blood: preliminary results in men with psychogenic impotence

In the present study venous plasma concentrations of testosterone (T), nitric oxide (NO) and endothelin 1-2 (ET1-2) in the flaccid penis and brachial blood were measured in men with psychogenic impotence. T and NO were significantly lower in the penile venous blood, while ET1-2 showed no statistical difference. These data support the hypothesis of testosterone dependence of penile nitric oxide synthesis (NOS).
