Salvage therapy of advanced squamous cell carcinoma of the mouth cavity and oropharynx. Results of interstitial high-dose-rate brachytherapy combined with ablative tumor surgery

PURPOSE: The clinical effect of high-dose-rate (HDR) interstitial brachytherapy combined with tumor resection was investigated in this retrospective study on patients with recurrent oral and oropharyngeal squamous cell carcinoma. PATIENTS AND METHODS: Oral and oropharyngeal squamous cell carcinoma in 38 patients were treated over a period of 7 years (1988-1994) by HDR interstitial brachytherapy using Gammamed 12i equipment. Pretreatment of patients in terms of irradiation and surgery differed (33 irradiated with a total dose between 60.0 and 75.6 Gy, including 8 patients with additional surgery; 1 patient with surgery alone, and 4 patients without any pretreatment). Indications for interstitial brachytherapy differed according to individual responses to treatment and medical histories. RESULTS: Interstitial brachytherapy was successful in the majority of patients, i.e. complete remission: 12, partial remission: 19, no change: 2, progression: 5. Local control and overall survival including patients with surgical treatment was 59% and 81% at 6 months and 47% and 49% after 12 months, respectively. CONCLUSION: Interstitial HDR brachytherapy with Iridium 192 is recommended in patients with local recurrences or second primary carcinomas after previous external radiotherapy in the head and neck region. Combination of interstitial brachytherapy and surgery is preferable for these patients.     

High‐error approximate dictionary search using estimate hash comparisons

A method for finding all matches in a pre‐processed dictionary for a query string q and with at most k differences is presented. A very fast constant‐time estimate using hashes is presented. A tree structure is used to minimize the number of estimates made. Practical tests are performed, showing that the estimate can filter out 99% of the full comparisons for 40% error rates and dictionaries of up to four million words. The tree is found to be efficient up to a 50% error rate. Copyright © 2006 John Wiley & Sons, Ltd.     

Parameter estimation using a sensor array in a Ricean fading channel

The estimation of the Direction-Of-Arrival (DOA) and the variance of the angular spread, using an array of sensors in the case of a Ricean channel is considered, using the Maximum-Likelihood, Least-Squares and Weighted Least Squares criteria. The Cramér-Rao bound is also obtained for the problem of interest. Simplification of the cost functions to reduce the dimension of the problem has been carried out and the performance of the methods has been studied based on numerical experiments. A major part of the work was carried out when K V S Hari was visiting the Department of Signals, Sensors and Systems during Jul–Sep 1995, on leave from the Indian Institute of Science

Heat transfer and pressure drop in a new type of corrugated channels

An experimental investigation of heat transfer and pressure drop from a new type of corrugated channels is presented. The investigation has been carried out for Reynolds numbers in the range of $\text{800 <}\text{Re}\text{< 5000}$ for one corrugated and one smooth channel. It is found that the heat transfer from the corrugated channel is up to 3.5 times higher than for the smooth one. The pressure drop is however large (5 – 6 times the value of a smooth channel) and it is suggested that the corrugation height and length should be altered in order to balance the increases in heat transfer and pressure drop.     

Evaluating the patient satisfaction questionnaire after inpatient treatment in phoniatrics and pediatric audiology--a contribution to quality control?]

Baker and confectioner apprentices have been included in a prospective follow-up study in the region of East Thuringia, to evaluate risk factors for the development of hand dermatitis. Starting in August 1996, the apprentices have been interviewed and examined in a standardized way right at the beginning (n=91) of their vocational training, 6 (n=79) and 12 months (n=63) later. To gain objective data, TEWL score was also assessed at 3 standardized sites on the hands. In their case histories, 3.3% (n=3) of the apprentices reported hand dermatitis. The 1st assessment after 2 to 4 weeks vocational training revealed hand eczema in 17.5% (n=16) of the individuals. At the follow-up after 6 months of training 29.1% (n=23), and after 12 month of training 27.0% (n=17), of the apprentices had hand dermatitis of mild to moderate severity. Within 6 months, the TEWL score rose significantly (p<0.001) from 11.9 g m(-2) h(-1) (-/+5.4) to 16.8 g m(-2) h(-1) (-/+9.5). After 12 months, the TEWL score was 14.9 g m(-2) h(-1) (-/+4.6). For the atopic skin diathesis (>10 points of Diepgen's atopy score), there was a significant tendency to be a predictive factor for the development of occupational hand dermatitis. TEWL score failed to correlate with the development of skin damage in our study. The same was true for respiratory atopy and metal sensitization.     

On growth and dispersal of populations

In his unpublished MA thesis of 1921, the young Hotelling invented an ingenious model of population growth and diffusion. The contribution remained widely unknown until Waldo Tobler and Alan Wilson edited it as an article in 1978. Not even then did it trigger off any outburst of contributions. Meanwhile, in 1951, Skellam invented exactly the same model for non-human populations. Unlike in economics, it was a great success in ecology. In a recent contribution Skellam is named Father of Ecological Diffusion and more than a thousand articles on the subject are listed. There were many attempts at solving the equation, but none were quite successful. The Hotelling and Skellam models both assumed a given saturation population that nature could support. In 1985 the present author suggested that an explicit production function be introduced in the Hotelling model, as man produces his own means of subsistence. Moreover it was proposed that diffusion be related to spatial differences in per capita productivity, rather than to those in population density. The present contribution is a rejoinder. It is shown that the stationary solutions to the original Hotelling model are periodic and dip into negative populations, whereas this is avoided by the modified model suggested. It can thus be used to show how agglomerative structures may evolve.
