BACKGROUND: Many experts have suggested that blunt splenic trauma in patients older than 55 years should not be managed by observation because of supposed increased fragility of the spleen and decreased physiologic reserve in elderly patients. We sought to determine the outcome of nonoperative management of blunt splenic trauma in patients older than 55 years. METHODS: For the years 1994 through 1996, data for patients with splenic injury older than 55 years from seven trauma centers in a single state were reviewed. RESULTS: Blunt splenic trauma occurred in 41 patients older than 55 years. Eight patients were excluded from further analysis because of death from massive associated injuries within 24 hours of admission. The remaining 33 patients (mean age, 72+/-10 years) were divided into two groups: immediate exploration (10 patients) and observation (23 patients). Observation of blunt splenic injury failed in 4 of 23 patients (17%). No patient deaths were related to the method of management of the splenic injury. CONCLUSIONS: Observation of the elderly patient with blunt splenic trauma has an acceptable failure rate of 17%. 相似文献

The effect of atmospheric turbulence on atmospheric optics telecommunication links employing dense wavelength division multiplexing (DWDM) systems is examined. For this purpose, transmittance obtained by using the MODTRAN (MODerate resolution atmospheric TRANsmission) code is modified by the transmittance due to turbulence when the incidence is a partially coherent optical beam. The spanned wavelengths cover the range employed in a practical DWDM system operating in the range of 1550?nm. The effect of turbulence is manifest as a decrease in the transmittance calculated by MODTRAN, being more effective when the incidence becomes less coherent. 相似文献

This study presents an investigation of transient local entropy generation rate in pulsating turbulent flow through an externally
heated pipe. The flow inlet to the pipe pulsates at a constant period and amplitude, only the velocity oscillates. The simulations
are extended to include different pulsating flow cases (sinusoidal flow, step flow, and saw-down flow) and for varying periods.
The flow and temperature fields are computed numerically with the help of the Fluent computational fluid dynamics (CFD) code,
and a computer program developed by us by using the results of the calculations performed for the flow and temperature fields.
In all investigated cases, the irreversibility due to the heat transfer dominates. With the increase of flow period, the highest
levels of the total entropy generation rates increase logarithmically in the case of sinusoidal and saw-down flow cases whereas
they are almost constant and the highest total local entropy is also generated in the step case flow. The Merit number oscillates
periodically in the pulsating flow cases along the flow time. The results of this study indicate that flow pulsation has an
adverse effect on the ratio of the useful energy transfer rate to the irreversibility rate. 相似文献

Field correlation of flat-topped beams in anisotropic non-Kolmogorov turbulent atmosphere is formulated and evaluated. Larger anisotropic factor causes higher field correlations. Smaller field correlations are seen when the transverse distance at the receiver plane increases. Smaller field correlations are observed at large off-axis transverse receiver points, which are valid for any anisotropic factor and for any power-law exponent of non-Kolmogorov turbulence. When the flat-topped beam is composed of large number of Gaussian beams, the field correlation becomes smaller. In anisotropic non-Kolmogorov turbulence, longer propagation distances, larger structure constants, smaller inner scales and smaller source sizes decrease the field correlation. Larger power law exponent of non-Kolmogorov turbulence increases the field correlations at any anisotropic factor. 相似文献

ABSTRACTThe performance of M-ary pulse position modulated (PPM) optical wireless communication (OWC) systems in atmospheric weak turbulence medium is evaluated by using adaptive optics corrections. Piston, tilt, defocus and coma components of adaptive optics corrections are applied to the avalanche photodetector (APD) type of receiver and the results are obtained depending on various turbulence and receiver parameters. The lognormal channel distribution is used to model the weak atmospheric turbulence conditions. Adaptive optics correction increases the bit-error-rate (BER) performance of an OWC system operating in atmospheric turbulence conditions. The aim of this investigation is to evaluate the treatability of synthetically prepared water with high concentration of humic substances by electrocoagulation in batch mode using iron cast electrodes. Effects of applied potential, initial humic substance concentration and supporting electrolyte type on humic substance removal efficiency were investigated. NaNO3, Na2SO4 and NaCl were used as supporting electrolyte. Among these supporting electrolytes, Na2SO4 and NaCl have provided high removal efficiencies, whereas in the experiments using NaNO3 as supporting electrolyte have been observed no flock formation. The highest removal rate is obtainable with NaCl as supporting electrolyte. Removal efficiencies for initial humic substance concentration of 500 mg L-1 with NaCl and Na2SO4 equal to 97.95% for 35 min and 92.69% for 70 min, respectively. This behavior of the system has been derived from oxidation products, available in the bulk solution, of chloride ions. When NaCl is used as supporting electrolyte, there is an advantage of providing the disinfection of water, but humic substances and chloride ions are available in the bulk solution with risk of formation undesirable organo-chlorine compounds, so the Na2SO4 is the most favorable supporting electrolyte. 相似文献

The purpose of this study was to select alternative landfill sites for Ankara based on the growing trends of Ankara toward the Gölba?? municipality, and to eventually select the best alternative through the use of decision-making tools. Geographic information systems (GIS) and multi-criteria decision analyses were employed to perform landfill site selection. Several criteria, including geology, slope, proximity to roads, availability and proximity of landfill containment material, settlement, suitability for agriculture, vegetation cover, erosion, and lineament system were gathered in a GIS environment. A weight value was assigned to each criterion by applying the pairwise comparison method and the analytical hierarchy method. An ideal point method, namely, the Technique for Order Preference by Similarity to the Ideal Solution (TOPSIS) was applied to choose the best alternative landfill site. The geotechnical properties of “Ankara clay”, which shows widespread distribution in Ankara, were reviewed and assessed for the clay's suitability as a single, compacted clay liner and as a component of a geomembrane-compacted clay composite liner for the alternative landfill site selected. The HELP model was employed in order to determine the cumulative, mean leachate head and cumulative, unitized expected leakage rate amounts through the landfill.
