STUDY OBJECTIVE: To determine if the addition of alfentanil to propofol is more effective than propofol alone to provide adequate conditions for placement of a retrobulbar block prior to cataract surgery. DESIGN: Randomized, double-blinded study. SETTING: Outpatients at a university hospital. PATIENTS: 40 adult ASA physical status I, II, and III outpatients scheduled for elective cataract surgery. INTERVENTIONS: Patients were randomly assigned to receive one of four drug combinations prior to the placement of a retrobulbar block: Group 1, propofol alone; Group 2, alfentanil 5 micrograms/kg plus propofol; Group 3, alfentanil 10 micrograms/kg plus propofol; Group 4, alfentanil 15 micrograms/kg plus propofol. All patients were preoxygenated by face mask for two minutes prior to drug administration. The quality of conditions for block placement were determined by: (1) assessing the amount of movement by the patients while the block needle was in place, (2) cooperativeness of the patients during the operation, (3) hemodynamic side effects, (4) incidence and severity of respiratory depression, (5) incidence of nausea and vomiting, (6) recall of placement of the block, and (7) time to discharge from the hospital. Measurements and Main Results: The addition of alfentanil to propofol for sedation prior to placement of the retrobulbar block resulted in a dose-dependent reduction in movement by the patients. However, the highest dose of alfentanil (15 micrograms/kg) resulted in the greatest frequency (40% of the patients in this group) of respiratory depression (SpO2 < 90%). All patients were cooperative during the operation and responsive to verbal command within 5 minutes of placement of the block. In addition, all of the patients denied being nauseated, having vomited, or recalling block placement in the recovery room or the next day. CONCLUSIONS: The combination of alfentanil and propofol may be used to sedate patients in order to limit movement and provide a cooperative, alert patient with stable hemodynamics and limited respiratory depression during placement of retrobulbar block prior to ophthalmic surgery. However, excessive dosage of these drugs may result in hazardous respiratory depression in this patient population.  相似文献

The structure and properties of solid solutions and the chemical compound Gd2Zr2O7 in the Gd2O3 - ZrO2 system are investigated. Sintering of the initial mixture is studied and the optimum concentration of the stabilizing additive is determined for the production of heat-resistant articles from granular mixtures based on ZrO2 stabilized by Gd2O3 and fired at 1750°C. The involvement of the intermediate area and B?tzinger complex (BOT) of the rostral ventral respiratory group (r-VRG) in laryngeal control and generation of the expiration reflex were studied in anaesthetized non-paralyzed cats Focal cooling (to 20 degrees C) of the nucleus paraambigualis (NPA) caused changes in the frequency and timing of breathing with the concomitant rise in laryngeal resistance. Cooling of the nucleus ambiguus resulted in a consistent drop in laryngeal resistance. Alterations in timing and intensity of breathing but no changes in laryngeal patency were found during cooling of the BOT. The expiration reflex was inhibited by cooling of either the NPA or BOT. The role of these medullary regions in the control of laryngeal patency and central integration of the expiration reflex is discussed.  相似文献

An experiment to determine the effect of gate electrode resistivity on circuit speed gave unexpected results: circuits with the lowest sheet resistance had the poorest circuit speed. Explanation of this behaviour required development of a new high-frequency method of measuring the impedance of the gate electrode. This method revealed that the circuits with a composite gate electrode had been formed with a partial discontinuity. The measurement technique is described, and the evidence of the discontinuity is shown. The effect of the discontinuity on device and circuit speed is demonstrated  相似文献

For the third project of the Hokkaido-Honshu HVDC Link in Japan, called the HVDC Link III project (rated at 250 kVDC-1200 A-300 MW), we developed an HVDC transmission line protection method based on a new working principle that allows high-speed and highly sensitive detection of faults, enhancing reliability in the supply of electric power. In general, increasing the sensitivity of relays will lead to an increased likelihood of undesired operation whereas lowering the sensitivity will impair the responsiveness of the relays. Our proposed method meets these apparently incompatible requirements very well. Basically classified as a differential scheme, the HVDC transmission line protection method compensates for a charging and discharging current that flows through the line-to-ground capacitance at times of voltage variations caused by a line fault or by the operation of DC power systems. The developed protection method is also characterized in that it uses current changes induced by voltage variations to restrain the operation of a relay. This configuration has made the proposed method far superior in responsiveness and sensitivity to the conventional protection method. A simulation using an EMTP (Electro-Magnetic Transients Program) was conducted on this method. Developed relay equipment embodying the new protection method was subjected to various verification tests, where this equipment was connected to a power system simulator, before being delivered to the HVDC Link III facility  相似文献
