OBJECTIVE: The audiologic presentation of vestibular schwannoma (VS) associated with neurofibromatosis type 2 (NF2) has not been well characterized. The goal of this study was to investigate the audiologic features of NF2-associated VS and to determine their relationship to the size of the tumor. STUDY DESIGN: A retrospective case review. SETTING: Quaternary governmental medical research institute evaluating patients fitting specific criteria for ongoing clinical studies. PATIENTS: Audiologic and magnetic resonance imaging data were available for 40 patients (25 males, 15 females), with an average age of 32 years, who had been recruited for ongoing clinical and genetic studies of NF2. MAIN OUTCOME MEASURES: The audiologic profile and magnetic resonance imaging characteristic of tumor were retrospectively reviewed. RESULTS: The average size of the tumor at presentation was 7.26 +/- 16.58 cm3 and measured 1.2, 1.6, and 1.1 cm in the anterior/posterior, lateral/medial, and superior/inferior dimensions, respectively. An increase in lateral/medial size of the tumor most significantly correlated with deterioration in mid- (1,000-2,000 Hz) and high- (4,000-8,000 Hz) frequency hearing levels, elevated speech reception threshold, and prolonged auditory brain stem response waves III and V latency. CONCLUSIONS: Patients with NF2 demonstrate a more predictable audiologic profile for a given size tumor than has been previously described with spontaneous or sporadic VS.  相似文献

The relative importance of fat and lean tissue mass in determining bone mineral mass among postmenopausal women was examined in this 1-year longitudinal study. Fifty postmenopausal Caucasian women entered the study; 45 of them completed a 1-year follow-up. Dual-energy X-ray absorptiometry was employed for measuring total and regional bone mineral density (BMD) and bone mineral content (BMC), fat tissue mass (FTM), lean tissue mass (LTM), and body weight. Results from linear regression analysis using the cross-sectional data (n = 50) of the study indicated that LTM explained a larger percentage of variation in bone mineral mass than did FTM. FTM and LTM were found to be moderately correlated (r = 0.55); when FTM was entered in the same predicting regression models, LTM was a significant predictor (p < 0.05) of the total and regional BMC, but not BMD. The percent FTM (and inversely %LTM) was correlated with BMD and BMC, but significant correlation was primarily found only for total body BMD (or BMC). Weight was the best predictor of total body BMD and BMC. Longitudinally (n = 45), annual changes in both FTM and weight were significantly associated with annual changes in regional BMD after adjustment for initial bone mineral values (p < 0.05). We conclude that bone mineral mass is more closely related to LTM than to FTM, while annual changes in regional BMD are more closely correlated with changes in FTM in healthy postmenopausal women. Meanwhile, increased body weight is significantly associated with increased bone mineral mass.  相似文献

The attenuated S. typhimurium SL3261 (aroA) strain causes mild infections in BALB/c mice. We were able to exacerbate the disease by administering anti-interleukin-12 (IL-12) antibodies, resulting in bacterial counts in the spleens and livers of anti-IL-12-treated mice that were 10- to 100-fold higher than the ones normally observed in premortem mice; yet the animals showed only mild signs of illness. Nevertheless, they eventually died of a slow, progressive disease. Mice infected with salmonellae become hypersusceptible to endotoxin. We found that IL-12 neutralization prevented the death of infected mice following subcutaneous injection of lipopolysaccharide. Granulomatous lesions developed in the spleens and livers of control animals, as opposed to a widespread infiltration of mononuclear cells seen in the organs of anti-IL-12-treated mice. In the latter (heavily infected), salmonellae were seen within mononuclear cells, indicating an impairment of the bactericidal or bacteriostatic ability of the phagocytes in the absence of biologically active IL-12. Gamma interferon (IFN-gamma) levels were reduced in the sera and tissue homogenates from anti-IL-12-treated mice compared to those in control animals. Furthermore, fluorescence-activated cell sorter analysis on spleen cells showed that IL-12 neutralization impaired the upregulation of I-Ad/I-Ed antigens on macrophages from infected mice. Inducible nitric oxide synthase and IFN-gamma mRNA production was down-regulated in anti-IL-12-treated mice, which also showed an increased production of IL-10 mRNA and a decrease in nitric oxide synthase activity in the tissues. Administration of recombinant IFN-gamma to anti-IL-12-treated mice was able to restore host resistance, granuloma formation, and expression of major histocompatibility complex class II antigens in F4/80(+) and CD11b+ spleen cells.  相似文献

The measurement of tumour cell proliferation is becoming increasingly recognised in defining prognostic groups. Proliferating cell nuclear antigen (PCNA) immunolocalisation can be used as an index of cell proliferation and may define the extent of departure from normal growth control. The monoclonal antibody PC10 stains PCNA in archival paraffin-embedded tissue. This study investigates its potential as a prognostic marker in early and advanced ovarian cancer. A three-stage immunoperoxidase technique was developed to detect the monoclonal antibody PC10. Archival paraffin-embedded tissue from 19 stage I ovarian tumours (13 malignant and six borderline) and 79 advanced (stage IIb-IV) ovarian tumours (patients entered into the Third North-West Thames Ovarian Cancer Trial) was immunostained with PC10. PC10 immunostaining was performed successfully in 91.8% of cases. The PC10 labelling index (PC10 LI) ranged from 1.5% to 88% with a mean value of 47.4%. Stage I borderline tumours had significantly lower PCNA labelling indexes than stage I malignant tumours (P < 0.048). In advanced disease there was an inverse correlation between PC10 and overall survival, and in those patients who underwent good debulking surgery (37 patients with disease < 2 cm diameter) a low PC10 value (< 36.5%) correlated with improved survival (log-rank trend test for survival, chi 2 = 5.75, P = 0.017). PCNA immunostaining defines a good prognostic subgroup in adequately debulked patients with ovarian cancer.  相似文献

This work investigated the resuspension and subsequent translocation of particulate matter (PM) from carpeted flooring surfaces due to walking. In addition, the effect of HVAC systems and ceiling fans on mixing and/or translocation of resuspended PM was studied. Testing took place both in a residence with a well-worn, soiled carpet and in an environmental test chamber. Prescribed walking occurred with PM measurements taken at multiple sampling heights. Scanning electron microscopy (SEM) of carpet fibers was used to determine the fraction of dust available for resuspension. These data, in conjunction with resuspended mass concentrations from this study, were used to generate emission factors by particle size for walking on both new and worn carpet.

Carpet loading does not affect the emission factor, indicating that the amount of resuspended PM is directly proportional to the available PM in the carpet. While relative humidity (RH) plays an important role in resuspension from new carpets, with high RH enhancing resuspension, it has the opposite affect with old carpets, with increased RH decreasing resuspension. With the HVAC system on, translocated particles 1.2 m horizontally from the source had number concentrations of approximately 20–40% of those at the source. With a ceiling fan on, extensive mixing was noted with little difference seen in particle resuspension by height. With the ceiling fan off, there was very little mixing present and particle size varied substantially by height.  相似文献

The PM2.5 standard proposed by the U.S. Environmental Protection Agency (EPA) has stimulated research on the relationships between particulate matter concentrations and the exposures and subsequent health responses of sensitive subpopulations, such as the elderly. Since individuals in these subpopulations may spend more than 90% of their time indoors, understanding the relationship between outdoor particle concentrations and those found in indoor microenvironments is critical. This research resulted in a time-dependent indoor air quality model incorporating all potential particle sources and loss mechanisms. Monte Carlo simulations of the model identified the mechanisms, such as particle loss during penetration through the building envelope, that modify the outdoor particle size distribution during transport into the interior of a building, calculated indoor-to-outdoor (I/O) concentration ratios, and estimated penetration factors as a function of particle size. Indoor particle generation and transport of outdoor particles through the HVAC system were the most important contributors to the indoor concentration in residential and commercial buildings, respectively. The most significant removal mechanisms included ventilation through and particle removal by the HVAC filter if an HVAC system was present, or particle deposition on indoor surfaces if an HVAC system was not present. The modeled I/O concentration ratios varied between 0.05 and 0.5, depending on particle size and type of ventilation system, and agreed well with published experimental results. Penetration factors less than unity were calculated for particles with aerodynamic diameters larger than 0.2 θ m if the air exchange rate and steady-state I/O concentration ratio were correlated during the simulations. An additional correlation between the air exchange rate and particle deposition velocity is required if penetration factors less than unity are to be modeled for particles with aerodynamic diameters smaller than 0.2 θ m. The 2-domain gammaS-crystallin, a highly conserved early evolutionaryoff-shoot of the gamma-crystallin family, is located in the water-richregion of eye lenses. The expressed C-terminal domain, gammaS-C, has beencrystallized and the 2.56 A X-ray structure determined. There are twodomains in the asymmetric unit which pair about a distorted twofold axis.One of the domains has an altered conformation in a highly conserved regionof the protein, the tyrosine corner. The distorted gammaS-C dimer ofdomains is compared with the highly symmetrical, equivalent recombinantdimer of C-terminal domains from gammaB- crystallin. Sequence changes closeto the interface, that distinguish gammaS from the other gamma-crystallins,are examined in order to evaluate their role in symmetrical domain pairing.  相似文献
