A program for the integration of chronic psychiatric patients into independent living in the community is described and data indicating its success are presented. The program is based on an educational, behavioral, social systems model, where "educational" refers directly to teaching skills which a person lacks, "behavioral" deals with providing the motivation to perform these skills and to precisely operationalize and measure the effectiveness of the steps in learning them, and "social systems" describes influencing clients' social environments in their interests. The model has wider implications for reintegrating a variety of socially marginal individuals into the mainstream of society.  相似文献

Preoperative radiotherapy for adenocarcinoma of the rectosigmoid   总被引:2,自引:0,他引:2  
Ninety-seven patients with adenocarcinoma of the rectosigmoid have been treated with high dose (5000-6000 rad) preoperative irradiation from 1960 through 1972 at the University of Oregon Health Sciences Center. Fifty-seven were initially clinically resectable and 40 were initially inoperable. Forty of the 57 initially clinically resectable patients had "curative" resections and are at risk for more than 5 years. An increase in 5-year survival (from 38% to 53%) and an absence of pelvic recurrence have occurred in those patients receiving preoperative irradiation and "curative" resection. Four of the 40 initially inoperable patients are alive without tumor. Three of the four survivors had irradiation and surgery; one had irradiation only. An additional four patients had no evidence of tumor at death. Tumor was totally sterilized by irradiation and nine patients and reduced to microfocal extent in an additional three of the 97 patients. Incidence of complications was no greater than has been reported in a surgical series from the same institution.  相似文献

Sodium‐ion hybrid supercapacitors (Na‐HSCs) have potential for mid‐ to large‐scale energy storage applications because of their high energy/power densities, long cycle life, and the low cost of sodium. However, one of the obstacles to developing Na‐HSCs is the imbalance of kinetics from different charge storage mechanisms between the sluggish faradaic anode and the rapid non‐faradaic capacitive cathode. Thus, to develop high‐power Na‐HSC anode materials, this paper presents the facile synthesis of nanocomposites comprising Nb2O5@Carbon core–shell nanoparticles (Nb2O5@C NPs) and reduced graphene oxide (rGO), and an analysis of their electrochemical performance with respect to various weight ratios of Nb2O5@C NPs to rGO (e.g., Nb2O5@C, Nb2O5@C/rGO‐70, ‐50, and ‐30). In a Na half‐cell configuration, the Nb2O5@C/rGO‐50 shows highly reversible capacity of ≈285 mA h g?1 at 0.025 A g?1 in the potential range of 0.01–3.0 V (vs Na/Na+). In addition, the Na‐HSC using the Nb2O5@C/rGO‐50 anode and activated carbon (MSP‐20) cathode delivers high energy/power densities (≈76 W h kg?1 and ≈20 800 W kg?1) with a stable cycle life in the potential range of 1.0–4.3 V. The energy and power densities of the Na‐HSC developed in this study are higher than those of similar Li‐ and Na‐HSCs previously reported.  相似文献

Numerical simulations were conducted using computational fluid dynamics to evaluate the effect of ventilation conditions on radon (222Rn) reduction performance in a residential building. The results indicate that at the same ventilation rate, a mechanical ventilation system is more effective in reducing indoor radon than a natural ventilation system. For the same ventilation type, the indoor radon concentration decreases as the ventilation rate increases. When the air change per hour (ACH) was 1, the indoor radon concentration was maintained at less than 100 Bq/m3. However, when the ACH was lowered to 0.01, the average indoor radon concentration in several rooms exceeded 148 Bq/m3. The angle of the inflow air was found to affect the indoor air stream and consequently the distribution of the radon concentration. Even when the ACH was 1, the radon concentrations of some areas were higher than 100 Bq/m3 for inflow air angles of 5° and 175°.  相似文献

Both powder-type and film-type mesoporous silica materials have been prepared by using a sol-gel process in highly acidic, solvent-rich and mild conditions by solvent evaporation method. Powder mesoporous silica which has hexagonal arrays was well synthesized within only 10 minutes, and mesoporous film was formed on the glass plate within only several seconds by supramolecular interactions between the silicate species and surfactant-templates. In particular, the synthesis time was very short compared with that of hydrothermal synthesis owing to the solvent evaporation. This method has some characteristics different from the hydrothermal method such as the following. There is no precipitation because particles do not form in the solution before solvent evaporation, bulk powder is rigid, whereas that of hydrothermal synthesis is very soft, and mesoporous materials are synthesized in acidic condition. Solvent evaporation method for the synthesis of mesoporous materials is promising because the method is very simple and time-saving.  相似文献

For the cancer cells which have overcome the second mitotic clock (M2), activated telomerase is essential and used as another marker of immortality. Many trials had been initiated to target telomerase, which is known to be specific to tumors. To determine the best in vitro cell system for testing the efficacy of telomerase inhibitors, we evaluated the telomerase activity of various cancer cell lines and measured their telomere lengths. We also treated some cancer cell lines with adriamycin and measured the changes of telomerase activity. Telomerase activity was evaluated in various cell lines with the TRAP (telomeric repeat amplification protocol) assay. Telomerase activity was calculated and translated into arbitrary units by computer-assisted densitometry with the control of telomerase activity in the 293 control cell line. Also, terminal restriction fragment lengths were measured using Southern blotting. We also measured telomerase activity and telomere lengths in 11 benign breast tumor tissues and 19 paired stomach cancer and normal tissues. Cancer cell lines treated with adriamycin we evaluated for changes of telomerase activity and the cell proliferation by MTT assay and dye exclusion test. Telomerase activity of cell lines was 95.3 24.1 unit with a range of 27.6-129.6 unit, while the telomere lengths of those cell lines were variable from 5.0 to 10.4 kbp with a median of 6 kbp. In 11 cancer cell lines which were not yet firmly established, we could not detect any telomerase activity. Low telomerase activity was detected in only 2 benign tumor tissues of breast with a median telomere length of 8.8 (7-10.5) kbp. Among paired 19 gastric cancer and normal tissues, only 7 cancer tissues showed weak telomerase activity. After adriamycin treatment, telomerase activity in YCC-S-1, YCC-S-3, MCF-7 and MCF-7/ADR was decreased in accordance with the changes of the cell numbers. Telomerase is specific to cancer tissues and is expressed differently from organ to organ. Telomerase activity by TRAP assay could be used as a chemosensitivity assay.  相似文献

The effects of in utero cocaine exposure on the development of the mRNAs encoding the dopamine transporter (DAT) and the D1, D2 and D5 dopamine receptor subtypes were determined in fetal monkey brains at day 45 and day 60 of gestation. Pregnant monkeys were treated with cocaine 3 mg/kg or saline i.m., four times a day from day 18 of gestation until the pregnancy was terminated at day 45 or day 60. The fetal brains were dissected, and tissue RNA extracted and quantified using ribonuclease protection assay analysis. In day 45 fetal monkeys, dopamine D1 and D2 receptor subtype mRNAs and DAT mRNA were found in low quantities both in control and cocaine-treated subjects. In day 60 fetal monkeys, D1 receptor mRNA levels were highest in the frontal cortex/striatal area, and low to moderate quantities were found in diencephalic and mesencephalic fetal brain regions. Dopamine D2 receptor mRNA levels were highest in the frontal cortex/striatal area, diencephalon and the midbrain, moderate in the brainstem and low in the caudal temporal lobe and surrounding cortical areas. Dopamine D5 receptor mRNA was expressed in low quantities throughout the day 60 fetal monkey brain, whereas DAT mRNA was found in the midbrain only. In utero cocaine exposure caused a significant increase in dopamine D1, D2 and D5 receptor subtype mRNAs in the frontal cortex/striatal area of day 60 fetal monkeys. These results support the hypothesis that dopamine synthesis and release may be reduced in cocaine-treated fetuses, which results in dopamine receptor up-regulation.  相似文献

In 50 patients aged 60 +/- 4 operated for colorectal carcinoma simultaneous combined radionuclide phlebography (RNP) and pulmonary perfusion scintigraphy (PPS) using 99Tcm labeled macroaggregates of the human serum albumin (MAHSA) were performed within 20 postoperative days aiming to detect deep vein thrombosis (DVT) and pulmonary thromboembolism (PTE). The aim of the study was to determine the incidence and segmental DVT localization as well as incidence, localization and clinical characteristics of developed pulmonary perfusion disorders. Deep vein thrombosis was detected in 33 (66%) patients with rather uniform distribution in vein segments. According to their scintigraphic characteristics the findings suggested recent thrombosis in almost all cases (only two of them had signs of chronic thrombosis). Of patients with detected DVT 17 (52%) had pulmonary perfusion disorders of which 10 (59%) were unilateral (7 right and 3 left) and 7 (42%) bilateral. Characteristics and extent of perfusion defects suggested very probable PTE in 11 (65%) patients and less probable in 6 (35%). It has been concluded that patients operated for colorectal carcinoma were highly exposed to DVT and PTE development which necessitates all measures contributing to their prevention.  相似文献
