OBJECTIVE: To explore whether there are different disease patterns of rheumatoid arthritis (RA) in women and men. METHODS: We studied 55 male case patients and 110 female control patients who developed RA between 1970 and 1985 and who resided and received medical care in Olmsted County, Minnesota, for at least 10 years after the diagnosis of RA. Case and control patients were matched for the date of first diagnosis. The pattern and extent of joint involvement, the frequency of joint surgeries, and the presence and type of extraarticular manifestations were determined by retrospective chart review. RESULTS: Incidence rates in women were variable and age dependent, whereas the risk in men older than 36 years was constant over their lifetime. Erosive disease was more frequent in men than in women (72% versus 55%, respectively; P < 0.05) and tended to occur earlier (47% versus 31% for erosive disease within the first 4 years of RA). Although male sex was correlated with a higher risk of bony erosions and an accelerated course of RA, structural consequences of joint destruction were more pronounced in women. Joint surgery was performed more frequently in women (50%) than in men (27%) (P = 0.01). In particular, the frequencies of arthroplasties and arthrodeses of hand and foot joints were different (34 procedures in women versus 1 procedure in men; P < 0.001). Sex influenced the risk as well as the pattern of organ involvement in RA. Nodules and rheumatoid lung disease were typical manifestations in men (P = 0.001 and P < 0.001, respectively), whereas women typically developed sicca syndrome (P = 0.05). Despite differences in disease aggressiveness and disease pattern, there was little difference in the medical therapy in the men compared with the women. CONCLUSION: RA is a heterogeneous disease with variations in phenotype. Sex-associated factors influence disease severity as well as disease pattern. Because sex-related effects influence treatment goals, treatment responses, and side effects, they should be considered in clinical study design and analysis as well as in the treatment decisions for individual patients with RA.  相似文献

The presence of nitric oxide (NO) in the kidney has been implicated in the pathogenesis of human glomerulonephritis. However, the exact type of glomerular cells that express NO synthase (NOS) and the NOS isoform involved in the local production of NO has not been identified in the human diseased kidney. We examined the expression of three isoforms of NOS, inducible NOS (iNOS), endothelial NOS (eNOS) and brain NOS (bNOS) in the renal tissue of patients with IgA nephropathy (IgAN, N = 10), lupus nephritis (LN, N = 5), membranous nephropathy (MN, N = 5) and minimal change nephrotic syndrome (MCNS, N = 5). Sections were immunostained and the correlation between the expression of each NOS and the degree of glomerular injury in that section was also examined. Normal portions of surgically resected kidneys served as controls. eNOS was present in glomerular endothelial cells and endothelium of cortical vessels in the control and diseased kidneys. iNOS was localized in mesangial cells, glomerular epithelial cells and infiltrating cells in the diseased glomeruli, whereas immunostaining for iNOS was hardly detected in control kidneys. In addition, the expression pattern of eNOS in each glomerulus was the reverse of that of iNOS. In IgAN and LN, the extent of staining for eNOS correlated negatively with the degree of glomerular injury, while the extent of staining for iNOS correlated positively with the degree of glomerular injury in the same tissues. bNOS was not detected in normal or nephritic glomeruli. Our results indicate the presence of a NO pathway in human diseased kidney, and suggest that NO derived from eNOS and iNOS may be involved in the progression of renal diseases and that NO derived from each NOS may play an important role in different way in human inflamed glomeruli.  相似文献

The standard Gibbs energies of formation of Fe2Mo3O12, Fe2Mo3O8, FeMoO4, and Fe2MoO4 of the Fe-Mo-O ternary system and the μ phase of the Fe-Mo binary system have been determined by measuring electromotive forces of galvanic cells having an Y2O3-stabilized ZrO2 solid electrolyte. Functional assembly of the plasminogen-dependent proteolytic system on the cell surface requires multiple interactions involving urokinase (uPA), urokinase receptor (uPAR), plasminogen activator inhibitors, and other molecules that mediate cell migration and adhesion. We analyzed the in vitro interaction of uPAR-containing particulate cell fractions with the amino-terminal fragment (ATF) of human urokinase and the matrix-like form of vitronectin. Binding and cross-linking of 125I-labeled ATF to crude membrane extracts from LB6-19 mouse cells overexpressing human uPARs in the presence of 25 nM urea-denatured vitronectin led to the formation of Mr 137,000, 92, 000, and 82,000 covalent complexes. Immunoprecipitation of the preformed cross-linked 125I-labeled complexes with anti-vitronectin, anti-uPA, or anti-uPAR antibodies revealed that the Mr 82,000 and 92, 000 species do contain ATF and vitronectin and identified the Mr 137, 000 species as a ternary complex formed by ATF, uPAR, and vitronectin. A similar electrophoretic pattern was displayed by acid-pretreated membranes extracted from MCF-7 breast carcinoma or HT1080 fibrosarcoma cell lines, as well as a ductal breast carcinoma specimen; the latter exhibited complex formation at concentrations of vitronectin lower than 10 nM. Finally, uPAR-vitronectin interaction was further documented by the decreased reactivity of an anti-uPAR polyclonal antibody to acid-pretreated sections of 10 breast carcinomas that had been preincubated with vitronectin. Our findings highlight the ability of uPAR to interact simultaneously with vitronectin and uPA in breast cancer, supporting a dynamic coupling of the molecular mechanisms underlying plasminogen-dependent matrix degradation and cell adhesion.  相似文献

Intoxication after ingestion of toad is rare, but it results in severe morbidity and high mortality. Recently, we encountered 2 children, one a 15-month-old boy and one a 20-month-old girl, who were fed cooked toad (Bufo melanostictus) soup. The boy expired of ventricular fibrillation. The girl presented with varying degrees of A-V block with congestive heart failure which, however, was completely resolved after intensive treatment. In our experience, early identification and intervention are necessary to decrease absorption of toxins; to correct hyperkalemia. Dysrhythmia should be treated with atropine, antiarrhythmic agents, and a temporary pacemaker, in order to avoid lethal results.  相似文献

Abstract

A novel adsorbent Zr(IV)-immobilized resin was prepared to remove fluoride ions from tap water and industrial wastewater. In order to enhance both the kinetics and efficiency, large pathways were formed in the resin for fluoride ion adsorption and the Zr(IV)-phosphate complexes were immobilized on the polymer surface by surface-template polymerization. The Zr(IV)-immobilized resin had a fluoride adsorption capacity of 0.30 mmol/g. The morphology of the Zr(IV)-immobilized resin was evaluated by measuring the specific surface area, pore volume, and pore size distribution. The resin possessed large amounts of large macropores with diameters around 300 nm. The molecular structure at the fluoride adsorption sites was investigated by measuring the amounts of phosphorus, zirconium, and fluoride ion in the resin, and developing a model complex using computational chemistry. On the polymer surface, a fluoride ion/Zr(IV)/dioleyl phosphoric acid complex with an ideal F:Zr:P mole ratio of 3:1:3 could be formed.  相似文献

The role of acetic acid added to the reaction media for the enantio-differentiating hydrogenation of methyl acetoacetate over a (R,R)-tartaric acid-in-situ-modified nickel catalyst was studied from the viewpoint of the hydrogenation rate during repeated runs. The hydrogenation of methyl acetoacetate on the “enantio-differentiating sites” of a tartaric acid-modified nickel catalyst was specifically accelerated by the acetic acid added to the reaction media to increase the enantio-differentiating ability of the catalyst. In order to increase the enantio-differentiating ability, the addition of acetic acid to the reaction media was required in each run during the repeated use of the catalyst.  相似文献

Nickel oxide was prepared by the decomposition of nickel hydroxide and then the nickel oxide was reduced to form the nickel catalyst. The properties of the nickel oxide and the reduced nickel were studied in relation to the enantio-differentiating ability (e.d.a.) of a tartaric acid-NaBr-modified reduced nickel catalyst. The modified nickel catalyst prepared from nickel oxide with less non-stoichiometric oxygen produced a high e.d.a. for the hydrogenation of methyl acetoacetate. The high crystallinity of the nickel oxide and the resultant nickel would be required to attain a high e.d.a.  相似文献

A mesogenic epoxy resin (DGETAM) was cured with a series of curing agents having different lengths of long alkyl chain (nBAB, n = 4, 8, 12). Properties of the curings were compared with those of the DGEBA cured with the same curing agents revealing the achievement of a balance between certain levels of thermal properties and excellent mechanical properties. Moreover, some curing systems were prepared with twin mesogenic type epoxy resins (DGEnMA, n = 4, 6, 8, 10, 12) having different lengths of alkyl chain as a flexible spacer and the same curing agents (n′BABs). Combinations of the same concentrations of chemical structures in the basic units of the network structure were applied, and the thermal and mechanical properties of their curing systems were investigated. The fracture energy of each system increased considerably with the increase of the alkyl chain length that adjoins the two mesogenic groups in the epoxy resins. © 2016 Wiley Periodicals, Inc. J. A high-quality single crystal of Pb((Zn1/3Nb2/3)0.91Ti0.09)O3 (PZNT 91/9), 40 mm in diameter and 20 mm in length, was successfully grown using the solution Bridgman method with a slight excess amount of ZnO. High-quality wafers were sliced from the light-brown single crystal. No PbO inclusions or opaque areas were observed in the transparent wafers. An array probe for echocardiography was constructed with the single-crystal wafer, and its superior performance was demonstrated.  相似文献
