Diabetic foot ulcer (DFU) is a diabetes complication which greatly impacts the patient’s quality of life, often leading to amputation of the affected limb unless there is a timely and adequate management of the patient. DFUs have a high economic impact for the national health system. Data have indeed shown that DFUs are a major cause of hospitalization for patients with diabetes. Based on that, DFUs represent a very important challenge for the national health system. Especially in developed countries diabetic patients are increasing at a very high rate and as expected, also the incidence of DFUs is increasing due to longevity of diabetic patients in the western population. Herein, the surgical approach focused on the targeted use of the acellular dermal matrix has been integrated with biochemical and morphological/histological analyses to obtain evidence-based information on the mechanisms underlying tissue regeneration. In this research report, the clinical results indicated decreased postoperative wound infection levels and a short healing time, with a sound regeneration of tissues. Here we demonstrate that the key biomarkers of wound healing process are activated at gene expression level and also synthesis of collagen I, collagen III and elastin is prompted and modulated within the 28-day period of observation. These analyses were run on five patients treated with Integra® sheet and five treated with the injectable matrix Integra® Flowable, for cavitary lesions. In fact, clinical evaluation of improved healing was, for the first time, supported by biochemical and histological analyses. For these reasons, the present work opens a new scenario in DFUs treatment and follow-up, laying the foundation for a tailored protocol towards complete healing in severe pathological conditions. 相似文献

Heart failure (HF) affects up to over 20% of patients with type 2 diabetes (T2DM), even more in the elderly. Although, in T2DM, both hyperglycemia and the proinflammatory status induced by insulin resistance are crucial in cardiac function impairment, SGLT2i cardioprotective mechanisms against HF are several. In particular, these beneficial effects seem attributable to the significant reduction of intracellular sodium levels, well-known to exert a cardioprotective role in the prevention of oxidative stress and consequent cardiomyocyte death. From a molecular perspective, patients’ exposure to gliflozins’ treatment mimics nutrient and oxygen deprivation, with consequent autophagy stimulation. This allows to maintain the cellular homeostasis through different degradative pathways. Thus, since their introduction in the clinical practice, the hypotheses on SGLT2i mechanisms of action have changed: from simple glycosuric drugs, with consequent glucose lowering, erythropoiesis enhancing and ketogenesis stimulating, to intracellular sodium-lowering molecules. This provides their consequent cardioprotective effect, which justifies its significant reduction in CV events, especially in populations at higher risk. Finally, the updated clinical evidence of SGLT2i benefits on HF was summarized. Thus, this review aimed to analyze the cardioprotective mechanisms of sodium glucose transporter 2 inhibitors (SGLT2i) in patients with HF, as well as their clinical impact on cardiovascular events. 相似文献

S1P is the final product of sphingolipid metabolism, which interacts with five widely expressed GPCRs (S1P1-5). Increasing numbers of studies have indicated the importance of S1P3 in various pathophysiological processes. Recently, we have identified a pepducin (compound KRX-725-II) acting as an S1P3 receptor antagonist. Here, aiming to optimize the activity and selectivity profile of the described compound, we have synthesized a series of derivatives in which Tyr, in position 4, has been substituted with several natural aromatic and unnatural aromatic and non-aromatic amino acids. All the compounds were evaluated for their ability to inhibit vascular relaxation induced by KRX-725 (as S1P3 selective pepducin agonist) and KRX-722 (an S1P1-selective pepducin agonist). Those selective towards S1P3 (compounds V and VII) were also evaluated for their ability to inhibit skeletal muscle fibrosis. This study investigated whether European, Mediterranean and natural landscapes might trigger identification responses among residents of Mediterranean Europe. The participants (N = 546) were students from three Italian and three Spanish regions who rated their level of place identification by looking at eight pictures of prototypical landscapes, each depicting a unique combination of attributes in a 2×2×2 research design (European/non-European, Mediterranean/non-Mediterranean and natural/built). The results confirmed both European and natural landscapes as stronger carriers of identification than non-European and built landscapes, respectively, whereas no differences emerged between Mediterranean and non-Mediterranean landscapes. The statistical analysis of two-way interactions showed the amplification effect of the combined European–natural landscape, whilst a less clear picture emerged from the other two interactions. These findings suggest that landscape images may be used to foster a more inclusive identity at the supranational level and thus increase the sense of belonging among European citizens. 相似文献

The widespread acceptance of carotid artery stenting (CAS) to treat carotid artery stenosis and its effectiveness compared with surgical counterpart, carotid endarterectomy (CEA), is still a matter of debate. Transient or permanent neurological deficits may develop in patients undergoing CAS due to distal embolization or hemodynamic changes. Design, development, and usage of embolic protection devices (EPDs), such as embolic protection filters, appear to have a significant impact on the success of CAS. Unfortunately, some drawbacks, such as filtering failure, inability to cross tortuous high-grade stenoses, malpositioning and vessel injury, still remain and require design improvement. Currently, many different designs of such devices are available on the rapidly growing dedicated market. In spite of such a growing commercial interest, there is a significant need for design tools as well as for careful engineering investigations and design analyses of such nitinol devices. The present study aims to investigate the embolic protection filter design by finite element analysis. We first developed a parametrical computer-aided design model of an embolic filter based on micro-CT scans of the Angioguard? XP (Cordis Endovascular, FL) EPD by means of the open source pyFormex software. Subsequently, we used the finite element method to simulate the deployment of the nitinol filter as it exits the delivery sheath. Comparison of the simulations with micro-CT images of the real device exiting the catheter showed excellent correspondence with our simulations. Finally, we evaluated circumferential basket-vessel wall apposition of a 4 mm size filter in a straight vessel of different sizes and shape. We conclude that the proposed methodology offers a useful tool to evaluate and to compare current or new designs of EPDs. Presented in this paper are results for the elastic instability of Pultruded Fibre Reinforced Polymer (PFRP) portal frames with semi-rigid connections. The kinematical model used is based on a second-order displacement field, accounting for the shear strain influence on both non-uniform bending and torsion. A two-node locking-free finite element with seven degrees of freedom per node is adopted. Joint flexibility at member ends is included by means of a simple manipulation of the stiffness matrix to the finite element, such that the influence of joint behaviour on membrane, shear, bending and torsion deformations, as well as to cross-section warping, can easily be taken into account. Numerical examples are reported and discussed to illustrate the influence on elastic buckling loads of in-plane and out-of-plane moment-rotation behaviour of joints, as well as the role played by the base warping restraint and deformable lateral bracings. 相似文献

In recent years, the need for a more accurate dependability modelling (encompassing reliability, availability, maintenance, and safety) has favoured the emergence of novel dynamic dependability techniques able to account for temporal and stochastic dependencies of a system. One of the most successful and widely used methods is Dynamic Fault Tree that, with the introduction of the dynamic gates, enables the analysis of dynamic failure logic systems such as fault‐tolerant or reconfigurable systems. Among the dynamic gates, Priority‐AND (PAND) is one of the most frequently used gates for the specification and analysis of event sequences. Despite the numerous modelling contributions addressing the resolution of the PAND gate, its failure logic and the consequences for the coherence behaviour of the system need to be examined to understand its effects for engineering decision‐making scenarios including design optimization and sensitivity analysis. The thermal stability of nitrogen (N) functionalities on the sidewalls of N-doped multi-walled carbon nanotubes was investigated at temperatures ranging between 1000 °C and 2000 °C. The structural stability of the doped tubes was then correlated with the electrical conductivity both at the bulk and at the individual tube levels. When as-grown tubes were thermally treated at 1000 °C, we observed a very significant decrease in the electrical resistance of the individual nanotubes, from 54 kΩ to 0.5 kΩ, which is attributed to a low N doping level (e.g. 0.78 at% N). We noted that pyridine-type N was first decomposed whereas the substitutional N was stable up to 1500 °C. For nanotubes heat treated to 1800 °C and 2000 °C, the tubes exhibited an improved degree of crystallinity which was confirmed by both the low R value (I(D)/I(G)) in the Raman spectra and the presence of straight graphitic planes observed in TEM images. However, N atoms were not detected in these tubes and caused an increase in their electrical resistivity and resistance. These partially annealed doped tubes with enhanced electrical conductivities could be used in the fabrication of robust and electrically conducting composites, and these results could be extrapolated to N-doped graphene and other nanocarbons. 相似文献

The term “4D printing” refers to the development of stimulus‐responsive structures through 3D printing of active smart materials, typically shape memory polymers. A noteworthy aim of this research field is to obtain objects able to display complex shape‐shifting motions, such as sequential transformations over time. In this work, this peculiar response is studied on a commercial photopolymer, printed by stereolithography and featuring, on the basis of its inherent broad glass transition, the so‐called “temperature‐memory effect” (TME). The TME, that is, a response in which the shape memory effect occurs on a region controlled by the deformation temperature, is studied in shape memory cycles where the deformation temperature is systematically varied, so to provide a correlation between deformation and recovery temperatures. This also allows to properly select two temperatures at which deforming a specimen along a multistep history, so as to finally separate each recovery process on the temperature and time scales. This sequential recovery is studied in double folded bars, with arms deformed at different temperatures, and on a properly designed self‐locking clamp. The obtained results are promising for the realization of smart temperature‐responsive structures printed with one single polymer and capable of multiple shape transformations. 相似文献

In foregoing papers we have used the compression/decompression method of images based on the concept of discrete fuzzy transform (and its inverse) of a function f defined on a real interval with respect to the fuzzy sets A1,…,An forming a fuzzy partition of such interval. Here we make a detailed experimental comparison with the similar method based on the fuzzy transforms F↑ and F↓ of f defined via a continuous triangular norm and its corresponding residuum, respectively. We consider some images of sizes 256 × 256 (pixels) extracted from the well-known database Corel Galery (Arizona Directory). By using the same compression rate in both methods, we have that the PSNR (Peak Signal to Noise Ratio) obtained with the discrete fuzzy transform (and its inverse) of f is more higher than the PSNR determined with the operators F↑ and F↓ defined via the usual Lukasiewicz, product and minimum triangular norms. Moreover, we compare our results with the classical JPEG method for values of compression rate approximately equal to those used in the previous methods. 相似文献
