Two cases with acute myeloblastic leukemia (AML M4-FAB) associated with diabetes insipidus (DI) are presented here. Both patients presented with hyperleucocytosis. One had a white blood cell count (WBC) of 150 x 10(9)/L and the second patient had 200 x 10(9)/L. One of these patients was a 40 year-old male and MRI of the hypophysis showed an infindibuler mass. This patient did not respond to remission induction chemotherapy and reinduction chemotherapy was given. The other patient was a 16-year-old male with a normal CT scan of the head. Both patients had DI with typical clinical and laboratory findings. The first patient died early on during reinduction chemotherapy and the second patient died of intracranial bleeding before induction chemotherapy was given. These findings are consistent with the data in the literature suggesting that the prognosis of AML associated with DI is poor and that these cases generally present with hyperleucocytosis.  相似文献

OBJECTIVE: To determine the frequency of late-onset adrenal hyperplasia (LOCAH) due to 21-hydroxylase (21-OH) and 11 beta-hydroxylase (11 beta-OH) deficiency in women with clinical and biochemical features of polycystic ovary syndrome (PCOS). DESIGN: Eighty-three consecutively selected women with PCOS and eighteen normal women were included in the study. METHODS: Ultrasound, clinical and hormonal parameters were used to define PCOS. Basal FSH, LH, testosterone, free testosterone, androstenedione, dehydroepiandrosterone sulfate (DHEA-S), sex hormone-binding globulin (SHBG) and cortisol levels were measured. Serum 17-hydroxyprogesterone (17-OHP) and 11-deoxycortisol (11-DOC) levels were also measured before, 30 and 60 min after a single bolus injection of 0.25 mg ACTH (1-24) at 0900 h during the mid-follicular phase of the cycle. ACTH-stimulated 17-OHP levels > 30 nmol/l were considered as the criteria of 21-OH deficiency. The diagnosis 11 beta-OH deficiency was made if the adrenal 11-DOC response to ACTH stimulation exceeded threefold the 95th percentile of controls. RESULTS: Basal serum testosterone, free testosterone, androstenedione, DHEA-S, cortisol and 11-DOC levels were significantly higher in PCOS than in control subjects. ACTH-stimulated 17-OHP (P < 0.05) and 11-DOC (P < 0.0005) levels were found to be significantly higher in patients with PCOS than in controls. Seven (8.4%) patients had an 11-DOC response to ACTH higher than threefold the 95th percentile of controls, while no patients showed evidence of 21-OH deficiency. CONCLUSIONS: We have found that 8.4% of the women with clinical and biochemical features of PCOS could be presumed to have 11 beta-OH deficiency. No patients among the women with PCOS showed evidence of 21-OH deficiency. 11 beta-OH deficiency is unexpectedly more common than 21-OH deficiency in women with PCOS.  相似文献

The basic hydrometallurgical operation of the tungsten filament manufacturing, the molybdenum mandrel wire dissolving has been reviewed from environmental and hygienic aspects presenting the published efforts for streamlining of the traditional H2SO4---HNO3 mixture based process.

The fundamentals of a new environment-preserving and hygienic process based on H2O2-catalyst mixture are outlined as well as its development and industrial implementation to a zero effluent discharging process and equipment consisting of selective molybdenum dissolving from tungsten coils and the recovery of the dissolved molybdenum and process water.

Some desirable development potentials of the new process are sketched considering scientific, technical, economical and environmental aspects.  相似文献

In Turkey, there is a much more potential for renewables, but represent about 37% of total energy production and 10% of total energy consumption. This share is not enough for the country and the governments should be increase to this situation. Renewable energy technologies of wind, biomass, hydropower, geothermal, solar thermal and photovoltaics are finally showing maturity and the ultimate promise of cost competitiveness. With respect to global environmental issues, Turkey's carbon dioxide emissions have grown along with its energy consumption. States have played a leading role in protecting the environment by reducing emissions of greenhouse gases. In this regard, renewable energy resources appear to be the one of the most efficient and effective solutions for clean and sustainable energy development in Turkey. Turkey's geographical location has several advantages for extensive use of most of these renewable energy sources. Certain policy interventions could have a dramatic impact on shaping the relationship between geological, geographic and climatic conditions and energy production. This study shows that there is enough renewable energy potential in Turkey for fuels and electricity. Especially hydropower and biomass are very well.  相似文献
