Synthesis and pharmacological evaluation of new flosulide analogues, synthesized from natural safrole

OBJECTIVE: To study the effect of adjuvant chemotherapy after curative hepatic resection in patients with hepatocellular carcinoma. DESIGN: A randomized controlled trial. SETTING: A tertiary referral center. PATIENTS: During a 54-month period, 142 patients with hepatocellular carcinoma underwent hepatic resection at 1 institution. Sixty-six patients who survived the operation and had no demonstrable evidence of residual disease on ultrasonographic examination and hepatic angiographic testing at 1 month after surgery agreed to participate in the study. The median follow-up time was 28.3 months. INTERVENTION: Thirty patients received a combination of intravenous epirubicin hydrochloride (8 doses of 40 mg/m2 each at 6-week intervals) and transarterial chemotherapy using an emulsion of iodized oil and cisplatin (3 courses with a maximum dose of 20 mL each at 2-month intervals). Thirty-six patients had no adjuvant treatment. MAIN OUTCOME MEASURES: Recurrence rate and disease-free survival. RESULTS: A total of 138 courses of intravenous epirubicin was given to the 30 patients. Sixty-one courses of transarterial chemotherapy were given to only 29 of the 30 patients assigned to the treatment group, because the hepatic artery in 1 patient was thrombosed. Six patients (20%) had chemotherapy-related complications with no mortality. Twenty-three of 30 patients in the treatment group and 17 of 36 patients in the control group had recurrences (P=.01). Patients who received adjuvant chemotherapy had a higher incidence of extrahepatic metastases (11 patients vs 5 patients; P=.03). The respective disease-free survival rates at 1, 2, and 3 years were 50%,36%, and 18% for the treatment group and 69%, 53%, and 48% for the control group (P=.04). CONCLUSION: In a group of patients who underwent curative resection of hepatocellular carcinoma, postoperative adjuvant chemotherapy using the present regimen was associated with more frequent extrahepatic recurrences and a worse outcome.     

The role of glycoproteins in neural development function, and disease

Glycoproteins play key roles in the development, structuring, and subsequent functioning of the nervous system. However, the complex glycosylation process is a critical component in the biosynthesis of CNS glycoproteins that may be susceptible to the actions of toxicological agents or may be altered by genetic defects. This review will provide an outline of the complexity of this glycosylation process and of some of the key neural glycoproteins that play particular roles in neural development and in synaptic plasticity in the mature CNS. Finally, the potential of glycoproteins as targets for CNS disorders will be discussed.     

Radiation from curved single-mode fibres

A study of propagation in curved single-mode fibres shows that the transmission loss increases sharply below a critical bend radius. However the radiation does not leak away uniformly with distance but in a series of discrete well-defined rays.     

Analytic solution for the birefringence produced by thermal stress in polarization-maintaining optical fibers

Polarization-maintaining optical fibers are usually made by inducing a large anisotropic thermal stress in the core so that it appears highly birefringent. A simple analytic solution has been found for the birefringence in terms of the cross-sectional distribution of the high-expansion material used to create the thermal stress. The analysis is able to predict optimal structures which efficiently utilize the available stress and thus maximize the birefringence. It is shown that the optimum structure has a cross-sectional geometry resembling a bow-tie. Design rules are given whereby the dimensions may be chosen and these are verified in a simple experiment.     

Stereochemical requirements for intercalation of platinum complexes into double-stranded DNA's

The complexes 1,10-phenanthrolineethylenediamineplatinum(II) and 2,2'-bipyridineethylenediamineplatinum(II) have a planar, aromatic ligand system that facilitates intercalation, as shown by their ability to unwind closed circular duplex DNA. Nonbonded steric interactions can rotate the pryidine ligands out of the coordination plane in bis(pyridine)ethylenediamineplatinum(II), thus preventing intercalation. Fiber x-ray diffraction patterns of the two metallointeracalators indicate that the binding is governed by the neighbor exclusion principle.     

Allocation of freeway ramp metering volumes to optimize corridor performance

A simple macroscopic freeway traffic corridor model is formulated for the purpose of developing optimal allocations of freeway on-ramp metering volumes. The model represents a freeway and a single parallel "equivalent surface street," both unidirectional, which are interconnected by freeway on-ramps. Since a choice of routes is included in the problem formulation, the fairly standard methodology of "traffic assignment" is employed to predict how drivers will react to the on-ramp metering. An explicit traffic assignment algorithm which includes ramp queueing is developed. Both the corridor and traffic assignment models are restricted to stationary traffic patterns. Two specific corridor performance criteria, freeway travel rate and total travel-time rate, are formulated in terms of the model variables. Then the problem of choosing ramp metering volumes to optimize a performance criterion is posed. A computationally efficient suboptimal dynamic programming scheme is developed and illustrated by an example.
