PURPOSE: Though case management has been recommended to improve the outcomes of patients with costly or morbid conditions, it has seldom been studied in controlled trials. We performed a randomized, controlled clinical trial of an intensive, multidisciplinary case management program for patients with chronic renal insufficiency and followed patients for 5 years. PATIENTS AND METHODS: We enrolled 437 primary-care patients (73% of those eligible) with chronic renal insufficiency (estimated creatinine clearance consistently < 50 mL/min with the last serum creatinine level > 1.4 mg/dL) who were attending an urban academic general internal medicine practice. The intensive case management, administered during the first 2 years after enrollment, consisted of mandatory repeated consultations in a nephrology case management clinic staffed by two nephrologists, a renal nurse, a renal dietitian, and a social worker. Control patients received usual care. Primary outcome measurements included serum creatinine level, estimated creatinine clearance, health services use, and mortality in the 5 years after enrollment. Secondary measures included use of renal sparing and potentially nephrotoxic drugs. RESULTS: There were no differences in renal function, health services use, or mortality in the first, second, or third through fifth years after enrollment. There were significantly more outpatient visits among intervention patients, mainly because of the added visits to the nephrology case management clinic. There were also no significant differences in the use of renal sparing or selected potentially nephrotoxic drugs. The annual direct costs of the intervention were $89,355 ($484 per intervention patient). CONCLUSION: This intensive, multidisciplinary case-management intervention had no effect on the outcomes of care among primary-care patients with established chronic renal insufficiency. Such expensive and intrusive interventions, despite representing state-of-the-art care, should be tested prospectively before being widely introduced into practice.  相似文献

In this study, we proposed an in vitro tumor model to simulate the mechanical microenvironment and investigate the effect of compressive stress on the invasion process of malignant tumors. It has been pointed out that the biomechanical environment, as well as the biochemical environment, could affect the transformation of cancer cell migration, invasion, and metastasis. We hypothesized that the solid stress caused by the exclusion of surrounding tissue could transform tumor cells from noninvasive to invasive phenotypes. Colorectal cell spheroids were embedded and cultured in agarose gels of varying concentrations to simulate the earliest stages of tumor formation and invasion. The spheroids embedded in gels at higher concentrations showed peculiar growth after 72 h of culture, and the external compressive loading imposed on them caused peculiar growth even in the gels at lower concentrations. In conclusion, the mechanical microenvironment caused the transformation of tumor cell phenotypes, promoting the growth and invasion of tumor cell spheroids.  相似文献

The role of oxygen for the hole doping has been investigated for single-phased (Bi,Pb)2Sr2Ca2Cu3O. z Defects introduced by oxygen have been clarified to be interstitial atoms by pycnometric density measurements. The c axis decreases and the modulation periods along the b axis also decrease with increasing oxygen content in the crystal. It is likely that extra oxygen ions exist between the BiO double layers and those oxygen ions are responsible for the modulation. Samples with oxygen content ranging from 9.99 to 10.18 showed superconductivity. The critical temperature T c , exhibited a maximum value (109 K) at 10.07 oxygen atoms in the formula unit, which coincided with the average Cu valence of 2.13. The data indicated that hole formation was governed by oxygenation reaction in this system.  相似文献

The thermal conductivity of polycrystalline Al-doped ZnO was controlled through the fabrication of nanostructured polycrystalline materials, by hot-pressing nanosized Zn1– x Al x O ( x = 0.01, 0.02) particles, which were synthesized by a coprecipitation and spray-drying method. This process resulted in an improved thermoelectric power factor because of the nanosized Zn0.99Al0.01O particles of the polycrystalline products. The thermal conductivity also was decreased as a result of the formation of nanocrystalline Zn0.99Al0.01O.  相似文献

Abstract

A novel adsorbent Zr(IV)-immobilized resin was prepared to remove fluoride ions from tap water and industrial wastewater. In order to enhance both the kinetics and efficiency, large pathways were formed in the resin for fluoride ion adsorption and the Zr(IV)-phosphate complexes were immobilized on the polymer surface by surface-template polymerization. The Zr(IV)-immobilized resin had a fluoride adsorption capacity of 0.30 mmol/g. The morphology of the Zr(IV)-immobilized resin was evaluated by measuring the specific surface area, pore volume, and pore size distribution. The resin possessed large amounts of large macropores with diameters around 300 nm. The molecular structure at the fluoride adsorption sites was investigated by measuring the amounts of phosphorus, zirconium, and fluoride ion in the resin, and developing a model complex using computational chemistry. On the polymer surface, a fluoride ion/Zr(IV)/dioleyl phosphoric acid complex with an ideal F:Zr:P mole ratio of 3:1:3 could be formed.  相似文献

Angiogenesis is an important process for tumor growth and progression of various solid tumors including urological cancers. Thrombospondins (TSPs), especially TSP-1, are representative “anti”-angiogenic molecules and many studies have clarified their pathological role and clinical significance in vivo and in vitro. In fact, TSP-1 expression is associated with clinicopathological features and prognosis in many types of cancers. However, TSP-1 is a multi-functional protein and its biological activities vary according to the specific tumor environments. Consequently, there is no general agreement on its cancer-related function in urological cancers, and detailed information regarding regulative mechanisms is essential for a better understanding of its therapeutic effects and prognostic values. Various “suppressor genes” and “oncogenes” are known to be regulators and TSP-1-related factors under physiological and pathological conditions. In addition, various types of fragments derived from TSP-1 exist in a given tissue microenvironment and TSP-1 derived-peptides have specific activities. However, a detailed pathological function in human cancer tissues is not still understood. This review will focus on the pathological roles and clinical significance of TSP-1 in urological cancers, including prostate cancer, renal cell carcinoma, and urothelial cancer. In addition, special attention is paid to TSP-1-derived peptide and TSP-1-based therapy for malignancies.  相似文献

Metabolic syndrome is associated with an increased risk of colorectal cancer. This study investigated the impact of hypertension, a component of metabolic syndrome, on azoxymethane (AOM)-induced colorectal carcinogenesis using SHRSP/Izm (SHRSP) non-diabetic/hypertensive rats and SHRSP.Z-Leprfa/IzmDmcr (SHRSP-ZF) diabetic/hypertensive rats. Male 6-week-old SHRSP, SHRSP-ZF, and control non-diabetic/normotensive Wister Kyoto/Izm (WKY) rats were given 2 weekly intraperitoneal injections of AOM (20 mg/kg body weight). Two weeks after the last injection of AOM, the SHRSP and SHRSP-ZF rats became hypertensive compared to the control WKY rats. Serum levels of angiotensin-II, the active product of the renin-angiotensin system, were elevated in both SHRSP and SHRSP-ZF rats, but only the SHRSP-ZF rats developed insulin resistance, dyslipidemia, and hyperleptinemia and exhibited an increase in adipose tissue. The development of AOM-induced colonic preneoplastic lesions and aberrant crypts foci, was significantly accelerated in both SHRSP and SHRSP-ZF hypertensive rats, compared to WKY normotensive rats. Furthermore, induction of oxidative stress and exacerbation of inflammation were observed in the colonic mucosa and systemically in SHRSP and SHRSP-ZF rats. Our findings suggest that hypertension plays a role in the early stage of colorectal carcinogenesis by inducing oxidative stress and chronic inflammation, which might be associated with activation of the renin-angiotensin system.  相似文献

An industrial melt‐spinning process of tetrafluoroethylene– hexafluoropropylene copolymer (FEP) using an extruder was studied. The novel “spinneret,” having both a large‐diameter spinning nozzle and a high‐temperature vessel, was used to solve the problem of filament breakage on the spinning line caused by high melting viscosity of FEP. The extruder, with its long feed zone, was newly designed to function with a geared pump. The strength of fibers increased with drawing of as‐spun fiber. FEP fibers up to six denier were continuously produced through long‐run production. Layered -titanate materials, NaxMx/2Ti1−x/2O2 (M=Co, Ni and Fe, x=0.2–0.4), were synthesized by flux reactions, and electrical properties of polycrystalline products were measured at 300–800 °C. After sintering at 1250 °C in Ar, all products show n-type thermoelectric behavior. The values of both d.c. conductivity and Seebeck coefficient of polycrystalline Na0.4Ni0.2Ti0.8O2 were ca. 7×103 S/m and ca. −193 μV/K around 700 °C, respectively. The measured thermal conductivity of layered -titanate materials has lower value than conductive oxide materials. It was ca. 1.5 Wm−1 K−1 at 800 °C. The estimated thermoelectric figure-of-merit, Z, of Na0.4Ni0.2Ti0.8O2 and Na0.4Co0.2Ti0.8O2 was about 1.9×10−4 and 1.2×10−4 K−1 around 700 °C, respectively.  相似文献

In order to improve the weatherability of acryonitrile—styrene—butadiene rubber graft polymer (ABS resin), an attempt was made to develop a resin (AAS resin) in which acrylic rubber of good weatherability was used instead of butadiene rubber. First, by copolymerizing dicyclopentenyl-methacrylate (DCP-MA,3%) with butyl acrylate, crosslinked acrylic rubber was obtained. This also introduced grafting sites into the rubber. Next, methods of graft copolymerizing styrene and acrylonitrile with this rubber were examined. An emulsion–suspension polymerization method was developed in which the initial stage of the polymerization, emulsion polymerization, changed into suspension polymerization during the process. By this method of polymerization, rubber particles were combined and enlarged, bringing about a graft-type resin with high impact resistance. This polymerization method is industrially useful because particle-shaped resins are obtained without the need of a salting-out process. The AAS resin, obtained in this way, has much improved weatherability over ABS resin and shows strength equal to that of ABS resin. © 1992 John Wiley & Sons, Inc.  相似文献
