We investigated the clinical usefulness of individualization of chemotherapeutic regimen in neoadjuvant intra-arterial chemotherapy for locally invasive bladder cancer. Anticancer drugs were selected according to the results of an in vitro chemosensitivity test (collagen matrix assay or succinic dehydrogenase inhibition test). Nine patients with locally invasive bladder cancer received 1 to 4 courses of neoadjuvant intra-arterial chemotherapy, followed by radical cystectomy. Histopathological responses in the cystectomized specimens were grade 3 in 3 cases, grade 2 in 2, grade 1b in 2 and no response in 2. Pathologically, a complete response and downstaging were observed in 3 and 4 cases, respectively. Seven of the 9 patients were alive no evidence of disease with a mean follow-up period of 38.9 months, whereas 2 patients died of metastasis within 2 years. Six of the 7 patients who showed a complete response or down staging have been free of recurrence. These findings suggest that our chemotherapeutic strategy may improve the prognosis for locally invasive bladder cancer.  相似文献

A novel subset of peripheral T cells, peripheral NK T cells, is found to be a major population comprising 5% of splenic T and 40% of bone marrow T cells. The majority of peripheral NK T cells are characterized by the expression of an invariant TCR-alpha encoded by V alpha 14/J alpha 281 with a one nucleotide N region. Moreover, a specific reduction of V alpha 14+ NK T cells has been demonstrated to be tightly associated with various autoimmune diseases, indicating their decisive role in autoimmune disease development. In this study, we investigated the phenotypes of peripheral V alpha 14+ NK T cells and their TCR-beta repertoire. Peripheral V alpha 14+ NK T cells, comprise two populations, i.e., small and large sized cells, at an equal frequency, belonged to the CD4- CD8- fraction, and are heat stable antigen(bright), macrophage-1bright, B220bright, CD45RBdim, and Mel-14dim, but CD5-, distinct from thymic NK T cells. TCR-beta analysis clearly showed that peripheral V alpha 14+ NK T cells utilized two to three dominant invariant TCR-beta, such as V beta 8.2 D beta J beta 2.5/V beta 7 D beta J beta 2.1 in the spleen and liver, V beta 8.2 D beta J beta 2.5/V beta 8.3 D beta J beta 2.2/V beta 7 D beta J beta 2.6 in the bone marrow, and V beta 7 D beta J beta 2.1/V beta 3 D beta J beta 1.2 in intestinal intraepithelial lymphocytes. Judging from the unusual surface phenotypes, such as heat stable antigen, macrophage-1, B220, CD45RBdim, and Mel-14dim, which are known to be T cell activation markers, peripheral V alpha 14+ NK T cells may always be activated under physiologic conditions, resulting in the oligoclonal expansion of V alpha 14+ NK T cells with different invariant TCR-beta in different peripheral organs. The prevalence of normal tension glaucoma (NTG) or primary open angle glaucoma (POAG) is higher in myopic populations and the prevalence of myopia and NTG is relatively high in the Japanese. To evaluate the influence of myopic refractive error on the central visual field defect in NTG eyes, the data obtained from NTG eyes with the Humphrey Visual Field Analyser were analysed in comparison with those from POAG eyes. One hundred and twenty NTG eyes and one hundred and ninety-seven POAG eyes were included. All of them had clear ocular media, but no myopic degeneration in the fundus. Multiple linear regression analysis was performed with a dependent variable of the deviation of the measured threshold value from normal reference value (total deviation, STATPAC) and independent variables of refractive error and mean deviation (STATPAC). Myopic power was found to be positively correlated with the depression in the lower papillo-macular area in both NTG and POAG eyes, and it was negatively correlated with the depression in an upper arcuate area extending just nasal to the fixation only in NTG eyes.  相似文献

Grain boundaries in polycrystals have a prominent impact on the properties of a material, therefore stimulating the research on grain boundary engineering. Structure determination of grain boundaries of molecule-based polycrystals with submolecular resolution remains elusive. Reducing the complexity to monolayers has the potential to simplify grain boundary engineering and may offer real-space imaging with submolecular resolution using scanning tunneling microscopy (STM). Herein, the authors report the observation of quasi-periodic nanoscale chirality switching in self-assembled molecular networks, in combination with twinning, as revealed by STM at the liquid/solid interface. The width of the chiral domain structure peaks at 12–19 nm. Adjacent domains having opposite chirality are connected continuously through interdigitated alkoxy chains forming a 1D defect-free domain border, reflecting a mirror twin boundary. Solvent co-adsorption and the inherent conformational adaptability of the alkoxy chains turn out to be crucial factors in shaping grain boundaries. Moreover, the epitaxial interaction with the substrate plays a role in the nanoscale chirality reversal as well.  相似文献

Thermally activated delayed fluorescence (TADF) materials, which enable the full harvesting of singlet and triplet excited states for light emission, are expected as the third‐generation emitters for organic light‐emitting diodes (OLEDs), superseding the conventional fluorescence and phosphorescence materials. High photoluminescence quantum yield (ΦPL), narrow‐band emission (or high color purity), and short delayed fluorescence lifetime are all strongly desired for practical applications. However, to date, no rational design strategy of TADF emitters is established to fulfill these requirements. Here, an epoch‐making design strategy is proposed for producing high‐performance TADF emitters that concurrently exhibiting high ΦPL values close to 100%, narrow emission bandwidths, and short emission lifetimes of ≈1 µs, with a fast reverse intersystem crossing rate of over 106 s?1. A new family of TADF emitters based on dibenzoheteraborins is introduced, which enable both doped and non‐doped TADF‐OLEDs to achieve markedly high external electroluminescence quantum efficiencies, exceeding 20%, and negligible efficiency roll‐offs at a practical high luminance. Systematic photophysical and theoretical investigations and device evaluations for these dibenzoheteraborin‐based TADF emitters are reported here.  相似文献

In recent years, serious damage has been done to retaining structures because of large earthquakes. In order to establish practical methods for evaluating the seismic earth pressure, which is one of the important external forces acting on retaining structures during large earthquakes, a series of shaking table tests was conducted on retaining wall (RW) models. The experiments revealed that the seismic active earth pressure was considerably smaller than that obtained by the Mononobe-Okabe theory, particularly under a large seismic load. Furthermore, it was demonstrated that the seismic earth pressure had an upper limit, which was determined by the force equilibrium of the soil wedge at the critical state when the RW lost its stability. On the basis of the test results, a new method to evaluate the seismic earth pressure for practical designs under a large seismic load has been suggested. This proposed method provides a reasonable earth pressure as well as an angle of failure plane, those of which depend on the seismic stability of the retaining wall. It has been confirmed that earth pressure obtained by the proposed method agrees well with the measured seismic earth pressure exerted on several retaining walls with different degrees of stability.  相似文献

Tomakomai Large Scale Crude Oil Fire Experiments   总被引:6,自引:0,他引:6  
This paper summarizes the results of large-scale crude oil fire experiments conducted in Tomakomai, Japan, in 1998 to obtain information that could be applied to the development of firefighting strategies for, and the design of, huge petroleum storages. Arabian light-equivalent crude oil was burned in pans 5-, 10-, and 20-m in diameter. Most of the experiments were performed under favorable conditions. Measured data include external radiation, infrared image of the flame, flame temperature, gas concentration inside the flame, and other burning characteristics. The height of the strongest radiant emittance was H/D=0.1 to 0.2, where D=pan diameter and H=height from the initial fuel surface, and a kind of fireball appeared occasionally at the intermittant flame zone. Emitted smoke particles were sampled on the ground and observed with a scanning electron micrograph, and the distribution of the diameters of primary smoke particles was examined. The average diameter of primary smoke particles is 53.0 (±10.5)nm. The dependence of burning characteristics and flame structure on pan diameter is discussed. The flame height of the 20-m diameter pan fire is 1.9 (±0.3) D. The burning rate increases as the pan diameters increase, but the radiative fraction decreases as pan diameter increases.  相似文献

Modeling an embankment by subjecting it to different upstream conditions in a centrifuge is challenging. However, the response of an embankment to shaking under different upstream conditions needs to be studied to ensure that the necessary precautions are taken during its construction and maintenance. Herein, the influence of different upstream conditions and embankment densities are investigated. The pore water pressure values at different locations in an embankment were recorded during seepage and under induced shaking. Accelerometers recorded the response of the embankment to shaking, and a linear variable differential transformer (LVDT) measured the settlement at the top of the embankment as a result of the shaking. An image analysis was used to trace the resulting deformation of the embankment due to the shaking. The results indicate that the upstream conditions of the embankment determine the failure mechanism of the embankment in cases of lower density. In addition, the distribution of the water content within the embankment was found to have a greater influence on its response to shaking than the water level height on its upstream side.  相似文献
