A ten year study of medullary carcinoma of the breast

In this large series of patients treated for medullary carcinoma of the breast by radical mastectomy, the over-all five year survival rate was 63.7 per cent and the ten year survival rate, 49.5 per cent. Although survival was adversely affected by axillary lymph node involvement, there was an equal incidence of such involvement, there was an equal incidence of such involvement in tumors less than 4 centimeters compared with those greater than 4 centimeters in size; however, patients with tumors greater than 4 centimeters in size fared poorer categorically than did those with smaller lesions. Women in the premenopausal period had one-third of the cancers and had a significantly better survival rate than did those in the postmenopausal period, despite a similar incidence of axillary lymph node involvement. Medullary carcinoma is among the small group of malignant tumors of the breast that have distinctly better five and ten year survival rates than other more common varieties.     

Reversal of pretreatment hypotension and control of hemorrhage in trauma patients by a simple device

The injection of a suspension of Walker 256 carcinoma cells into the carotid artery of rats produced a model of hematogenously spread cerebral metastases. Most animals died from massive extracerebral tumors of the head and jaw; brain tumors were present in only one-quarter. External carotid artery ligation prior to tumor inoculation did not increase the incidence of fatal brain tumor. When cyclophosphamide, 15 mg/kg, was injected as a single dose on the fourteenth day after tumor inoculation, most of the extracerebral tumor disappeared. Fifty percent of the animals so treated were cured of tumor, but 90% of the remainder died of large intracerebral tumors. Severe cytopathic changes resulting from cyclophosphamide were present in extracerebral or choroid plexus tumors but were mild or nonexistent in intracerebral tumors. These selective effects of cyclophosphamide suggest that water-soluble agents are less effective against tumor in the brain than against extracerebral tumors despite the fact that metastatic tumor breaks down the blood-brain barrier.     

Development of a new stabilized four-phase oscillator based on partial conservativity

The paper treats the development of a non-linearly stabilized oscillator model for generating four sine waves in quadrature. the treatment appears important although quadrature signals can also be generated by a well-known twophase oscillator. the latter generates only two of the signals needed in a four-phase system, but the rest of the phases signals could nevertheless be generated by simply inverting the two existing signals. For certain applications, however, the generation of all four signals in a completely cyclic and symmetrical manner is preferable. This method of generation is dealt with in the present paper. A previous paper of ours has treated a system of a similar objective. the present system, however, is much simpler. This simplicity is manifested not only through the mathematical representation but also when an electronic realization of the system is considered. the present model realization appears considerably less expensive, since the number of necessarily non-linear operations is much reduced. This reduction is due to the fact that the non-linearly stabilized model imbeds a ‘partially conservative’ linear oscillator. the concept of ‘partial conservativity’ has been introduced recently. the present work treats comprehensively the advantages that are gained by imbedding a partially conservative oscillator.     

Periodic limb movement in sleep in children with Williams syndrome

Difficulty differentiating physiologic genu varum from early Blount's disease persists. Drennan's metaphyseal-diaphyseal (MD) angle remains the most consistently valuable radiographic parameter despite measurement error. Clinical risk factors also should be considered. All patients receiving orthoses for genu varum since 1985 were reviewed. The focus of the study was those patients with an MD angle of >16 degrees or between 9 and 16 degrees with a clinical risk factor for progression. Risk factors considered were ligamentous instability, obesity, asymmetry, and being female, black, or Hispanic. Thirty-eight patients with 60 tibiae were included. The success rate was 90%. Risk factors for failure (six cases) were instability, obesity, and delayed bracing. In cases with MD angles >16 degrees, the success rate was 86%. The results of orthotic treatment, restricted to patients meeting the stated parameters, represent improvement on the reported natural history.

Another stabilized generator model for three phase sinewaves

A new three dimensional oscillator for generating three phases of sinewaves is derived. The model is similar to the previous ones (Ref. 1 and 2) in enabling an undistorted stabilized generation of sinewaves. It possesses certain advantages in comparison to the previous models. The oscillator appears as a viable model for representing synchronous generators in a simplified manner in the simulation of power systems.     

Anomalous double blood supply to the lung

We report an unusual case in which an apparently normal upper lobe of the right lung was supplied by major systemic arterial and pulmonary arterial vessels. The anomalous artery arose from the descending aorta. Following interruption of this vessel, the machinery-like murmur previously present disappeared.
