This study examines health-related quality of life (HRQoL) in patients with leg ulceration and determines which patient groups are affected most by this condition. A cross-sectional study using the Nottingham Health Profile (NHP) and age/sex-matched normal scores in patients entering six clinical audit cycles was carried out. The 758 patients included in the study (mean age 74.6 years, 64% women) had been affected by leg ulceration for a median of 10.5 months (range 0.5 to 708). Patients produced significantly higher scores than age/sex-matched normal values for all domains of the NHP, indicating poorer HRQoL (all p < 0.001). Increasing age led to greater deficits in energy (p < 0.001) and mobility (p < 0.001) with greater social isolation (p = 0.044). Women experienced poorer energy, sleep patterns, mobility and emotional reactions (all p < 0.001), and increased physical pain and social isolation (p < 0.05) compared to men. However, higher scores may be expected for older women based on age/sex-matched normal values. After adjustment for age/sex-matched normal values, it was the younger patients who experienced a greater deficit in HRQoL over all domains of the NHP, with men scoring higher than women in the domains of bodily pain, sleep and social isolation (p < 0.001) and energy (p = 0.015). Leg ulceration has a major impact on patients' HRQoL as detected by the NHP. The excess in scores compared with age/sex-matched normal values indicate that it is younger male patients in whom ulceration makes a greater impact on HRQoL. 相似文献

The majority of spine fusions currently performed are for degenerative conditions. Controversy exists regarding whether to routinely brace patients during the postoperative period. The benefits of a rigid orthosis have yet to be documented in a scientific study, and the cost of a custom-molded orthosis can be quite high. An extensive literature search reveals few articles dealing with the subject, and none with an adequate study design to convincingly support or refute the use of external braces. In addition to the questions of whether an external brace is effective, the mechanism of action also remains unclear. It has been difficult to document mechanical effectiveness, so perhaps the effect is psychologic. In addition, it is possible that some, not all, fusion patients may benefit from a brace--yet our ability to select such a patient is poor. As a result, we have solicited the views of two experienced surgeons on the topic. Dr. Connolly argues that an external orthosis is advisable in many cases; Dr. Grob feels that the rigidity of internal fixation should be adequate to obviate the need for external bracing. 相似文献

BACKGROUND: Long-term results on LASIK are not available to date. We therefore evaluated the predictability, stability and complication rate after LASIK in moderate--to-high myopia. PATIENTS AND METHODS: We treated 70 eyes (41 patients) using the Automatic Corneal Shaper and the Keracor 116 excimer laser. Patients were followed for 1, 6, 12 and 24 months. Spectacle refraction, visual acuity, rate of retreatment, and patient satisfaction were evaluated. RESULTS: At 24 months the results were as follows: Myopia -5 to -9.9 D (n = 18): 94% within 1 D; regression between 1 and 12 (12 and 24) months > 1 D in 6% (6%); uncorrected acuity 20/40 or better in 83%; no loss of 2 ore more lines of visual acuity; 89% highly satisfied. Myopia -10 to -14.9 D (n = 12): 88% within 1 D; regression between 1 and 12 (12 and 24) months > 1 D in 20% (0%); uncorrected acuity 20/40 or better in 72%; 4% lost 2 or more lines of visual acuity; 96% highly satisfied. Myopia -15 to -29 D (n = 22): 33% within 1 D; regression between 1 and 12 (12 and 24) months > 1 D in 41% (18%); uncorrected acuity 20/40 or better in 7%; no loss of 2 or more lines of visual acuity; 67% highly satisfied. CONCLUSION: LASIK is an accurate, effective and stable procedure for correcting myopia of -5 to -10 D. Results are less precise in myopia up to -15 D, and some visual loss occurs in a number of patients. In myopia > -15 D, results are not satisfactory because of poor accuracy and low stability. 相似文献

Fas ligand (FasL) is produced by activated T cells and natural killer cells and it induces apoptosis (programmed cell death) in target cells through the death receptor Fas/Apol/CD95. One important role of FasL and Fas is to mediate immune-cytotoxic killing of cells that are potentially harmful to the organism, such as virus-infected or tumour cells. Here we report the discovery of a soluble decoy receptor, termed decoy receptor 3 (DcR3), that binds to FasL and inhibits FasL-induced apoptosis. The DcR3 gene was amplified in about half of 35 primary lung and colon tumours studied, and DcR3 messenger RNA was expressed in malignant tissue. Thus, certain tumours may escape FasL-dependent immune-cytotoxic attack by expressing a decoy receptor that blocks FasL. 相似文献

A correlation has been observed between the dephosphorylation and downregulation of cPKC and the induction of apoptosis. This relationship is shown to be causal in COS cells where expression of dominant negative PKC alpha is shown to induce apoptosis. This response is rescued by co-expression of wild-type PKC alpha. STUDY DESIGN: Electromyographic profiles from the neck muscles of five young, healthy men were obtained to study the muscular, activation patterns during dynamic external loading. OBJECTIVES: To examine the myoelectric activity of selected cervical muscles during dynamic loading, and to explore the reaction of the cervical muscles to different loading conditions. SUMMARY OF BACKGROUND DATA: The response of cervical muscles throughout dynamic external loading is not well understood. Electromyography provides information, within certain limitations, about the neural drive to various components of the musculature. Such information on activation, combined with geometric parameters of the musculoskeletal tissues, constitutes a tool for helping to understand the mechanism of head and neck impact-related injury. METHODS: Subjects were tested with the head and neck in the neutral and prebent positions. Dynamic loads were applied laterally to each subject by free-falling masses to create peak dynamic loads ranging from about 40 to 100 N. Force and electromyographic data were recorded, and cross-correlations from linear envelope electromyography were calculated. RESULTS: The peak electromyogram showed large differences between the two applied loads and between subjects. There were time delays (75-165 msec) between the peak applied force and the peak electromyogram. The muscles on the contralateral side demonstrated different electromyographic profiles between subjects. Within subjects, the electromyographic profiles from each muscle were consistent for the same trial conditions. CONCLUSIONS: Electromyographic profiles and the cross-correlator coefficients for cervical muscles have shown reproducible intrasubject muscle synergies, which were not sensitive to the magnitude of applied load and the posture of the head. Pneumocystis carinii is an important pulmonary pathogen responsible for morbidity and mortality in patients with AIDS. The acute-phase response (APR), the primary mechanism used by the body to restore homeostasis following infection, is characterized by increased levels of circulating fibrinogen (FBG). Although the liver is the primary site of increased FBG synthesis during the APR, we unexpectedly discovered that FBG is synthesized and secreted by lung alveolar epithelial cells in vitro during an inflammatory stimulus. Therefore, we sought to determine whether lung epithelial cells produce FBG in vivo using animal models of P. carinii pneumonia (PCP). Inflammation was noted by an influx of macrophages to P. carinii-infected alveoli. Northern hybridization revealed that gamma-FBG mRNA increased two- to fivefold in P. carinii-infected lung tissue, while RNA in situ hybridization demonstrated increased levels of gamma-FBG mRNA in the lung epithelium. Immunoelectron microscopy detected lung epithelial cell-specific production of FBG, suggesting induction of a localized inflammatory response resembling the APR. A systemic APR was confirmed by a two- to fivefold upregulation of the levels of hepatic gamma-FBG mRNA in animals with PCP, resulting in a corresponding increase in levels of FBG in plasma. Furthermore, immunoelectron microscopy revealed the presence of FBG at the junction of cell membranes of trophic forms of P. carinii organisms aggregated along the alveolar epithelium. These results implicate FBG in the pathogenesis of PCP in a manner similar to that of the adhesive glycoproteins fibronectin and vitronectin, which are known to participate in intra-alveolar aggregation of organisms and adherence of P. carinii to the lung epithelium. 相似文献
