A retrospective study of neonatal meningitis for a period of 6 years (1970 to 1975) is described. It was found that though the survival rate had increased since the introduction of the aminoglycosides, it was at the expense of major neurological sequelae in the survivors. It is suggested that breast feeding of all infants in the first few days of life will reduce the incidence of neonatatl meningitis. It is recommended that pooled human breast milk should not be heat-sterilized as this destroys anti-Escherichia coli immunoglobulin A. 相似文献

IFNs were first described as potent antiviral agents 40 years ago, and recombinant IFN-alpha2a and IFN-alpha2b were approved for the treatment of hairy cell leukemia just 11 years ago. Today, alpha-IFNs are approved worldwide for the treatment of a variety of malignancies and virologic diseases. Although the exact mechanism of action of IFN-alpha in the treatment of such diseases is not fully understood, many advances have been made in the characterization of the physicochemical and diverse biological properties of this highly pleiotropic cytokine. Here we review recent developments in our understanding of the antiviral and immunoregulatory properties of IFN-alpha, the nature of the multisubunit IFN-alpha receptor, and the molecular mechanisms of signal transduction. Where available, we have included comparative data on recombinant alpha-IFNs derived from both naturally occurring and nonnaturally occurring synthetic genes. We also review clinical data and data on the side effects and antigenicity of different sources of recombinant alpha-IFNs in humans. These latter topics are of clinical interest, because they may potentially affect the efficacy of these various products. Hopefully, what is already known about IFN will prompt further exploration into the mechanism(s) of action of IFN-alpha and thus deliver new applications for this prototypic cytokine, whose full therapeutic potential is yet to be realized. 相似文献

Most school-based smoking prevention studies employ designs in which schools or classrooms are assigned to different treatment conditions while observations are made on individual students. This design requires that the treatment effect be assessed against the between-school variance. However, the between-school variance is usually larger than the variance that would be obtained if students were individually randomized to different conditions. Consequently, the power of the test for a treatment effect is reduced, and it becomes difficult to detect important treatment effects. To assess the potential loss of power or to calculate appropriate sample sizes, investigators need good estimates of the intraclass correlations for the variables of interest. The authors calculated intraclass correlations for some common outcome variables in a school-based smoking prevention study, using a three-level model-i.e., students nested within classrooms and classrooms nested within schools. The authors present the intraclass correlation estimates for the entire data set, as well as separately by sex and ethnicity. The effects of kinetic parameters Hatta number Ha and ?, and transport parameters Biot number Bi and ψ on the migration of a reactive additive have been modeled. The convective boundary layer can be considered inert to fully reactive for Ha → 0 to 2, respectively. The migration is controlled by diffusion, reaction rate, equilibrium partitioning of the migrant between the plastic film and the solvent, and boundary layer resistance, depending on the time scale and values of the above parameters. For a nonreactive boundary layer, at high Bi, migration is initially diffusion-controlled but becomes reaction-rate controlled at later times. However, under similar conditions, a slight increase in the reactivity of the boundary layer immediately changes the transport process from diffusion-controlled to reaction rate-controlled. With further increase in reactivity, migration spontaneously reaches equilibrium. At low Bi, equilibrium partitioning and reactive depletion of the migrant in the fluid phase change predominantly to boundary layer-controlling mass transport phenomenon. For a given sampling time (τ = 1.0), the migration increases with the increase in Bi only for a reactive boundary layer. The two temperature-sensitive (ts) lesions present in influenza A/Hong Kong/68-ts-1[E] (H3N2 68) virus were transferred via genetic reassortment to influenza A/Georgia/74 (H3N2 74) wild-type virus. A recombinant clone possessing both ts lesions and the shutoff temperature of 38 C of the Hong Kong/68 ts donor and the two surface antigens of the Georgia/74 wild-type virus was administered to 32 seronegative adult volunteers. Thirty-one volunteers were infected, of whom only five experienced mild afebrile upper respiratory tract illness. The wild-type recipient virus was a cloned population that induced illness in five of six infected volunteers. Therfore, the attenuation exhibited by the Georgia/74-ts-1[E] virus could reasonably be assumed to be due to the acquisition of the two ts-1[E] lesions by the Georgia/74 wild-type virus. The serum and nasal wash antibody responses of the ts-1[E] vaccinees were equivalent to those of the volunteers who received wild-type virus. The two ts lesions present in the Hong Kong/68-ts-1[E] virus have now been transferred three times to a wild-type virus bearing a new hemagglutinin, and in each instance the new ts recombination exhibited a similar, satisfactory level of attenuation and antigenicity for adults. It seems likely that the transfer of the ts-1[E] lesions to any new influenza virus will regularly result in attenuation of a recombinat virus possessing the new surface antigens. 相似文献

Flash floods leading to complete submergence of rice plants for 10–15 days is one of the major constraints for rice production, mainly in rainfed lowland areas. In India, 30% of the rice growing area (12–14 M ha) is prone to flash flooding with average productivity of only 0.5–0.8 t ha?1. Coastal Odisha is one of the most flood prone areas of India where the paddy crop in the wet season is often devastated by flash floods, forcing farmers either to discontinue paddy cultivation or to revert to traditional varieties with very low yields. The flood tolerant variety, Swarna-Sub1 (SS1), was introduced through cluster demonstrations in some villages of the Bari block of Odisha in 2013. SS1 seeds were provided to 355 farmers in two villages affected by floods twice in 2013. The majority of the farmers cultivated SS1 in 2013 and about 75% of them persisted with the variety in 2014. There has been a steady and significant increase in paddy cultivation and yields per unit area between 2012 and 2014 and a sharp increase in paddy sales during 2013 and 2014. The present study showed that farmers preferred to cultivate low yielding traditional land races owing to their better flood tolerance and the unavailability of flood tolerant high yielding varieties (HYVs). Traditional varieties gave 30–42% higher yields than non-SS1 HYVs when flooding occurred. SS1 on the other hand offered a significant yield advantage of about twice that of traditional varieties, in both flooding and non-flooding years. There is an additional social advantage of SSI in that famers belonging to scheduled castes in flood prone villages, have greater numbers of plots that are exposed to prolonged floods.
