BACKGROUND: Most resectable pancreatic or peripancreatic neuroendocrine tumors are treated by enucleation or distal pancreatectomy. A minority of tumors may require pancreaticoduodenectomy for complete tumor excision because of their large size, location, or lymph node involvement. METHODS: This study reviews the management of 50 patients treated by pancreaticoduodenectomy for periampullary neuroendocrine tumors between 1962 and 1996 at a single institution. RESULTS: There were 30 men and 20 women with a mean age of 52 +/- 2 years. Functional tumors were resected in 17 patients: insulinoma, seven tumors; gastrinoma, eight tumors; vipoma, one tumor; and glucagonoma, one tumor. Tumors were classified as malignant in 29 patients and benign in 21. The median intraoperative blood loss was 800 ml, and the median number of units of blood transfused was zero. The postoperative length of stay was 20 +/- 2 days. Postoperative morbidity included 11 patients (24%) with a pancreatic fistula and four patients (8%) with a biliary fistula. There was one in-hospital death (2%), in 1967. The actuarial survival rates at 2, 5, and 7 years are 81%, 73%, and 65%, respectively. Patients with benign tumors had a significantly improved 5-year survival rate (94%) compared with those with malignant tumors (61%; p = 0.03). CONCLUSIONS: Selected patients with periampullary neuroendocrine tumors can be managed successfully by pancreaticoduodenectomy, with low mortality and acceptable morbidity rates.  相似文献

Increased activity of the Na(+)-H+ exchanger (NHE-1 isoform) has been observed in cells and tissues from hypertensive humans and animals, including the spontaneously hypertensive rat (SHR). No mutation in NHE-1 DNA sequence or alteration in NHE-1 mRNA and protein expression has been demonstrated in hypertension, indicating that alterations in proteins that regulate NHE-1 activity are responsible for increased activity. The recent finding that NHE-1 phosphorylation in SHR vascular smooth muscle cells (VSMCs) was greater than in Wistar-Kyoto rat (WKY) VSMCs suggested that NHE-1 kinases may represent an abnormal regulatory pathway present in hypertension. To define NHE-1 kinases altered in the hypertensive phenotype. We measured NHE-1 kinase activity by an in-gel-kinase assay using a recombinant glutathione S-transferase NHE-1 fusion protein as a substrate. At least 7 NHE-1 kinases (42 to 90 kD) were present in VSMCs. We studied a 90-kD kinase because it was the major NHE-1 kinase and exhibited differences between SHR and WKY. Comparison of 90-kD kinase activity revealed that SHR VSMCs had increased activity in growth-arrested cells and in cells stimulated by angiotensin II (100 nmol/L for 5 minutes). Activation of the 90-kD kinase by angiotensin II was Ca2+ dependent, PKC independent, and partially dependent on the mitogen-activated protein kinase pathway. These findings indicate that increased activity of a 90-kD NHE-1 kinase is a characteristic of SHR VSMCs in culture and suggest that alterations in the 90-kD NHE-1 kinase and/or proteins that regulate its activity may be a pathogenic component in hypertension in the SHR.  相似文献

Intermediate products of the metabolism of glucose, fat and amino-acid are important in the evaluation of such metabolic disorders as diabetes mellitus, liver disease and metabolic acidosis. In the present study, methods for the measurement of intermediate metabolites (lactate, pyruvate, alanine, β-hydroxybutyrate and glycerol) have been adapted to a fast centrifugal analyzer: the COBAS FARA. Correlation coeffcients rangedfrom 0.90 to 0.99, compared to established manual spectrophotometric methods. Within-run coeffcients of variation (CVs) ranged between 2.9 and 8.8% at low levels, between 1.5 and 5.7% at medium levels and between 1.2 and 5.6% at high levels. Between-run CVs were between 4.0 and 15.0% at low levels, between 1.7 and 7.0% at medium levels and between 1.3 and 2.7% at high levels. These fluorimetric assays for the determination of intermediate metabolites on COBAS FARA (Roche) have a good sensitivity and precision, are less costly than manual methods and can be used on a routine basis.  相似文献

Production scheduling involves all activities of building production schedules, including coordinating and assigning activities to each person, group of people, or machine and arranging work orders in each workplace. Production scheduling must solve all problems such as minimizing customer wait time, storage costs, and production time; and effectively using the enterprise’s human resources. This paper studies the application of flexible job shop modelling on scheduling a woven labelling process. The labelling process includes several steps which are handled in different work-stations. Each workstation is also comprised of several identical parallel machines. In this study, job splitting is allowed so that the power of work stations can be utilized better. The final objective is to minimize the total completion time of all jobs. The results show a significant improvement since the new planning may save more than 60% of lead time compared to the current schedule. The contribution of this research is to propose a flexible job shop model for scheduling a woven labelling process. The proposed approach can also be applied to support complex production scheduling processes under fuzzy environments in different industries. A practical case study demonstrates the effectiveness of the proposed model.  相似文献

Salivary gland dysfunction induces salivary flow reduction and a dry mouth, and commonly involves oral dysfunction, tooth structure deterioration, and infection through reduced salivation. This study aimed to investigate the impact of aging on the salivary gland by a metabolomics approach in an extensive aging mouse model, SAMP1/Klotho -/- mice. We found that the salivary secretion of SAMP1/Klotho -/- mice was dramatically decreased compared with that of SAMP1/Klotho WT (+/+) mice. Metabolomics profiling analysis showed that the level of acetylcholine was significantly decreased in SAMP1/Klotho -/- mice, although the corresponding levels of acetylcholine precursors, acetyl-CoA and choline, increased. Interestingly, the mRNA and protein expression of choline acetyltransferase (ChAT), which is responsible for catalyzing acetylcholine synthesis, was significantly decreased in SAMP1/Klotho -/- mice. The overexpression of ChAT induced the expression of salivary gland functional markers (α–amylase, ZO-1, and Aqua5) in primary cultured salivary gland cells from SAMP1/Klotho +/+ and -/- mice. In an in vivo study, adeno-associated virus (AAV)-ChAT transduction significantly increased saliva secretion compared with the control in SAMP1/Klotho -/- mice. These results suggest that the dysfunction in acetylcholine biosynthesis induced by ChAT reduction may cause impaired salivary gland function  相似文献

We consider the classic problem of pole placement by state feedback. We offer an eigenstructure assignment algorithm to obtain a novel parametric form for the pole-placing feedback matrix that can deliver any set of desired closed-loop eigenvalues, with any desired multiplicities. This parametric formula is then exploited to introduce an unconstrained nonlinear optimisation algorithm to obtain a feedback matrix that delivers the desired pole placement with optimal robustness and minimum gain. Lastly we compare the performance of our method against several others from the recent literature.  相似文献

Effects of hydrodynamic conditions, current density and solution temperature on the d.c. electrodeposition of Fe–Ni–Co alloys have been investigated with stationary planar and rotating cylindrical electrodes. The deposition rate of Fe showed mass transfer effects at cathodic potentials –1.35 V/sce. Deposition of Ni appeared to be kinetically controlled; deposition of Co appeared to be under kinetic control at potentials –1.35 V/sce but under mixed control at –1.65 V. Current efficiency of the codeposition process increased with increasing current density and decreased with increasing rotation rate. Higher solution temperatures and rotation rates extended the applied current density range where smooth, adherent, and metallic-looking deposits could be obtained. An increase in solution temperatures also decreased anomalous codeposition of Fe–Ni–Co. Calculations based on the Hessami-Tobias model provide qualitative agreement with dependence of experimental electrodeposition on applied current density, hydrodynamics and temperature.  相似文献
