Involuntary contractions of the striated anal sphincters as a cause of constipation: report of a case

PURPOSE: To report a case of corneal ulcer associated with deposits of norfloxacin. METHOD: Case report. A 40-year-old man with right trigeminal and facial nerve palsies and decreased tear secretion developed a corneal ulcer with white deposits in the right eye. The deposits were removed and analyzed by high-performance liquid chromatography. RESULTS: High-performance liquid chromatography results disclosed that the deposits on the corneal surface had the same retention time as norfloxacin. The patient discontinued norfloxacin ophthalmic solution and recovered successfully. CONCLUSION: Clinicians should be aware that frequent applications of topical norfloxacin in patients with decreased tear secretion may result in deposition of the drug on the cornea.     

Nutritional status and serum lipids of a rural population in Northeast Thailand--an example of health transition

The authors retrospectively evaluated the etiology and clinical findings of patients with first manifestations of psychotic symptoms after the age of 65. Nearly 10% of over 1,700 consecutive geriatric patients admitted to an acute inpatient psychogeriatric unit had late-life onset psychotic symptoms. About three-fourths of these were women, usually in their seventies. Dementia of the Alzheimer's type was the most common cause of psychosis arising in late life, followed by major depression, medical/toxic causes, delirium, bipolar disorder, delusional disorder, schizophrenia, and schizoaffective disorder. Clinical manifestations consisted mostly of delusions and hallucinations.     

Functional magnetic resonance imaging of motor cortex: hemispheric asymmetry and handedness

A hemispheric asymmetry in the functional activation of the human motor cortex during contralateral (C) and ipsilateral (I) finger movements, especially in right-handed subjects, was documented with nuclear magnetic resonance imaging at high field strength (4 tesla). Whereas the right motor cortex was activated mostly during contralateral finger movements in both right-handed (C/I mean area of activation = 36.8) and left-handed (C/I = 29.9) subjects, the left motor cortex was activated substantially during ipsilateral movements in left-handed subjects (C/I = 5.4) and even more so in right-handed subjects (C/I = 1.3).     

Characterization of clay scales forming in Philippine geothermal wells

Smectite scales occur in 24 out of 36 blocked wells located in Tongonan, Palinpinon and Bacon-Manito. These comprise 2–85% of the well scales and form at depths of 36–2620 m. where measured and fluid inclusion temperatures are 41–320°C. Most, however, occur below the production casing shoe where temperatures are 230°C, often at depths coinciding with aquifers. The clay scales are compositionally and structurally different from the bentonite used in drilling, which is essentially a sodium-rich montmorillonite. The clay deposits are expanding, generally disordered, and combine the characteristics of a montmorillonite, saponite and vermiculite in terms of reaction to cationic exchange treatments, structure and composition. Six types of clay scales were identified, but the predominant one, comprising 60–100% of the clay deposits in a well, is Mg- and Fe-rich and referred to as a vermiculitic species. The crystallinity, degree of disorder, textures, optical characteristics, structure and relative amounts of structural Al, Mg and Fe vary with time, temperature and fluid composition, but not with depth and measured pressure. Despite its variance from bentonite characteristics one of the dominant suggested mechanisms for clay scale formation uses the drilling mud in the well as a substrate, from which the Mg and Fe-rich clay evolves. Another important possible mechanism for formation of the clay scale is the precipitation of the clays from hydrothermal fluids, wherein Mg and Al appear to be the limiting elements. Other, less important, mechanisms of formation are: alteration of chloritized cuttings in the well to the vermiculitic species and the probable introduction of smectite from the formation.     

Coping disposition, perceived risk, and psychological distress among women at increased risk for ovarian cancer.

The authors examined predictors of psychological distress among women who were at increased risk for ovarian cancer. Participants were 103 women who had at least 1 first degree relative with ovarian cancer. Specifically, the authors tested the relationship between the dispositional attentional style of monitoring (the tendency to scan for threat-relevant information), perceptions of risk for ovarian cancer, intrusive thoughts regarding ovarian cancer, and psychological distress. Overall, this sample exhibited moderately high levels of psychological distress. High scores on monitoring were associated with high perceived risk for ovarian cancer and elevated levels of intrusive thoughts and psychological distress. Finally, the authors proposed and tested a path model describing the interrelationships between these variables. Results of this study are discussed in terms of their implications for treating the psychological distress associated with being at increased risk for ovarian cancer. Helper activity of chicken V beta 1+ and V beta 2+ alpha beta T cells for in vitro IgA antibody synthesis

Chickens have only two T cell receptor variable beta gene families: V beta 1 and V beta 2 (1). In our previous work we found that IgA production was almost completely suppressed in chickens depleted of V beta 1+ alpha beta T cells by treatment with a TCR V beta 1-specific monoclonal antibody (2), while IgM and IgG production was not affected. Our present results indicate that, in vitro, both V beta 1+ and V beta 2+ chicken cecal tonsil T cells provide help for the differentiation of cecal tonsil IgA B cells, suggesting that the failure of V beta 1+ T cell-depleted chickens to produce IgA is not caused by the inability of V beta 2+ T cells to provide help for IgA production by B cells, but rather by the scarcity of these T cells in mucosal tissues (3), where most IgA responses are induced (4).     

Lymphoma: role of whole-body 2-deoxy-2-[F-18]fluoro-D-glucose (FDG) PET in nodal staging

PURPOSE: To compare 2-deoxy-2-[fluorine-18]fluoro-D-glucose (FDG) positron emission tomography (PET) with computed tomography (CT) in primary nodal staging of malignant lymphoma. MATERIALS AND METHODS: Sixty consecutive patients with untreated, histopathologically proved malignant lymphoma (aged 7-72 years; 33 with non-Hodgkin lymphoma, 27 with Hodgkin disease) underwent FDG PET and contrast material-enhanced CT for nodal staging. Lymph node regions identified at both CT and PET were regarded as actual locations of disease. Discordant results were verified with biopsy or clinical follow-up whenever possible. RESULTS: One hundred sixty of 740 evaluated lymph node regions were identified as diseased at both CT and PET. Of the 25 additional regions seen with PET, seven were true-positive; two, false-positive; and 16, unresolved. CT showed six additional disease manifestations; three were false-positive, and three were unresolved. Staging was changed in the four patients with the seven confirmed additional PET findings: from stage I to II in one patient and from stage II to III in three patients. Staging was changed from stage II to I in one of the three patients with false-positive CT findings. CONCLUSION: FDG PET may be more accurate for detecting nodal lymphoma than incremental CT.
