Education for the nurse of tomorrow: a community-focused curriculum

A competitive enzyme-linked immunoadsorbent assay (ELISA) technique has been developed to facilitate quantitative analysis of the earliest step in the initiation of the extrinsic pathway of coagulation, i.e., complex formation of factor VII/VIIa with tissue factor. The ELISA measures the binding of biotinylated human plasma factor VII to relipidated recombinant human tissue factor. Quantitation of the relative affinity (expressed as IC50) of any factor VII molecular population or structural analogue for tissue factor can be determined by competitive binding. Subnanomolar concentrations of both wild-type recombinant human factor VII (rFVII) and rFVII(R152Q), a mutation at the FVII activation site, competed effectively with biotinylated plasma-derived factor VII in binding to tissue factor. In contrast, the affinity of rFVII(R79Q), a mutation in the first epidermal growth factor-like domain, was 12-fold lower. Following activation of rFVII(R79Q), its affinity for tissue factor and enzymatic activity increased 4-fold and 6-fold, respectively. For wild-type rFVII, enzymatic activity rose significantly following activation. However, its affinity for tissue factor was unchanged. We conclude that both the activation state of factor VII and the mutation of amino-acid residues within the first epidermal growth factor-like domain may alter the affinity of factor VII for tissue factor.     

Arthroscopic replacement of the anterior cruciate ligament with a double semitendinosus tendon

At the university hospital of emergency surgery in Innsburck and at the emergency services department of the hospital in Schwaz, anterior cruciate ligament reconstruction was performed in 467 patients between January 1992 and September 1993. In 117 cases, a double semitendinous tendon was used as an autograft. A total of 60 of these patients were followed up at an average of 20 months (13-33) after reconstruction. The results were rated according to the OAK and the Tegner scores. Objective measurements of instability were carried out by a KT 1000 arthrometer. In addition, 21 patients underwent computer-interfaced dynamometer testing using a Cybex 6000. Standardized loading of the thigh muscles was performed at angular velocities of 60 degrees/s (3 repetitions) and 240 degrees/s (30 repetitions): peak torques and total work were analyzed. The mean age was 23.5 years (17-55): 48 were male, 21 female. The main reasons for the ACL ruptures were sports injuries (51 cases). The patients were classified into three groups according to the data of the repair (group 1 reconstruction up to 1 week after trauma-28 patients: group 2: reconstruction between the 2nd and 6th week after trauma-15 patients: group 3: reconstruction 6 and more weeks after trauma-26 patients). In group 1. additional ruptures of the menisci. lesions of the MCL. and chondromalacia were seen in 71.4%. in group 2 in 73.3% and in group 3 in 80.8%. The average rating in the OAK score was 90.16 points: 38 patients (55.70%) had excellent results. 18(26.09%) were good. 9(13.04%) were fair and 4(5.80%) were poor. The Tegner activity score showed a reduction of 0.36 points on average. The largest amount of anterior translation was performed with the KT 1000 manual drawer test (2.29 mm on average compared with the contralateral side). Dynamometer testing showed a statistically significant difference in flexor and extensor mechanism (compared with the non-involved side) in both peak torques and total work. A repeat arthroscopy became necessary in five cases: arthrofibrosis in three and incipient joint infection in two cases.     

Impedance, attenuation and power-handling characteristics of doubleL-septa waveguides

Rectangular waveguides with two L-shaped septa attached to the broad walls in antipodal configuration have been shown theoretically by the authors (see ibid. vol.40, no.4, p.777-781, 1992) to be an improved variant of the double T-septa guide having larger cut-off wavelength and broader bandwidth of the dominant TE mode. Numerical data on the attenuation, impedance, and power handling capability of this new type of broadband guide are presented as additional design information     

Crystallization of glass in fireclay refractories; part III: Detailed study on the mullite crystal size in the ‘synthetic’ glass

Poor-grade fireclay products contain substantial amount of glass. A glass of similar composition was synthesized, nucleated and heat-treated for crystallization of mullite. The size and size-distribution of mullite crystals, the rate of change of size and the aspect ratio of the crystals were investigated in relation to the nucleating agents and temperature.     

Transition metal joints for steam generators—An overview

The transition metal joint (TMJ) between an austenitic stainless steel and a chromium-molybdenum (Cr-Mo) ferritic steel used widely in steam generators of power plants has for a long time presented problems relating to premature failures in service. The direct (bimetallic) TMJ presently in use is designed for a service life of about 200,000 h; but such TMJs with iron-base weld metals have been failing in service within about one-third of their design lifetime, while their counterparts with nickel-base weld metals fail within about one-half of their design lifetime. The causes for such premature service failures of these TMJs are discussed in detail, leading to the development of improved TMJs. One of the improved TMJs with a trimetallic configuration of austenitic stainless steel/Alloy 800/Cr-Mo ferritic steel is discussed in detail, covering its development, characterisation and evaluation. Accelerated performance tests in the laboratory have indicated a four-fold improvement in the service life of the TMJ with this trimetallic configuration compared to the bimetallic configuration. The metallurgical details of these studies are also discussed in this paper.     

Minimal inhibition concentrations of selected antibiotics for strains of Staphylococcus aureus

Minimal inhibition concentrations (MIC) of gentamycin (Ge), neomycin (Neo), rifampicin (Rif), ampicillin (Amp), lincomycin (Lin), erythromycin (Ery), and streptomycin (STM) were determined by the agar dilution technique using 46, 130, 131, 125, 140, 139 and 142 strains of Staphylococcus aureus, respectively. The strains, selected from the collection of the authors' laboratory, were isolated from mammary gland secretions of cows affected with clinical or subclinical mastitis. The following ranges of MIC (micrograms/ml) were assessed for the antibiotics under study: Ge 0.125-0.50, Neo 0.06-0.50, Rif 0.0039-0.030, Amp 0.015-1.00, Lin 0.25-1.00, Ery 0.06-0.25, STM 0.50-64.0. Modal MIC (micrograms/ml) were as follows; Ery 0.125 (86%), Lin 0.5 (71.4%), Rif 0.007 (68.7%), Ge 0.25 (56.5%), STM 1.00 (54.2%), Neo 0.25 (53.8%), Amp 0.06 (41.6%). The order of efficiency expressed in MIC 90 (micrograms/ml) was as follows: Rif (0.015), Ery (0.125), Ge (0.25), Neo (0.25), Amp (0.5), STM (4.0).     

Dynamic arrays for fast, efficient, data manipulation during image analysis: a new software tool for exploratory data analysis.

Memory reallocation is used to construct a run-time data structure for fast/efficient storage of information during collection and analysis. The data structure presented uses dynamic memory but does not require the use of pointers to link nodes of information together. It allows for simple and efficient access to data via array indexing rather than through the use of lists or tree structures and it provides flexibility for competing storage requirements that are determined dynamically. The data structure is developed in the C programming language and a suite of ANSI standard C subroutines that make up a run-time data structure management system is provided.     

Analysis of aperture blockage in reflector antennas by usingobstacle-located blockage currents

An improved method is presented to account for blockage effects in the analysis of reflector antennas. Commonly this is done by introducing shadows on the reflector surface according to the location of the obstacles when performing the physical optics integration. By using physical optics blockage currents located at the blocking obstacle instead of at the main reflector surface, the effect of the different locations in the axial direction is accurately accounted for. This can easily be included by a single phase factor in existing computer programs based on physical optics integration     

New wire interface for graded 3-D TLM

A new 3-D graded TLM model for thin radiating wires is presented. The model differs from previous TLM models because the wire is modelled at the interfaces between cells, rather than through the centre of the node. The connection between the TLM symmetrical condensed-node link lines and the line forms a 'wire interface'. The scattering algorithm for the interface and simulation results are given.<>
