Objectives

Short-distance household falls are a common occurrence in young children, but are also a common false history given by caretakers to conceal abusive trauma. The purpose of this study was to determine the severity of injuries that result from accidental short-distance household falls in children, and to investigate the association of fall environment and biomechanical measures with injury outcomes.

Methods

Children aged 0–4 years who presented to the Emergency Department with a history of a short furniture fall were included in the study. Detailed case-based biomechanical assessments were performed using data collected through medical records, interviews, and fall scene investigations. Injuries were rated using the Abbreviated Injury Scale (AIS). Each case was reviewed by a child abuse expert; cases with a vague or inconsistent history and cases being actively investigated for child abuse were excluded.

Results

79 subjects were enrolled in the study; 15 had no injuries, 45 had minor (AIS 1) injuries, 17 had moderate (AIS 2) injuries, and 2 had serious (AIS 3) injuries. No subjects had injuries classified as AIS 4 or higher, and there were no fatalities. Children with moderate or serious injuries resulting from a short-distance household fall tended to have fallen from greater heights, have greater impact velocities, and have a lower body mass index than those with minor or no injuries.

Conclusion

Children aged 0–4 years involved in a short-distance household fall did not sustain severe or life-threatening injuries, and no children in this study had moderate or serious injuries to multiple body regions. Biomechanical measures were found to be associated with injury severity outcomes in short-distance household falls. Knowledge of relationships between biomechanical measures and injury outcomes can aid clinicians when assessing whether a child's injuries were the result of a short-distance fall or some other cause.  相似文献

We have investigated the dynamics of the record-erase process of holograms in photochromic glass using continuum Nd:YVO? laser radiation (λ=532 nm). A bidimensional microgrid pattern was formed and visualized in photochromic glass, and its diffraction efficiency decay versus time (during reconstruction step) gave us information (D, Δn) about the diffusion process inside the material. The recording and reconstruction processes were carried out in an off-axis setup, and the images of the reconstructed object were recorded by a CCD camera. Measurements realized on reconstructed object images using holograms recorded at a different incident power laser have shown a two-stage process involved in silver atom kinetics.  相似文献

Changes in maternal nutrient intake during gestation alter IGF receptor abundance and leptin (LEP) mRNA expression in fetal adipose tissue. It is not known whether such changes persist into adult life and whether they are associated with an effect on phenotype. We investigated the effect of high (240%) and low (70%) levels of recommended daily crude protein intake for beef heifers during the first and second trimesters of gestation on singleton progeny (n=68): subcutaneous (SC) adipose tissue depth at rump (P8) and rib (RF) sites from 65 until 657 days of age; plasma leptin concentrations from birth until 657 days and expression of IGF1 and IGF2, their receptors (IGF1R and IGF2R) and LEP mRNA in perirenal (PR), omental (OM) and SC adipose tissue at 680 days of age. High-protein diets during the first trimester increased LEP and IGF1 mRNA in PR of males and females, respectively, compared with low-protein diets, and decreased IGF1R mRNA in SC of all progeny but increased RF depth of males between 552 and 657 days. High-protein diets compared with low-protein diets during the second trimester increased IGF1R mRNA in PR and OM of all progeny; LEP mRNA in PR of males; and IGF2 and IGF2R mRNA in OM of all progeny. Conversely, LEP mRNA in OM and IGF2 mRNA in PR of all progeny were decreased following exposure to high- compared with low-protein diets during the second trimester. Heifer diet during gestation has permanent sex- and depot-specific effects on the expression of adipogenic and adipocytokine genes and offspring adiposity.  相似文献

Results concerning the magnetic, magnetostrictive, structural, morphological, and topological properties of amorphous and nanocrystalline Fe 73.5Cu 1Nb 3Si 15.5 B 7 thin films deposited using the high power impulse magnetron sputtering (HiPIMS) technique are reported. In as-deposited state, the samples are amorphous, the nanocrystalline state being achieved for samples isothermally annealed at adequate temperatures, in an electric furnace. For the optimum annealing temperature (475 °C), a decrease by about 70 % for the coercive magnetic field (50 A/m) and up to 1 order of magnitude for the saturation magnetostriction (~1×10?6), compared to the as-deposited state, was obtained. The X-ray diffractometry (XRD) and scanning electron microscopy (SEM) results for samples thermally treated at 475 °C revealed a 53.6 % crystalline volume fraction of α-Fe(Si) nanograins with an average size of about 15 nm and a Si content of 10.78 %, uniformly dispersed in a residual amorphous matrix. Using the saturation magnetostriction values determined using the cantilever deflection method and the crystalline volume fraction of α-Fe(Si) nanograins, the contribution of crystalline phase to the saturation magnetostriction was also determined.  相似文献

Creaming of the dispersed oil phase in opaque oil-in-water emulsions was followed as a function of time and position using proton magnetic resonance. Measurements of T1 along a vertical slice were used to quantify the oil-phase concentration. Emulsions were also described by laser-diffraction particle-size analysis to compare particle-size analysis with creaming behavior as determined by T1 measurements in model triolein-in-water emulsions and more complex milk fat-in-water emulsions. Particle-size distributions failed to accurately predict the measured creaming behavior in the emulsions studied. Measurements of component nuclear relaxation were shown to be useful for characterizing oil-phase migration as a function of surfactant level, dispersed phase volume, and crystallization behavior of the lipid phase in emulsions.  相似文献

Persistent organic pollutants have been associated with disease susceptibility and decreased immunity in marine mammals. Concentrations of polychlorinated biphenyls (PCBs), dichlorodiphenyltrichloroethane and its metabolites (DDTs), polybrominated diphenyl ethers (PBDEs), chlordanes (CHLDs), and hexachlorocyclohexane isomers (HCHs) were evaluated in terms of stage of development and likely exposure routes (in utero, suckling, fasting) in the blubber of 202 stranded and wild-caught, primarily young of the year (n = 177), harbor seals (Phoca vitulina) in the central California coast. This is the first report of HCH concentrations in the blubber of California seals. Lipid normalized concentrations ranged from 200 to 330,000 ng/g for sum PCBs, 320-1,500,000 ng/g for sum DDTs, 23-63,000 ng/g for sum PBDEs, 29-29,000 ng/g for sum CHLDs, and 2-780 ng/g for sum HCHs. The highest concentrations were observed in harbor seal pups that suckled in the wild and then lost mass during the post-weaning fast. Among the pups sampled in the wild and those released from rehabilitation, there were no differences in mass, blubber depth, or percent lipid although contaminant concentrations were significantly higher in the pups which nursed in the wild. When geographic differences were evaluated in a subset of newborn animals collected near their birth locations, the ratio of sum DDTs to sum PCBs was significantly greater in samples from an area with agricultural inputs (Monterey), than one with industrial inputs (San Francisco Bay). A principal components analysis distinguished between seals from San Francisco Bay and Monterey Bay based on specific PCB and PBDE congeners and DDT metabolites. These data illustrate the important influence of life stage, nutritional status, and location on blubber contaminant levels, and thus the need to consider these factors when interpreting single sample measurements in marine mammals.  相似文献

Grapes (Vitis vinifera var. Sugraone and Vitis labrusca var. Crimson Seedless) were treated with 400, 600, and 800 Gy and the effects on physicochemical factors were measured alongside sensory testing during 3 wk of storage. Significant changes in texture and color with irradiation and age were measured but little visual difference was seen between control and irradiated grapes. However, age had a greater effect on firmness than irradiation for Sugraone grapes. Irradiation did not significantly (P ≤ 0.05) affect the SSC/TA ratio, which increased during storage. The trained panel detected significant changes in the berry texture and rachis color but rated sweetness and flavor significantly higher (P ≤ 0.05) for irradiated Sugraone as compared to the control. Consumers liked both the untreated and 800 Gy treated Sugraone grapes, but liked the untreated grapes more for texture (P ≤ 0.05). However, there was no difference in liking between irradiated (600 Gy or 800 Gy) and control samples of Crimson Seedless for any attribute. The results show that there are varietal differences in response to irradiation but the overall maintenance in quality of irradiated grapes during 3 wk of storage indicates that irradiation can serve as a viable phytosanitary treatment.  相似文献

Individual whole body homogenates of 4 year old lake trout (Salvelinus namaycush) samples collected in 2001 from each of the Great Lakes were extracted using a novel fluorophilicity cleanup step and analyzed for perfluoroalkyl compounds (PFCs). Standard addition and internal standardization were used for quantification. Results were reported (+/- SE) for perfluorinated carboxylates (PFCAs), perfluorinated sulfonates (PFSAs), and unsaturated fluorotelomer carboxylates (8:2 and 10:2 FTUCA). The lowest average concentration of sigmaPFC was found in samples from Lake Superior (13+/-1 ng g(-1)), while the highest average concentration was found in samples from Lake Erie (152+/-14 ng g(-1)). Samples from Lake Ontario (60+/-5 ng g(-1)) and Lake Huron (58 +/-10 ng g(-1)) showed similar average sigmaPFC concentrations, although the perfluorinated sulfonate/carboxylate ratios were different. The major perfluoroalkyl contaminant observed was perfluorooctane sulfonate (PFOS) with the highest concentration found in samples from Lake Erie (121+/-14 ng g(-1)), followed by samples from Lake Ontario (46+/-5 ng g(-1)), Lake Huron (39 +/-10 ng g(-1)), Lake Michigan (16+/-3 ng g(-1)), and Lake Superior (5+/-1 ng g(-1)). Perfluorodecane sulfonate (PFDS) was detected in 89% of the samples, with the highest concentration in Lake Erie samples (9.8+/-1.6 ng g(-1)), and lowest concentration in samples from Lake Superior (0.7 +/- 0.1 ng g(-1)). Statistically significant correlations were observed between PFOS and PFDS concentrations, and PFOS concentration and body weight, respectively. The PFCAs were detected in all samples, with the highest total average concentration in samples from Lake Erie (19 ng g(-1)), followed by samples from Lake Huron (16 ng g(-1)), Lake Ontario (10 ng g(-1)), Lake Michigan (9 ng g(-1)) and Lake Superior (7 ng g(-1)). The compounds with significant contributions to the sigmaPFCA concentrations were PFOA and C9-C13-PFCAs. The 8:2 FTUCA was detected at concentrations ranging between 0.1 and 0.2 ng g-1, with the highest level in samples showing also elevated concentrations of PFOA (4.4 ng g(-1) for Lake Michigan vs 1.5 ng g(-1) for all other samples). The 10:2 FTUCA was detected only in 9% of all samples (nd, 45 pg g(-1)). For those PFCs where we determined lake water concentrations, the highest log BAFs were calculated for PFOS (4.1), PFDA (3.9), and PFOSA (3.8).  相似文献
