BACKGROUND: A durable power of attorney for health care (DPA) allows a person to appoint a surrogate decision-maker for any future period of mental incapacity. The absence of advance directives can lead to confusion and the expenditure of resources while trying to exert a substituted judgment. METHODS: The Wisconsin DPA was presented with an organized pilot program to 150 residents who had been judged by their social workers to have the capacity to make informed decisions regarding medical care. The reasons residents gave for accepting or rejecting a DPA were analyzed. RESULTS: Seventy-nine percent prepared a DPA. Reasons for signing included allowing the resident to decide who would make medical decisions and assuring that specific wishes would be carried out. Twenty-one percent did not execute a DPA. Reasons were categorized as confusion and misunderstanding regarding the legal system, mistrust, or social isolation. CONCLUSIONS: The high rate (79%) of DPA completion is probably related to individually counseling residents. However, competent residents who despite counseling do not choose to execute a DPA can have detailed advance directives ("living wills") prepared without appointing a decision-maker.  相似文献

The frequent difficulties encountered in the diagnosis of pediatric sarcomas, caused by the lack of observable differentiation at the light microscopic level, has led to the routine use of immunohistochemistry in pediatric surgical pathology. To a large degree the advent of this staining technique has led to the correct assessment of many perplexing lesions that previously would have been given inconclusive diagnoses. However, with increased usage and testing, it has become apparent that there are few, if any, "magic bullets" in immunohistochemistry for pediatric pathologists. Thus, it behooves diagnosticians to be careful in the usage of this technique, to be aware of possible discrepancies in its results, and to remember the ancillary nature of its application. The following article will review selected markers commonly used in pediatric surgical pathology, from both previous reports and the author's perspective, and will briefly consider several new phenotypic markers which have potential utility with childhood sarcomas.  相似文献

Isoenzymes of creatine kinase (ATP:creatine phosphotransferase; EC 2.7.3.2; CK) were measured by electrophoresis in serum from cord blood and skin-puncture blood taken from 45 healthy full-term infants during the first three postnatal days. Mean total CK activities (in U/L at 30 degrees C) were 185 in cord samples, 536 in samples taken between 5--8 h postnatally, 494 between 24--33 h, and 288 in the 72-100 h samples. Values for all three isoenzymes increased to a peak over this period, with the highest values generally being found in the samples taken 5--33 h after birth; the subsequent decline was most rapid for CK-BB. Serum CK isoenzymes in cord samples and those taken at 72--100 h in the 11 babies delivered by cesarian section did not differ significantly from those of babies delivered vaginally. However the postnatal increases in total CK, CK-MM, and CK-MB (but not in CK-BB) were significantly greater in those patients born by vaginal delivery. The reasons for the increases in CK isoenzymes after birth are not clear, but our results and reported studies on the ontogeny of CK suggest that CK-MB cannot be regarded as a "cardiac-specific" isoenzyme in the neonatal period.  相似文献

The present paper reports the preparation of full IPNs of gelatin and polyacrylonitrile. Various compositions of gluteraldehyde crosslinked gelatin and N,N′-methylene-bis-acrylamide crosslinked PAN were characterized by SEM and DSC techniques. The IPNs were also thermally pretreated by the annealing process. The effects of annealing temperature on the microhardness of IPNs were studied using the Vickers method. SEM indicates the homogeneous morphological features for IPN. The role of gelatin, AN and crosslinker on the developed hard biopolymer has been described with the help of DSC thermograms and microhardness measurements of annealed specimens and good correlation is observed.  相似文献

Biofuel cells (BFCs) are the devices made to transform the chemical energy of organic matter to electrical energy utilizing metabolic reactions occurring in microorganisms during degradation of organic contaminants. In spite of having many applications such as waste water treatment, biosensors and portable uses of BFCs, promoting the uses of BFCs is very challenging because of short life-time and low-power density. Most of the BFC developed till date is only capable to fulfill energy needs of biomedical short-term implanted devices. Use of materials with nano dimensions in the construction of BFCs has been studied extensively and reported as a worthwhile strategy to increase its efficiency. Usually, it is difficult to achieve efficient electron transfer on planar electrode from biocatalyst due to its non-specific orientational the interface. Nonmaterials provide close wiring for the electron transfer between biocatalyst and electrode. Use of various nanomaterials is the most effective way to decrease the gap between active sites (electron producing area)deep inside the enzyme or proteins and the electrodes to achieve better electron transfer. Also, various nanomaterials are utilized to improve the membrane materials for better electron barrier. Many carbon nanostructures, conducting polymers, metal and metal oxides are promising nonmaterials to enhance the current output from BFC. This review highlights recent progress registered in the development of various nanomaterials for construction of electrode and membranes of biofuel cells for better efficiency. It also emphasized the utilization of different metallic nanomaterials, inorganic nanomaterials, conducting polymer-based nanomaterials and carbon-based nanomaterials such as graphene, fullerenes, and carbon nanotubes.  相似文献

The present study introduces an efficient algorithm for automatic segmentation and detection of mass present in the mammograms. The problem of over and under-segmentation of low-contrast mammographic images has been solved by applying preprocessing on original mammograms. Subtraction operation performed between enhanced and enhanced inverted mammogram significantly highlights the suspicious mass region in mammograms. The segmentation accuracy of suspicious region has been improved by combining wavelet transform and fast fuzzy c-means clustering algorithm. The accuracy of mass segmentation has been quantified by means of Jaccard coefficients. Better sensitivity, specificity, accuracy, and area under the curve (AUC) are observed with support vector machine using radial basis kernel function. The proposed algorithm is validated on Mini-Mammographic Image Analysis Society (MIAS) and Digital Database for Screening Mammography (DDSM) datasets. Highest 91.76% sensitivity, 96.26% specificity, 95.46% accuracy, and 96.29% AUC on DDSM dataset and 94.63% sensitivity, 92.74% specificity, 92.02% accuracy, and 95.33% AUC on MIAS dataset are observed. Also, shape analysis of mass is performed by using moment invariant and Radon transform based features. The best results are obtained with Radon based features and achieved accuracies for round, oval, lobulated, and irregular shape of mass are 100%, 70%, 64%, and 96%, respectively.  相似文献

Abstract

Surfactant-based fracturing gels are considered as clean gels due to the absence of insoluble residues after hydraulic fracturing of tight reservoir. Conventionally used highly viscous polymer gels damage the formation by blocking the fluid path in a fractured formation. The present work has been accomplished with the purpose of evaluating an anionic surfactant-based gel for hydraulic fracturing. Rheological properties and proppant carrying capacity of the synthesized gel were studied as a function of shear rate, temperature, and pH, which affect the gel behavior at the downhole. Proppant carrying capacity was also reported with varying proppant loadings. Rheological properties and proppant carrying capacity of synthesized gel were compared with conventionally used guar gel.  相似文献

Password‐based two‐party authenticated key exchange (2PAKE) protocol enables two or more entities, who only share a low‐entropy password between them, to authenticate each other and establish a high‐entropy secret session key. Recently, Zheng et al. proposed a password‐based 2PAKE protocol based on bilinear pairings and claimed that their protocol is secure against the known security attacks. However, in this paper, we indicate that the protocol of Zheng et al. is insecure against the off‐line password guessing attack, which is a serious threat to such protocols. Consequently, we show that an attacker who obtained the users' password by applying the off‐line password guessing attack can easily obtain the secret session key. In addition, the protocol of Zheng et al. does not provide the forward secrecy of the session key. As a remedy, we also improve the protocol of Zheng et al. and prove the security of our enhanced protocol in the random oracle model. In this investigation, cryogels composed of poly(vinyl alcohol) (PVA) were prepared by repeated freeze‐thaw method. The prepared cryogels served as templates for producing highly stable and uniformly distributed silver nanoparticles via in situ reduction of silver nitrate (AgNO3) using alkaline formaldehyde solution as reducing agent. The structure of the PVA/Ag cryogel nanocomposites was characterized by a Fourier transform infrared and Raman spectroscopy. The morphologies of pure PVA cryogels and PVA/Ag nanocomposites were observed by a scanning electron microscopy (SEM) and transmission electron microscopy (TEM) techniques. The SEM analysis suggested that cryogels show a well defined porous morphology whereas TEM micrographs revealed the presence of nearly spherical and well separated Ag nanoparticles with diameter about 100 nm. XRD results showed all relevant Bragg's reflections for crystal structure of silver nanoparticles. The amount of silver in cryogel nanocomposites and thermal stability were determined by inductively coupled plasma atomic emission spectrometry (ICP‐AES) and thermogravimetric analysis measurements. Mechanical properties of nanocomposites were observed in terms of tensile strength. The antibacterial studies of the synthesized nanosilver containing cryogels showed good antibacterial activity against both gram‐negative and gram‐positive bacteria. The prepared PVA/Ag nanocomposites were also investigated for swelling and deswelling behaviors. The results reveal that both the swelling and deswelling process depends on the chemical composition of the cryogel silver nanocomposites, number of freeze‐Thaw cycles and pH and temperature of the swelling medium. The biocompatibility of the prepared nanocomposites was judged by in vitro methods of percent hemolysis and protein (BSA) adsorption. POLYM. In the present study, poly(vinyl alcohol) (PVA)/Carrageenan(Car) composite films have been prepared by ionic cross-linking of the two polymers with Borax (Bx) and K+ ions, respectively, in aqueous medium at room temperature. The films were characterized by FTIR, SEM, TGA, and AFM analysis. The equilibrium percent swelling (EPS) of PVA/Carr films, so produced, exhibited negative dependence on the amount of cross-linkers. A detailed investigation of their water absorption and moisture permeation behavior has been carried out. The films showed non-thrombogenic and non-cytotoxic behaviors. Finally, drug minocycline-loaded films showed excellent antimicrobial behavior.  相似文献
