Definition of the problem The development of personalized medicine is currently a popular topic. Particularly the personalized pharmacotherapy is a matter of relative importance due to pharmacological and molecular–genetic developments. For this reason it is required to examine the implications of personalized pharmacotherapy to statutory health insurance and health care of patients. Arguments In this context the question of orphanization arises: If medical drugs of personalized medicine could be regularly indicated as orphan drugs, i.e., as drugs to medicate orphan diseases, it would not be necessary according to the “Arzneimittelneuordnungsgesetz” (AMNOG) to prove an auxiliary value within the assessment of the “Gemeinsamer Bundesausschuss” (GBA). Crucial to the drug’s indication as an orphan drug is among other things the rate of prevalence of the patient group, whose disease should be medicated with the drug. In this connection, there is importance attached to the stratification of patient groups within personalized medicine. Conclusion An orphanization causes consequences in terms of prices to the statutory health insurance due to the lack of knowledge about an auxiliary value. For patients, the implications must be considered in a more differentiated manner.  相似文献

In order to reduce the computational complexity of model predictive control (MPC) a proper input signal parametrization is proposed in this paper which significantly reduces the number of decision variables. This parametrization can be based on either measured data from closed-loop operation or simulation data. The snapshots of representative time domain data for all manipulated variables are projected on an orthonormal basis by a Karhunen-Loeve transformation. These significant features (termed principal control moves, PCM) can be reduced utilizing an analytic criterion for performance degradation. Furthermore, a stability analysis of the proposed method is given. Considerations on the identification of the PCM are made and another criterion is given for a sufficient selection of PCM. It is shown by an example of an industrial drying process that a strong reduction in the order of the optimization is possible while retaining a high performance level.  相似文献

In this article, the authors introduce and test a group-level perspective on the role of empathy and interpersonal attraction in helping. In line with our predictions, Study 1, a longitudinal field study of 166 AIDS volunteers, confirmed that empathy was a stronger predictor of helping when the recipient of assistance was an in-group member than when that person was an out-group member. Also as hypothesized, attraction was a stronger predictor of helping when the recipient was an out-group member than when that person was an in-group member. Study 2 replicated and further extended these results in a laboratory experiment on spontaneous helping of a person with hepatitis. Strengthening the validity of the findings, in both studies the effects of empathy and attraction held up even when the authors statistically controlled for potential alternative predictors of helping. The concept of user-centred systems design (UCSD) has no agreed upon definition. Consequently, there is a great variety in the ways it is applied, which may lead to poor quality and poor usability in the resulting systems, as well as misconceptions about the effectiveness of UCSD. The purpose of this paper is to propose a definition of UCSD. We have identified 12 key principles for the adoption of a user-centred development process, principles that are based on existing theory, as well as research in and experiences from a large number of software development projects. The initial set of principles were applied and evaluated in a case study and modified accordingly. These principles can be used to communicate the nature of UCSD, evaluate a development process or develop systems development processes that support a user-centred approach. An efficient, accurate and practical process for prioritising requirements is of great importance in commercial software developments. This article improves an existing cost-value approach in which stakeholders compare all unique pairs of candidate requirements according to their value and their cost of implementation. Techniques for reducing the required number of comparisons are suggested, thus making the process more efficient. An initial approach for managing requirements interdependencies is proposed. A support tool for the improved process has been developed to make the process more practical in commercial developments. The improved process and its support tool have been applied and evaluated in an industrial project at Ericsson Radio Systems AB. The results indicate a pressing need for mature processes for prioritising requirements, and the work presented here is an important step in that direction.  相似文献

Generating a regular season schedule is a demanding task for any sports league. In Europe, the creation of a suitable schedule for every national top soccer league not only has to address numerous conflicting inner-league requirements and preferences. Additionally, the games of the European Cup matches (Champions League, UEFA Cup, National Cup Winners) have to be taken into account. In this paper we consider the case of Austria and Germany, that is the planning problem the “Deutsche Fußball-Bund” (DFB) and the “Österreichische Fußball-Bund” (ÖFB) are confronted with. For both leagues we develop models and algorithms which yield reasonable schedules quickly. The models borrow their expressive power from so-called partially renewable resources. Our approach generates schedules which have been accepted for play once by the DFB and six times by the ÖFB.  相似文献

The performance of transcritical R744 systems with direct expansion (DX) can be significantly improved by implementing a Flash Gas Bypass (FGB). The idea behind the concept is to bypass refrigerant vapor, created during the isenthalpic expansion process, around the evaporator. By feeding the evaporator with liquid refrigerant, pressure drop is reduced and refrigerant distribution is improved. With R744 as the working fluid, increased refrigerant-side heat transfer coefficients are expected as well. In addition, the FGB concept proves to be beneficial in terms of system design, in particular for combined air-conditioning and heat pumping applications. An experimental comparison to a conventional DX-system reveals that FGB increases the cooling capacity and COP at the same time by up to 9 and 7%, respectively. Even larger improvements are possible in case a variable speed compressor is utilized to match the performance of the conventional DX-system. A simulation model helps to separate the individual improvement mechanisms. It was found that the reduction of refrigerant-side pressure drop is the dominant improvement mechanism of FGB.  相似文献

Advanced personalized e-applications require comprehensive knowledge about their users’ likes and dislikes in order to provide individual product recommendations, personal customer advice, and custom-tailored product offers. In our approach we model such preferences as strict partial orders with “A is better than B” semantics, which has been proven to be very suitable in various e-applications. In this paper we present preference mining techniques for detecting strict partial order preferences in user log data. Real-life e-applications like online shops or financial services usually have large log data sets containing the transactions of their customers. Since the preference miner uses sophisticated SQL operations to execute all data intensive operations on database layer, our algorithms scale well even for such large log data sets. With preference mining personalized e-applications can gain valuable knowledge about their customers’ preferences, which can be applied for personalized product recommendations, individual customer service, or one-to-one marketing.  相似文献

Investigating the dynamical and physical properties of cosmic dust can reveal a great deal of information about both the dust and its many sources. Over recent years, several spacecraft (e.g., Cassini, Stardust, Galileo, and Ulysses) have successfully characterised interstellar, interplanetary, and circumplanetary dust using a variety of techniques, including in situ analyses and sample return. Charge, mass, and velocity measurements of the dust are performed either directly (induced charge signals) or indirectly (mass and velocity from impact ionisation signals or crater morphology) and constrain the dynamical parameters of the dust grains. Dust compositional information may be obtained via either time-of-flight mass spectrometry of the impact plasma or direct sample return. The accurate and reliable interpretation of collected spacecraft data requires a comprehensive programme of terrestrial instrument calibration. This process involves accelerating suitable solar system analogue dust particles to hypervelocity speeds in the laboratory, an activity performed at the Max Planck Institut fu?r Kernphysik in Heidelberg, Germany. Here, a 2 MV Van de Graaff accelerator electrostatically accelerates charged micron and submicron-sized dust particles to speeds up to 80 km s(-1). Recent advances in dust production and processing have allowed solar system analogue dust particles (silicates and other minerals) to be coated with a thin conductive shell, enabling them to be charged and accelerated. Refinements and upgrades to the beam line instrumentation and electronics now allow for the reliable selection of particles at velocities of 1-80 km s(-1) and with diameters of between 0.05 μm and 5 μm. This ability to select particles for subsequent impact studies based on their charges, masses, or velocities is provided by a particle selection unit (PSU). The PSU contains a field programmable gate array, capable of monitoring in real time the particles' speeds and charges, and is controlled remotely by a custom, platform independent, software package. The new control instrumentation and electronics, together with the wide range of accelerable particle types, allow the controlled investigation of hypervelocity impact phenomena across a hitherto unobtainable range of impact parameters.  相似文献
