Short hospitalizations for patients undergoing coronary artery bypass grafting (CABG) require continuous nursing evaluation of patients' discharge education. Six institutions collaborated in surveying 300 postoperative patients with CABG to identify learning priorities and patients' perceptions of the effectiveness of discharge education. Data analysis from the self-administered questionnaire demonstrated consistent patient priorities across institutions. Differences in teaching methods and content did not affect perceived preparedness or importance scores. Regional experience demonstrates that variable teaching efforts meet patients' priorities and provide high overall patient preparedness for discharge. Patients with the shortest hospitalizations had higher preparedness scores. 相似文献

OBJECTIVE: We examined the effects of aprotinin on graft patency, prevalence of myocardial infarction, and blood loss in patients undergoing primary coronary surgery with cardiopulmonary bypass. METHODS: Patients from 13 international sites were randomized to receive intraoperative aprotinin (n = 436) or placebo (n = 434). Graft angiography was obtained a mean of 10.8 days after the operation. Electrocardiograms, cardiac enzymes, and blood loss and replacement were evaluated. RESULTS: In 796 assessable patients, aprotinin reduced thoracic drainage volume by 43% (P < .0001) and requirement for red blood cell administration by 49% (P < .0001). Among 703 patients with assessable saphenous vein grafts, occlusions occurred in 15.4% of aprotinin-treated patients and 10.9% of patients receiving placebo (P = .03). After we had adjusted for risk factors associated with vein graft occlusion, the aprotinin versus placebo risk ratio decreased from 1.7 to 1.05 (90% confidence interval, 0.6 to 1.8). These factors included female gender, lack of prior aspirin therapy, small and poor distal vessel quality, and possibly use of aprotinin-treated blood as excised vein perfusate. At United States sites, patients had characteristics more favorable for graft patency, and occlusions occurred in 9.4% of the aprotinin group and 9.5% of the placebo group (P = .72). At Danish and Israeli sites, where patients had more adverse characteristics, occlusions occurred in 23.0% of aprotinin- and 12.4% of placebo-treated patients (P = .01). Aprotinin did not affect the occurrence of myocardial infarction (aprotinin: 2.9%; placebo: 3.8%) or mortality (aprotinin: 1.4%; placebo: 1.6%). CONCLUSIONS: In this study, the probability of early vein graft occlusion was increased by aprotinin, but this outcome was promoted by multiple risk factors for graft occlusion. 相似文献

Geologists interpret seismic data to understand subsurface properties and subsequently to locate underground hydrocarbon resources. Channels are among the most important geological features interpreters analyze to locate petroleum reservoirs. However, manual channel picking is both time consuming and tedious. Moreover, similar to any other process dependent on human intervention, manual channel picking is error prone and inconsistent. To address these issues, automatic channel detection is both necessary and important for efficient and accurate seismic interpretation. Modern systems make use of real-time image processing techniques for different tasks. Automatic channel detection is a combination of different mathematical methods in digital image processing that can identify streaks within the images called channels that are important to the oil companies. In this paper, we propose an innovative automatic channel detection algorithm based on machine learning techniques. The new algorithm can identify channels in seismic data/images fully automatically and tremendously increases the efficiency and accuracy of the interpretation process. The algorithm uses deep neural network to train the classifier with both the channel and non-channel patches. We provide a field data example to demonstrate the performance of the new algorithm. The training phase gave a maximum accuracy of 84.6% for the classifier and it performed even better in the testing phase, giving a maximum accuracy of 90%. 相似文献

A series of (Cd1–x

)S powders phosphors and thin layers prepared by thermal evaporation of solid solution were studied. The phosphors used were 41% ZnS: 59% CdS with a cobalt concentration from 0 to 0.325%. The analysis of the structure of films of different thicknesses using X-ray diffraction technique confirms that the calculated relative intensities of the planes show considerable differences from the experimental results. For film thicknesses 70 nm thea axis is normal to the substrate, while at greater thicknesses (273 nm) thec axis is practically normal to the substrate. The effect of the electron beam on the solid solution indicates that layers decomposed leaving the grain boundaries decorated by metallic cadmium and zinc particles. 相似文献

The limited effect of current medications on neuropathic pain (NP) has initiated large efforts to develop effective treatments. Animal studies showed that glycine transporter (GlyT) inhibitors are promising analgesics in NP, though concerns regarding adverse effects were raised. We aimed to study NFPS and Org-25543, GlyT-1 and GlyT-2 inhibitors, respectively and their combination in rat mononeuropathic pain evoked by partial sciatic nerve ligation. Cerebrospinal fluid (CSF) glycine content was also determined by capillary electrophoresis. Subcutaneous (s.c.) 4 mg/kg NFPS or Org-25543 showed analgesia following acute administration (30–60 min). Small doses of each compound failed to produce antiallodynia up to 180 min after the acute administration. However, NFPS (1 mg/kg) produced antiallodynia after four days of treatment. Co-treatment with subanalgesic doses of NFPS (1 mg/kg) and Org-25543 (2 mg/kg) produced analgesia at 60 min and thereafter meanwhile increased significantly the CSF glycine content. This combination alleviated NP without affecting motor function. Test compounds failed to activate G-proteins in spinal cord. To the best of our knowledge for the first time we demonstrated augmented analgesia by combining GlyT-1 and 2 inhibitors. Increased CSF glycine content supports involvement of glycinergic system. Combining selective GlyT inhibitors or developing non-selective GlyT inhibitors might have therapeutic value in NP. 相似文献

Hybrid ion exchange electrodialysis processes allow the removal of metal ions from dilute waste liquids and the recovery of more concentrated solutions. The work reported here was aimed at investigating the two steps in the treatment process, namely, adsorption of metal ions onto the packed bed of resin and electromigration (i.e., the transport of these ions in the complex system under the applied electrical field). The case of copper sulfate was investigated. Dowex resins with a cross-linking degree of 2 and 8% were used. The flux of copper through the resin bed and the current efficiency for ion transfer to the cathode compartment were determined as a function of potential gradient and copper ionic fraction in the bed. Apparent diffusion coefficients of Cu2+ in the overall system were deduced from the experimental data. 相似文献

Electrocopolymerization of a binary mixture of 3‐chloroaniline and 2‐amino‐4‐phenylthiazole on platinum electrode in acid medium was carried out under different reaction conditions such as temperature, current density, hydrochloric acid, and monomer concentrations with duration time. The initial rate of the electrocopolymerization reaction on platinum electrode is small and the rate law is Rp = K2 [D]1.29[HCl]0.97[M]1.94. The apparent activation energy is found to be 38.87 kJ/mol. The obtained copolymer film is characterized by 1H‐NMR, elemental analysis, GPC IR, UV‐visible, and cyclic voltammetry and compared with those of the two homopolymers. The mechanism of the electrocopolymerization reaction is also discussed and the monomer reactivity ratio (r1and r2) is calculated. The thermogravimetric analysis (TGA) is used to confirm the proposed structure and determination of the number of water molecules in the polymeric chain unit. The widespread availability of broadband internet access and the growth in server-based processing have provided an opportunity to run games away from the player into the cloud and offer a new promising service known as cloud gaming. The concept of cloud gaming is to render a game in the cloud and stream the resulting game scenes to the player as a video sequence over a broadband connection. To meet the stringent network bandwidth requirements of cloud gaming and support more players, efficient bit rate reduction techniques are needed. In this paper, we introduce the concept of game attention model (GAM), which is basically a game context-based visual attention model, as a means for reducing the bit rate of the streaming video more efficiently. GAM estimates the importance of each macro-block in a game frame from the player’s perspective and allows encoding the less important macro-blocks with lower bit rate. We have evaluated nine game video sequences, covering a wide range of game genre and a spectrum of scene content in terms of details, motion and brightness. Our subjective assessment shows that by integrating this model into the cloud gaming framework, it is possible to decrease the required bit rate by nearly 25 % on average, while maintaining a relatively high user quality of experience. In efforts to contribute to community development, particularly in the context of Egyptian communities, waste plastics materials were successfully recycled without the difficult task of separation and reused to economically produce new structural material. Recycling was performed by mixing molten waste plastics with sand to produce these new materials. Samples with different percentages of plastics and different particle sizes of sand were used in the process. Materials showed acceptable density and high compressive strength, which was shown to be at a maximum with contents of about 30–40% waste plastic. Furthermore, certain types of sand having different colors were used to produce attractive materials, suitable for decorative uses. © 2003 Wiley Periodicals, Inc.
