Reviews the literature on death anxiety as it relates to psychosocial care of the terminally ill. The literature strongly supports the need for caregivers to understand their own fears and feelings in order to deal with terminal patients. The negative potential consequences of misunderstood feelings about death are discussed as well as several major correlates (sex, socioeconomic status, occupation, general anxiety, and level of consciousness). Some results of groups emphasizing ventilation of fears about death are also reviewed briefly. Their positive effects on treatment suggest the value of gaining more accurate understanding of death anxiety through future research. In 3 experiments with 93 Long-Evans male hooded rats, cathodal electrolytic lesions of the medial forebrain bundle (MFB) at posterior hypothalamic levels produced a mild, transient hypodipsia and lowered jump thresholds to footshock. Lesions produced marked deficits in passive avoidance performance in a paradigm that paired discrete, linearly incrementing footshock intensities with contact of a water spout following 48 hrs of water deprivation. Injections of levo-dextro-5-hydroxytryptophan (75 mg g, ip), the immediate metabolic precursor of serotonin, had no effect on the passive avoidance performance of either experimental or operated control Ss. Lesions of the MFB also resulted in deficient acquisition in a 1-trial step-through passive avoidance paradigm not using motivation to drink and caused a severe acquisition deficit in a 1-way active avoidance task. Lesions of the septal nuclei produced lowered jump thresholds but did not affect acquisition in the 1st passive avoidance task. Results are interpreted as indicating a lesion-induced deficiency in fear learning, independent of the serotonergic functions of the MFB. Examined the effects of instructions and modeling in the reflection-of-feeling training of individuals low in interpersonal-communication skills. Participants with low scores on R. Carkhuff's Communication Index (25 males and 25 females) were randomly assigned by sex to 1 of the 5 training groups so that there were 5 males and 5 females in each group. After viewing videotaped training material, each participant conducted a 15-min interview with a volunteer "client" and completed the Communication Index a second time. Interviews were scored for frequency of reflection-of-feeling responses and level of empathic communication displayed. Analysis of the reflection-of-feeling data indicated that instructions and instructions-plus-modeling were facilitative procedures. Ratings of written and verbal empathy demonstrated the effectiveness of the 2 combined conditions: instructions-plus-modeling and modeling-plus-instructions. Calculation of correlations among scores on criterion measures revealed a positive correlation between reflection-of-feeling scores and ratings of empathic communication. From the results of this study, it appears that whole blood alone or red blood cells reconstituted with saline solution do not adequately replace the loss of albumin and concomitant decrease in colloid oncotic pressure that occur during extensive intraabdominal and intrathoracic operations. Since colloid oncotic pressure is a major factor in the restitution of intravascular volume from stores of interstitial fluid and since it may also play a role in the development of postoperative pulmonary problems, it is important to maintain a relatively normal colloid oncotic pressure during the operation and in the immediate postoperative period. Although dextran can be used for this purpose, its short half-life of four to six hours and associated coagulation problems militate against its use in large quantities. This leaves purified protein fraction or salt-poor albumin as the main sources of protein for the maintenance of colloid oncotic pressure. Both of these products are expensive and short in supply. The oncometer in present use is a clinically feasible and rapid, one to three minutes, means of determining the colloid oncotic pressure. It permits a rational approach to the use of albumin products, avoiding the pitfalls of under or excess administration in the operative and postoperative periods. 相似文献

The functional foods sector represents a significant and growing portion of the food industry, yet formulation of these products often involves the use of ingredients that elicit less than desirable oral sensations, including bitterness. Promising new functional ingredients, including polyphenolics, may be more widely and readily employed in the creation of novel functional foods if their aversive bitter taste can be significantly reduced. A number of approaches are used by the industry to improve the taste properties and thus the acceptance of conventional foods that elicit excessive bitterness. Breeder cows, cattle recently arrived at feedlots, and cattle about to be shipped for slaughter were tested for Salmonella spp. No Salmonella spp. were detected in fecal samples from breeding cows. Nineteen of 1,000 (1.9%) fecal samples from recently arrived feedlot cattle were positive for Salmonella spp. compared to only 2 of 1,000 (0.2%) fecal samples taken within 2 weeks of slaughter. The positive fecal samples were collected in 5 of 50 (10%) "recent arrival" pens tested and in 1 of 50 (2%) pens tested within 2 weeks of slaughter. The serotypes isolated were Salmonella Agona, Salmonella Enteritidis, Salmonella Typhimurium DT104, and Salmonella 4,5,12:i:-. Ground beef samples purchased from retail outlets throughout Alberta were processed for Salmonella spp. Thirteen of 1,002 (1.3%) samples were positive for Salmonella spp. The serotypes isolated from ground beef were Salmonella Anatum, Salmonella Heidelberg, Salmonella Montevideo, Salmonella Typhimurium, Salmonella Typhimurium var. Salmonella and Campylobacter continue to be major foodborne pathogens and raw poultry is considered to be an important source of these bacteria. In this study, the prevalence and numbers of Salmonella and Campylobacter spp. in relation to isolation/sampling methods were determined in 241 whole raw chickens purchased from retail outlets in England during the winters of 1998/1999 (101 chickens) and 1999/2000 (140 chickens). The packaging of the 140 chickens was also examined for the presence of the above pathogens. The prevalence and numbers of enterococci were examined in 21 of the 101 chickens. In total, Salmonella and Campylobacter spp. were present in 25% and 83% of the chickens, respectively. Salmonella were isolated from a sample representing both the inside and outside of the packaging in 19% of the chickens, while the corresponding figure for Campylobacter spp. was 56%. Both of these pathogens were isolated from the outside of the packaging in 6% of the chickens. Salmonella was more frequently isolated from samples containing chicken skin in comparison with those containing carcass-rinse fluid only. Two chickens (0.8%) were positive for Salmonella by direct enumeration methods with contamination levels of log10 3.8 and 4.5 colony forming units (cfu) per carcass, respectively. The most prevalent serotypes were S. Hadar, S. Enteritidis and S. Indiana and two different serotypes were identified in 5/20 salmonella-positive chickens. Resistance to at least one antibiotic was found in 70% of the strains, 46% were multiresistant (resistant to > or = four drugs) and 52% showed a lowered susceptibility to ciprofloxacin. The likelihood of isolating Campylobacter spp. from neck-skin, carcass-rinse or carcass-rinse plus whole skin samples was similar, Campylobacter spp. were found in higher levels in carcass-rinse or carcass-rinse plus whole skin samples than in neck-skin. The log10 cfu of Campylobacter spp. were 2.70-4.99 in 18% of the chickens and 5.00-6.99 in 20%. Campylobacter isolates (425) comprised Campylobacter jejuni (98%) and C. coli (2%) and 98 different sero/phagetypes of these two species were identified. Resistance to at least one antibiotic was found in 73% of the strains and 13% were multiresistant. Thirteen percent of the strains showed lowered susceptibility to ciprofloxacin, while 4.9% were resistant to erythromycin. Vancomycin-resistant enterococci (VRE), able to grow on agar containing 15 mg l(-1) vancomycin (VRE15), were present in 19 chickens. The log10 cfu of VRE15 was 2.90-3.99 in 10 chickens and between 4.00 and 4.99 in two chickens. The data presented here contribute to risk assessment and highlight the need to continue to emphasise the safe handling of raw retail poultry. 相似文献
