We report a case of life-threatening esophageal hemorrhage after metal stent implantation successfully treated by arterial embolization. An 85-year-old woman was admitted in shock secondary to massive hematemesis and melena. Recent medical history revealed esophageal cancer treated 8 weeks previously by endoesophageal radiotherapy (40 Gy) and endoscopic placement of a covered Wall-stent prosthesis. Selective arteriography of the fifth posterior right intercostal artery showed massive contrast extravasation in the esophagus. Embolization was performed with 150-250-micron polyvinyl alcohol particles. Follow-up at 5 months was uneventful. Arteriography and embolization are advised when severe hemorrhage occurs after esophageal implantation of metal stents. 相似文献

PURPOSE: Vascular enumeration has been claimed to be an independent prognosticator for invasive breast cancer. Most of the studies have performed a manual count of the vessels. Few investigators have used image analyzers to reduce subjectivity in the measures. The aim of this retrospective study was to compare the manual vessel count to the counts obtained with an image analyzer and to estimate their possible prognostic influence. MATERIALS AND METHODS: We selected 112 patients with invasive ductal carcinoma, treated with radical mastectomy followed by chemotherapy and with a minimal follow-up time of 60 months (5 years). After immunostaining with factor VIII, we performed a double count of the vessels. First, we performed a manual count following Gasparini's criteria, and second, we used an image analyzer (Microm, Barcelona, Spain) to count the vessels in eight adjacent microscopic fields with a 20x objective, to reach a total area of 0.98 mm2. The image analyzer calculated the total number of vessels, their size and also the percentage of the field occupied by the vessels. RESULTS: In our series, vascular enumeration showed no significant association with histologic grade (Bloom-Scarff-Richardson grading), tumor size, or staging. CONCLUSION: Vascular enumeration with both methods was an independent prognosticator for relapse-free-survival (RFS) in both node-negative and -positive patients in the univariate analysis, but only vascular enumeration with the image analyzer was an independent prognostic factor in the multivariate analysis, together with lymph node metastases. 相似文献

We examined the activation of genes induced by erythropoietin (Epo) in erythroid progenitor cells that were isolated from the spleens of mice infected with anemia-inducing strain of the Friend virus. These erythroid progenitor cells, termed FVA cells, undergo in vitro differentiation to erythrocytes under the influence of Epo within 2 to 3 days. We used a differential hybridization procedure to screen a cDNA library constructed from FVA cells that were treated with Epo 2U/mL in the absence of serum for 2 hours. Of 20,000 recombinant phages, 47 plaques hybridized preferentially to cDNA probe prepared from Epo-stimulated cells. We found at least three different Epo-responsive genes (ERGs) and one of them corresponds to the mouse virus-like (VL30) element, similar to already reported BVL-1. The induction of VL30, which was evident within 30 minutes after Epo exposure, reached a maximum by 1 hour and remained stable for up to 4 hours. The treatment of FVA cells with cycloheximide (CHX) 10 micrograms/mL, which in itself activates the expression of VL30 caused a superinduction of the Epo signal. Changes in intracellular Ca2+ concentrations, either raised by ionomycin or depleted by EGTA, had no effect on the Epo-induced VL30 expression. In addition, protein kinase C (PKC) inhibitors such as staurosporine (3 mumol/L) or H7 (20 mumol/L) and a tyrosine kinase inhibitor, genistein (200 mumol/L), did not inhibit the Epo-induced expression of VL30. TPA (100 ng/mL), a PKC agonist, did not induce VL30 expression. Although the physiologic role of VL30 in the differentiation of erythroid progenitor cells is not known, our findings demonstrate that VL30 is an early ERG, and may be a useful indicator of the initial molecular actions of Epo. 相似文献

Training has been identified as an effective tool in the development of human resources, especially in the current context of change. This work aims to analyse the realisation and demand on training courses in the Rioja wine sector in Spain. A representative sample of companies in the sector was interviewed using a questionnaire. Two types of analysis were conducted: univariate and a factorial analysis of variance. These analyses allow using a hypothesis test in which three independent variables are controlled – zone, ownership, and winery size. The results show that the realisation and demand for training are focused on topics related to wine production, and not on marketing. No significant differences were found regarding the realisation and demand for training and the independent variables. 相似文献

The extracts (0.5% [w/w]) from Caesalpinia spinosa or tara (CS) and Caesalpinia decapetala (CD) leaves were evaluated for their ability to inhibit lipid oxidation using oil‐in‐water (O/W) emulsions as the model system. Liquid chromatography (LC) combined with mass spectrometry (MS), using a hybrid quadrupole time‐of‐flight (QTOF) mass spectrometer, was employed for (1) the identification of potential antioxidant species in the extracts and (2) to follow their evolution through aging of the emulsions. CS extracts, containing esters and conjugated forms of gallic acid (GA), turned out to be more effective than CD, rich in flavonoid species, in stabilizing the O/W emulsions. After 3 wk at 33 °C, peroxide values of emulsions stabilized with CS and CD extracts were 6.7 and 18.2 meq hydroperoxides/kg, respectively. Responses measured for the oligomers and esterified forms of GA in CS containing emulsions decreased with time; however, the response for the free form of GA kept increasing. 相似文献

The effect of winemaking using blends of red grape varieties cultivated in La Mancha region (Spain) on the aroma profile of wines was researched by chemical characterization. Free and glycosidically bound aroma compounds were isolated by solid phase extraction using dichloromethane and ethyl acetate, respectively, as solvents in elution and then analyzed by gas chromatography-mass spectrometry. Free and bound volatile compounds were analyzed in Cencibel, Bobal, and Moravia Agria monovarietal wines, and in 3 wines obtained with the blending of grapes: Cencibel (50%) + Bobal (50%); Cencibel (50%) + Moravía Agria (50%); Cencibel (33%) + Bobal (33%) + Moravía Agria (33%). Aroma compounds were studied in terms of odor activity values (OAVs). Ninety free aroma compounds and sixty-five bound aroma compounds were identified and quantified. The odor activity values for the different compounds were classified into 7 odorant series. The fruity and sweet series contributed most strongly to the aroma profile of all wines, independently of the winemaking technique used. In general, co-winemaking wines present a more complex chemical profile than monovarietal wines. Practical Application: Some grape varieties could benefit from this process with the presence of other varieties that might have an excess of aroma compounds. In this study, the wines were elaborated by blending different grape varieties together; this process implies co-maceration and co-fermentation steps. The co-winemaking technique could benefit from additional molecules provided by the other varieties, which results in a more complex formation than in the case of monovarietal wines. This technique provides a viable alternative to traditional winemaking methods for improving and enhancing the sensory profile of elaborated wines. 相似文献

Milk minerals are important for calf growth, and they have other roles as well, such as immune regulation. This 2-yr study examined content of Ca, P, Mg, Na, K, Fe, and Zn in milk of 54 Iberian red deer hinds through 18 wk of lactation. Mean mineral composition of fresh milk was ash = 1.168 ± 0.007%, Ca = 2,330 ± 20 mg/kg, P = 640 ± 10 mg/kg, K = 1,100 ± 10 mg/kg, Na = 385 ± 3 mg/kg, Mg = 138 ± 1 mg/kg, Zn = 12.5 ± 0.2 mg/kg, and Fe = 0.65 ± 0.03 mg/kg. All minerals except Mg varied by week of lactation, but variation was usually <10% except for Fe (83% variation) and Zn (30% variation); both of those minerals increased as lactation proceeded. Increased concentrations of Fe and Zn in later lactation compensated for the reduction in milk production in mid and late lactation such that daily production was less variable for Fe (55% variation) or Zn (79% variation) than for other minerals (118 to 135% variation). Potassium content of milk decreased across time, but that effect occurred primarily during the last few weeks of lactation. Calving later vs. early in the calving season had variable effects on concentrations of different minerals: P, Mg, and K concentrations were not affected; Ca, Mg, and Na were all lower in milk from later calving hinds; and both Fe and Zn had higher concentrations in milk from hinds that calved later in the season. Lactating hinds seem to maintain a more stable daily yield of the microminerals Fe and Zn in milk compared with more variable concentrations of macrominerals as lactation progresses. Because of the essential role of Fe and Zn in immune function, a more stable supply of those minerals might be important to the health of growing red deer calves. 相似文献

Measurement of direct solar radiation has been shown to be very useful to improve control performance and disturbance rejection in solar fields by anticipating the effect of sudden changes in solar radiation due to clouds. Since direct solar radiation is measured locally by pyrheliometers, important errors in the estimation of the overall effective solar radiation can be produced when the pyrheliometer is covered by clouds while the rest of the solar field is not or viceversa. Furthermore, estimation of the overall efficiency affected by the reflectivity and absortance of metal tubes is very difficult because only local measurements can be obtained. This work proposes an algorithm for estimating overall solar radiation and efficiency at the field. The algorithm uses an unscented Kalman filter and it is validated by data obtained at the Plataforma Solar of Almer?´a (Spain). 相似文献
