OBJECTIVE: This study assesses intravascular density produced by ionic and nonionic contrast material and its effect on visualization of stenoses by CT angiography. MATERIALS AND METHODS: CT angiography was performed using a 32-vessel phantom to study grades of luminal stenoses (0-100%), three lengths of stenoses (1, 3, and 5 mm), and two angles of inclination into the stenoses (45 degrees and 75 degrees). Scans were obtained with a slice thickness of 2 mm, a slice interval of 1.5 mm, a pitch of 1.0, a voltage of 120 kV, and a current of both 100 and 200 mA. Vessels were oriented parallel to the z-axis, and opacified with ionic and nonionic contrast material that had densities of 100, 150, 200, 250, 300, and 350 H. Cross-sectional luminal diameters were measured in and out of the stenoses. Edge definition and halo artifact for each vessel were graded by an investigator who was unaware of the contrast material density used. RESULTS: A contrast density of 150 H as revealed by CT angiography yielded the most accurate stenosis measurements with ionic contrast material. For nonionic contrast material, attenuation values of 150 and 200 H produced the best results on CT angiography. A density of 100 H or greater than 250 H significantly increased the error of vessel measurement (p = .001) for both types of contrast material. For the two current levels tested (100 and 200 mA), no statistical difference was found. CONCLUSION: The accuracy of CT angiography in measuring carotid stenosis depends on the luminal attenuation value. An optimum contrast density is 150 H for ionic contrast material; for nonionic contrast material, 150-200 H (at the window and level settings of 300 H and 40 H).  相似文献

This study focuses on the different efficiencies of secretion of two fungal cutinases by Saccharomyces cerevisiae, a wild-type cutinase (CY000) and a hydrophobic mutant cutinase (CY028). Both cutinases are placed under control of the GAL7 promoter, by which the expression levels can be regulated. Wild-type cutinase was secreted at up to 25 mg per g (dry weight), while CY028 was secreted at a level of 2 mg per g (dry weight); this difference is nearly independent of the expression level. Pulse-chase experiments revealed that whereas CY000 cutinase is secreted, CY028 is irreversibly retained in the cell. Immunogold labelling followed by electron microscopy revealed colocalization of CY028 with immunoglobulin heavy-chain binding protein (BiP) in the endoplasmic reticulum (ER). The increase of wild-type cutinase expression did not result in higher levels of the molecular chaperone BiP, but BiP levels are raised by increased induction of the hydrophobic mutant cutinase. Immunoprecipitation studies showed that in contrast to the wild-type cutinase, the hydrophobic mutant cutinase interacts with BiP. These results indicate that the introduction of two exposed hydrophobic patches in cutinase results in a higher affinity for BiP which might cause the retention of this mutant cutinase in the ER.  相似文献

OBJECTIVE: To evaluate the affect of families' attitudes about the appropriateness of discussing psychosocial concerns on pediatric providers' identification of psychosocial problems. DESIGN: These data were collected as part of the Greater New Haven Child Health Study, New Haven, Conn. The study design was a prospective cohort. SETTING: Families were recruited from a stratified random sample of all primary care practices in the greater New Haven area. Nineteen of 23 invited practices agreed to participate including 2 prepaid practices, 2 neighborhood health centers, and 7 fee-for-service group and 8 fee-for-service solo practices. PARTICIPANTS: All families of children aged 4 to 8 years who attended these practices during 2 separate 3-week periods (1 in fall 1987 and 1 in spring 1988) were invited to participate in the study. Families were invited to participate only once, on the first contact with any eligible child, using approved procedures. Of 2006 eligible families, 1886 (94%) chose to participate. MAIN OUTCOME MEASURE: The outcome variable for these analyses is the identification of any behavioral, emotional, or developmental problem by the pediatrician on the 13-category checklist. Overall, pediatric clinicians identified 27.5% of children with 1 or more psychosocial problems. RESULTS: Our data suggest that there is a great deal of discrepancy between what parents report is appropriate to do when their children have psychosocial problems and what they actually do when they recognize such problems in their children. Most (81.1%) believed it was appropriate to discuss 4 or more of the 6 hypothetical situations with their children's physician, while only 40.9% actually did discuss any of these problems with a physician when a problem occurred. Given the correlates of parents who intended to discuss such problems (higher education, older age, Euro-American ethnicity, higher income, married, availability of medical insurance) the possibility that parents are providing socially acceptable responses to such questions seems likely. Further, our data indicate that parents' actual reports of discussions of psychosocial problems is unrelated to whether physicians identified those problems in children. CONCLUSIONS: Pediatricians'judgments about the presence of psychosocial problems in their young patients seem to be based on their own observations rather than on what parents report. Physician-parent communication about psychosocial problems will be increasingly important as primary care physicians assume their role as gatekeepers to more expensive services such as mental health interventions.  相似文献

The aim of this paper is to describe three emerging computer-aided diagnosis (CAD) systems induced by Japanese health care needs. CAD has been developing fast in the last two decades. The idea of using a computer to help in medical image diagnosis is not new. Some pioneer studies are dated back to the 1960s. In 1998, the first U.S. FDA (Food and Drug Administration) approved commercial CAD system, a film-digitized mammography system, was launched by R2 Technologies, Inc. The success was quickly repeated by a number of companies. The approval of Medicare CAD reimbursement in the U.S. in 2001 further boosted the industry. Today, CAD has its significance in the economy of the medical industry. FDA approved CAD products in the field of breast imaging (mammography, ultrasonography and breast MRI) and chest imaging (radiography and CT) can be seen. In Japan, as part of the "Knowledge Cluster Initiative" of the government, three computer-aided diagnosis (CAD) projects are hosted at the Gifu University since 2004. These projects are regarding the development of CAD systems for the early detection of (1) cerebrovascular diseases using brain MRI and MRA images by detecting lacunar infarcts, unruptured aneurysms, and arterial occlusions; (2) ocular diseases such as glaucoma, diabetic retinopathy, and hypertensive retinopathy using retinal fundus images; and (3) breast cancers using ultrasound 3-D volumetric whole breast data by detecting the breast masses. The projects are entering their final development stage. Preliminary results are presented in this paper. Clinical examinations will be started soon, and commercialized CAD systems for the above subjects will appear by the completion of this project.  相似文献

Abstract— A reflective color STN-LCD with a single polarizer and double retardation films has been investigated. The double retardation films arranged in front of the LC layer enabled the LCD to contain reflective electrodes inside the panel. This configuration achieves a bright image with no parallax. A new construction of a reflective STN-LCD with a single polarizer has been developed by means of our own method in which the color difference ΔE* as the optimizing parameter has been used. Further, RGB color filters have been newly designed for our reflective LCD, and an aluminum (Al) layer has been introduced as a reflective electrode. As a result, we have realized a 7.8-in.-diagonal reflective color STN-LCD (640 × 480) which has 15% reflectance, 14:1 contrast ratio, 4096-color capability, and sufficient color gamut. It has been confirmed that the single-polarizer reflective color STN-LCD has sufficient performance for mobile business tools. We believe that it will be a key device for this application.  相似文献

The automatic determination of the optic disc area in retinal fundus images can be useful for calculation of the cup-to-disc (CD) ratio in the glaucoma screening. We compared three different methods that employed active contour model (ACM), fuzzy c-mean (FCM) clustering, and artificial neural network (ANN) for the segmentation of the optic disc regions. The results of these methods were evaluated using new databases that included the images captured by different camera systems. The average measures of overlap between the disc regions determined by an ophthalmologist and by using the ACM (0.88 and 0.87 for two test datasets) and ANN (0.88 and 0.89) methods were slightly higher than that by using FCM (0.86 and 0.86) method. These results on the unknown datasets were comparable with those of the resubstitution test; this indicates the generalizability of these methods. The differences in the vertical diameters, which are often used for CD ratio calculation, determined by the proposed methods and based on the ophthalmologist's outlines were even smaller than those in the case of the measure of overlap. The proposed methods can be useful for automatic determination of CD ratios.  相似文献

NAREGI is a 5-year Japanese National Grid Project during 2003-2007, whose chief aim is to develop a set of grid middleware to serve as a basis for future e-Science. NAREGI also aims to lead the way in standardization of grid middleware, based on the OGSA architecture. Its super-scheduler is based on the proposed OGSA-EMS Architecture, in that it becomes the first working implementation that implements the documented component relationships within the OGSA-EMS architecture document v.1.0. Through the efforts and experience in the design and implementation, it has been confirmed that the documented OGSA-EMS architecture is quite feasible, but will require significant amount of refinement and speed improvements to finalize its detailed specifications. The super-scheduler also supports co-allocation across multiple sites to support automated execution of grid-based MPIs that execute across machines. Such a resource allocation requires sophisticated interactions between the OGSA-EMS components not covered in the current OGSA-EMS architecture, some of which are non-trivial. Overall, job scheduling with OGSA-EMS has proven to not only work, but also that its job allocation and execution time is within reasonable bounds.  相似文献

Time-decay stress-induced leakage current (SILC) has been systematically investigated for the cases of both Fowler-Nordheim (FN) stress and substrate hot-hole stress. From the three viewpoints of the reproducibility of the-current component for the gate voltage scan, the change of oxide charge during the gate voltage scan, and the resistance of the current component to thermal annealing, it has been found that time-decay stress-induced leakage current is composed of five current components, regardless of stress type. Trap models corresponding to each current component have been proposed. In addition, it has also been proven that holes generate the electron traps related to one of those current components  相似文献

Rats were exposed prenatally to the coplanar congener 3,3',4,4'-tetrachlorobiphenyl (TCB). The amount of long-term potentiation (LTP) was measured in slices from the visual cortex and hippocampus of the adult males as well as of controls. While in the cortical slices from the controls a stable LTP could be induced, LTP was inhibited in the TCB-exposed rats. In hippocampal slices, the amount of LTP was not significantly different between the two groups. Our results suggest that the visual cortex of adult animals is functionally altered by TCB if the exposure takes place during embryonic development.  相似文献

In the kidneys of anti-glomerular basement membrane (anti-GBM) antibody disease, binding of antibodies to tubular basement membrane (TBM) is often observed. The present work was performed to explore the mechanisms of binding of anti-GBM antibodies to TBM in vivo with special reference to 5I2Ag, a rat membrane inhibitor of complement which regulates complement activation at C3 convertase level. To suppress functions of renal 5I2Ag, F(ab')2 fragment of 5I2 (a neutralizing mAb against 5I2Ag) was perfused in the left kidney and then blood circulation was restored. Mild proteinuria ( < 10 mg/16 hr) was observed during first several days. Five days later, there were tubulointerstitial injuries defined by tubular vimentin staining and leukocyte infiltration. Significant deposition of C3 was observed in the capillaries and in TBM. In rats intravenously injected with rabbit anti-rat GBM antibodies five minutes after kidney perfusion with 5I2, strong binding of rabbit IgG to TBM was observed at one and five days after injection. Although these rats showed mild proteinuria comparable to those perfused with 5I2 and those injected with normal rabbit serum, tubulointerstitial injury was significantly enhanced at Day 5. In contrast, rats perfused with irrelevant mAb and injected with anti-GBM antibodies did not show any significant binding of antibodies to TBM nor tubulointerstitial injury. Furthermore, rats which were made proteinuric by puromycin aminonucleoside and injected with anti-GBM antibodies did not show any significant binding of rabbit IgG to TBM. These results indicate that 5I2Ag, a rat membrane inhibitor of complement at the C3 convertase level, regulates vascular permeability in the living kidney, and that dysfunction or decreased expression of this molecule leads to increased accessibility of anti-GBM antibodies to TBM.  相似文献
