BACKGROUND: Estimation of the mu-agonist opioid effect in anesthetized and paralyzed patients is often imprecise and can be obscured by concomitant administration of drugs that affect the sympathetic nervous system, such as beta-adrenergic blocking agents. As an alternative to hemodynamic measures of opioid effect, the authors tested the hypothesis that the pupillary light reflex or pupillary reflex dilation correlated with alfentanil concentrations during isoflurane anesthesia. METHODS: Six volunteers were anesthetized on 4 days with 0.8% isoflurane. Alfentanil was administered intravenously to target total plasma concentrations of 0, 25, 50, and 100 ng/ml. A 5-s tetanic electrical stimulus was applied to the skin. Pupil size and the pupillary light reflex were recorded before and after alfentanil administration, and before and for 8 min after the stimulus. RESULTS: Alfentanil exponentially impaired reflex pupillary dilation, decreasing the maximum response amplitude from 5 mm at 0 ng/ml, to 2.3 mm at 25 ng/ml, to 1.0 mm at 50 ng/ml, and finally to 0.2 mm at 100 ng/ml. In contrast, only the highest concentration of alfentanil depressed the dilation of the pupil in the first 2 s after the stimulus. Alfentanil administration had no effect on the pupillary light reflex. CONCLUSIONS: Dilation of the pupil in response to a noxious stimulus is a measure of opioid effect in isoflurane-anesthetized volunteers. In contrast, the pupillary light reflex is unaffected by alfentanil during isoflurane anesthesia. These data suggest that stimulus-induced pupillary dilation may be used to evaluate the analgesic component of a combined volatile and opioid anesthetic. 相似文献

BACKGROUND AND PURPOSE: Vulnerability to cerebral hypoxic-ischemic (H-I) insult and its relation to disruption of the blood-brain barrier were investigated in postnatal rats. METHODS: Pups of postnatal day (P) 7, P14, and P21 underwent ligation of a unilateral carotid artery and were exposed to hypoxic conditions. For the detection of early-phase deterioration, brains were perfusion-fixed 24 hours after H-I insult and examined by argyrophil III method. For the detection of later infarction, animals were fixed at 72 hours after the H-I insult. RESULTS: In either case, tissue damage was detected in the striatum, parietal cortex, and hippocampus. The vulnerability of P7 and P21 rats was remarkable, as compared with P14 rats. Although the developmental status of the vasculature was not significantly different at each age, the permeability of IgG after H-I injury was prominent in P7 rats and to a lesser extent in P14 rats. In P21 rats, however, there was little IgG leakage even 24 hours after the insult. Dexamethasone pretreatment blocked the extravasation of IgG and reduced the damaged tissue in P7 and P14 rats but not in P21 rats. Percentages of reduction in infarcted areas by the dexamethasone became smaller in proportion to ages. CONCLUSIONS: The results suggest that in younger rats vulnerability to H-I insult was in parallel with permeability of the blood-brain barrier, whereas in adults in might be more dependent on cellular vulnerability. 相似文献

A reliable method of forming very thin SiO2 films (<10 nm) has been developed by rapid thermal processing (RTP) in which in situ multiple RTP sequences have been employed. Sub-10-nm-thick SiO2 films formed by single-step RTP oxidation (RTO) are superior to conventional furnace-grown SiO2 on the SiO2 /Si interface characteristics, dielectric strength, and time-dependent dielectric-breakdown (TDDB) characteristics. It has been confirmed that the reliability of SiO2 film can be improved by pre-oxidation RTP cleaning (RTC) operated at 700-900°C for 20-60 s in a 1%HCl/Ar or H2 ambient. The authors discuss the dielectric reliability of the SiO2 films formed by single-step RTO in comparison with conventional furnace-grown SiO2 films. The effects and optimum conditions of RTC prior to RTO on the TDDB characteristics are demonstrated. Both the linear plasmids, pDHL1 (8.4 kb) and pDHL2 (9.2 kb), of Debaryomyces hansenii TK require the presence of a third linear plasmid pDHL3 (15.0 kb) in the same host cell for their replication. A 3.5 kb Bam HI-PstI fragment of pDHL1 strongly hybridized by Southern analysis to the 3.5 kb NcoI-AccI fragment of pDHL2, suggesting the importance of this conserved region in the replication of the two smaller pDHL plasmids. The 4.2 kb pDHL1 fragment containing the above hybridized region was cloned and sequenced. The results showed that the cloned pDHL1 fragment encodes a protein of 1000 amino acid residues, having a strong similarity to the DNA polymerase coded for by ORF1 of the killer plasmid pGKL1 from Kluyveromyces lactis. The catalytic and proof-reading exonuclease domains as well as terminal protein motif were well conserved as in DNA polymerases of pGKL1 and other yeast linear plasmids. Analysis of the cloned fragment further showed that pDHL1 encodes a protein partly similar to the alpha subunit of the K. lactis killer toxin, although killer activity was not known in the DHL system. Analysis of the 5' non-coding region of the two above pDHL1-ORFs reveal the presence of the upstream conserved sequence similar to that found upstream of pGKL1-ORFs. The possible hairpin loop structure was also found just in front of the ATG start codon of the pDHL1-ORFs like pGKL1-ORFs. Thus the cytoplasmic pDHL plasmids were suggested to possess a gene expression system comparable to that of K. lactis plasmids. 相似文献

A 100-channel optical frequency-division multiplexing (FDM) transmission/distribution experiment at 622 Mb/s is demonstrated for a fiber length of 50 km. The feasibility of a polarization-insensitive waveguide frequency selection switch for 10-GHz intervals and a frequency-shift-keying (FSK) direct-detection scheme employing a Mach-Zehnder filter is verified. The demodulation circuit employs a Mach-Zehnder filter and a balanced receiver, which utilizes optical power more efficiently than the Fabry-Perot filter. Degradation behavior and reliability of 1.55 μm highly coherent MQW DFB laser with a long cavity are examined and clarified. The increase in spectral linewidth, due to the increase in 1/f noise and internal optical loss and the instability of the side mode, and the changes in FM response, especially the increase in FM efficiency, occur during degradation, e.g. increase in threshold current. In addition to these basic degradations, the degradation behavior of multisection DFB lasers is also clarified from the viewpoint of device characteristics, such as wavelength and tunability. A 12-year-old boy was admitted to our hospital because of abnormal shadows on a chest radiograph, slight fever, and superficial lymphadenopathy. Laboratory examination showed anemia (Hb 9.9 g/dl) and hyperimmunoglobulinemia (IgG 5469 mg/dl) without M protein. A chest CT scan showed bilateral diffuse shadows and bilateral hilar lymphadenopathy. Biopsy specimens of an inguinal lymph node and a lung showed many lymphoid follicles with germinal centers, and marked infiltration of mature plasma cells in the interfollicular area without destruction of follicular structures. The polyclonality of the plasma cells was confirmed by immunohistochemistry. The patient was not treated because these results excluded malignant disease and he was asymptomatic. At the age of 17 years, he was admitted to our hospital again because of dyspnea and a tendency to bleed. Interstitial pneumonia, hyperimmunoglobulinemia (IgG 13900 mg/dl), and anemia (Hb 6.6 g/dl) were found, along with thrombocytopenia (2.5 x 10(4)/mm3) and proteinuria. The serum interleukin-6 level was high: 177 pg/ml. Bronchoalveolar lavage fluid contained many plasma cells. Therapy with corticosteroids and immunosuppressant medication was effective. Our diagnosis was plasma cell interstitial pneumonia as a manifestation of multicentric Castleman's disease. 相似文献

A powerful numerical solution is presented for bandpass filters in rectangular waveguides. The filters are composed of cascading of perfectly conducting inductive obstacles with zero thickness: asymmetrical windows, symmetrical windows, or symmetrical strips. The problem is reduced to a set of singular integral equations, which is solved by an extension of Lewin's quasi‐static procedure. After the accuracy of the classical formulas for the equivalent reactance is investigated, the simple inverse formulas are constructed to yield proper window widths for desired equivalent parameters. Nanoscale molecular cavity in crystalline polymer membranes studied by molecular dynamics simulation
The size, shape, and connectivity of cavities in the crystals of syndiotactic polystyrene were investigated by molecular dynamics simulation. Cluster analysis of the free volume in the crystals clearly reveals the cavity structures: large individual holes are in an orderly manner connected by narrow channels. We call such a cavity structure a ‘molecular cavity’. The diffusion behavior and solubility of gases in the molecular cavity were also simulated. The extremely high solubility of a larger gas and the controllable diffusion path in the narrow channels proved the applicability of the concept of the molecular cavity to high performance separation membranes. 相似文献
