The pathogenesis of hyperlactatemia during sepsis is poorly understood. We have previously described an increase in lactate concentration across the lung in the dog during early endotoxemia. Accordingly, we sought to determine if the lung releases lactate in humans and what relation this has with lung injury. METHODS: We measured lactate concentrations across the lung and lung injury scores (LIS) in two groups of patients. Group 1 consisted of nine patients with acute lung injury (LIS > or = 2.0) and elevated lactate concentrations (> 2.0 mmol/L). Group 2 contained 12 patients with no acute lung injury (LIS scores < or = 1.5), with or without increased lactate concentrations. Simultaneous measurements of plasma lactate and blood gases were obtained from indwelling arterial and pulmonary artery catheters. Measurements of cardiac output were also obtained. Lactate measurements were done using a lactate analyzer (YSI; Yellow Springs, Ohio). RESULTS: For each patient with acute lung injury and hyperlactatemia, an arterial-venous lactate gradient existed demonstrating release of lactate by the lung. This gradient persisted after correction for changes in hemoconcentration across the lung. The lactate gradient across the lung was 0.4 +/- 0.2 mmol/L for group 1 vs 0.05 +/- 0.1 mmol/L for group 2 (p = 0.001). This corresponded to a mean pulmonary lactate flux of 231.3 +/- 211.3 vs 5.0 +/- 37.2 mmol/h (p = 0.001). The lactate flux and the arterial-venous lactate difference correlated with LIS both for the entire sample and for the subgroup with hyperlactatemia (r = 0.69, p < 0.01). Pulmonary lactate flux was not related to arterial lactate levels (r = 0.25). CONCLUSION: In patients with acute lung injury and hyperlactatemia, the lung is a major source of lactate and lactate flux correlates with LIS. This lactate flux could explain some of the hyperlactatemia seen in sepsis. 相似文献

The perirhinal cortex (PRC) plays a major role in memory processes. This role may be influenced by activity in the adjacent entorhinal cortex (EC) and hippocampus (HPC), particularly during the processing of spatial information. In the current experiment we sought to determine whether the cholinergically mediated (type II) theta rhythm, which is a prominent electrophysiological feature of both HPC and EC activity, influenced neuronal firing in the PRC of urethane-anesthetized rats. When the spontaneous firing activity of single units recorded in PRC was related to theta recorded from the hippocampal fissure, it was determined that the firing of 50/163 (31%) PRC neurons exhibited a statistically significant phase relationship (mean phase angle = 188 degrees) to HPC theta. Thirty-three (66%) of these neurons tended to fire near the trough, and 17 near the peak, of this activity. These data indicate that a high proportion of PRC neurons participate in hippocampal-entorhinal theta activity. We propose a method for increasing word recognition accuracies by correcting the output of a handwriting recognition system. We treat the handwriting recognizer as a black box, such that there is no access to its internals. This enables us to keep our algorithm general and independent of any particular system. We use a novel method for correcting the output based on a “phrase-based” system in contrast to traditional source-channel models. We report the accuracies of two in-house handwritten word recognizers before and after the correction. We achieve highly encouraging results for a large synthetically generated dataset. To assess the effect of timing of human chorionic gonadotrophin (HCG) administration in ovarian stimulation cycles, the serum oestradiol concentration and follicle profile were compared with the clinical pregnancy rate in 582 ovarian stimulation-intra-uterine insemination (OS-IUI) cycles and 3917 in-vitro fertilization-embryo transfer (IVF-ET) cycles. The pregnancy rates increased exponentially with increasing oestradiol in both OS-IUI and IVF-ET cycles (R2 = 0.720, P < 0.001) but then decreased in OS-IUI cycles when the oestradiol concentration exceeded 5000 pmol/l (R2 = 0.936, P < 0.004) at HCG administration. In OS-IUI cycles the percentages of cycles with three or more mature follicles (> or = 18 mm diameter) increased up to an oestradiol concentration of 5000 pmol/l then declined, mirroring the pregnancy rate (R2 = 0.900, P = 0.01). The exponential increase in pregnancy rate with increasing oestradiol concentration in IVF-ET cycles suggests that high oestradiol concentration does not have a deleterious effect on endometrial receptivity. The decrease in pregnancy rate in OS-IUI cycles when oestradiol concentration exceeded 5000 pmol/l reflected fewer mature follicles, resulting from premature administration of HCG to avoid severe ovarian hyperstimulation syndrome (OHSS). We recommend that HCG administration be delayed until multiple follicles have reached maturity, and reducing the risk of severe OHSS by converting high risk OS-IUI cycles to IVF-ET, or if funds or facilities are unavailable, transvaginally draining all but four or five mature follicles. 相似文献

Experiments have been carried out in a draft tube fluidized bed bioreactor to study biodegradation of synthetic wastewater containing phenol. The microorganism employed in the study Psuedomonas putida has been immobilized on solid support particles. Studies have been carried out at different feed concentrations of phenol, air flow rates and feed flow rates. The mass transfer coefficient for phenol transfer from bulk phase to the surface of the biofilm on the solid particle has been estimated from observed experimental data using the conservation equations. The mass transfer coefficient was found to be in the range of 0.0726 x 10(-5) to 0.2012 x 10(-5) m s(-1). It was found to increase with increase in feed concentration, air flow rate and feed flow rate. A dimensionless correlation has been developed for the mass transfer coefficient in terms of Sherwood, Reynolds and Schmidt numbers, and the same has been compared with correlations available in literature. 相似文献

Contact resistance limits the performance of organic field‐effect transistors, especially those based on high‐mobility semiconductors. Despite intensive research, the nature of this phenomenon is not well understood and mitigation strategies are largely limited to complex schemes often involving co‐evaporated doped interlayers. Here, this study shows that solution self‐assembly of a polyelectrolyte monolayer on a metal electrode can induce carrier doping at the contact of an organic semiconductor overlayer, which can be augmented by dopant ion‐exchange in the monolayer, to provide ohmic contacts for both p‐ and n‐type organic field‐effect transistors. The resultant 2D‐doped profile at the semiconductor interface is furthermore self‐aligned to the contact and stabilized against counterion migration. This study shows that Coulomb potential disordering by the polyelectrolyte shifts the semiconductor density‐of‐states into the gap to promote extrinsic doping and cascade carrier injection. Contact resistivities of the order of 0.1–1 Ω cm2 or less have been attained. This will likely also provide a platform for ohmic injection into other advanced semiconductors, including 2D and other nanomaterials. 相似文献

Two types of highly active and selective vinyl acetate monomer (VAM) catalysts have been developed in our lab via traditional lab preparation and high-pressure reactor performance test methods. Experimental results are presented here.
Degussa is now engaged in high-throughput (HT) VAM catalyst powder preparation and HT (50 parallel reactor) testing to accelerate
the development of new higher performing commercial catalysts. Our first results on HT are presented here. Though HT systems
in both catalyst preparation and testing have existed for over a decade, HT heterogeneous catalyst preparation methods have
been very slow to develop. The HT preparation system described here provides a versatile method of preparing commercially
relevant catalysts as required by Degusssa. The HT reactor test system described here uses plant operating conditions and
provides reliable STY and selectivity data on par with that obtained from conventional lab VAM test systems. Both the HT preparation
and HT test systems combined allow for significant increases in the commercialization rate of new catalysts. 相似文献

The convenience of search, both on the personal computer hard disk as well as on the web, is still limited mainly to machine printed text documents and images because of the poor accuracy of handwriting recognizers. The focus of research in this paper is the segmentation of handwritten text and machine printed text from annotated documents sometimes referred to as the task of “ink separation” to advance the state-of-art in realizing search of hand-annotated documents. We propose a method which contains two main steps—patch level separation and pixel level separation. In the patch level separation step, the entire document is modeled as a Markov Random Field (MRF). Three different classes (machine printed text, handwritten text and overlapped text) are initially identified using G-means based classification followed by a MRF based relabeling procedure. A MRF based classification approach is then used to separate overlapped text into machine printed text and handwritten text using pixel level features forming the second step of the method. Experimental results on a set of machine-printed documents which have been annotated by multiple writers in an office/collaborative environment show that our method is robust and provides good text separation performance. 相似文献
