BACKGROUND: We have studied 64 patients with congestive heart failure, half of them also with chronic nonvalvular atrial fibrillation (AF). Patients were also stratified according to a history of prior stroke. METHODS: The generation of thrombin was investigated by means of the molecular markers prothrombin fragment 1 + 2 (F1 + 2) and thrombin-antithrombin III complex (TAT), because AF patients may have a hypercoagulable state. There was only a trend toward higher values of TAT and F1 + 2 for AF patients, while subjects with previous stroke (irrespective of AF) had increased levels of the markers of thrombin generation (TAT stroke+ 18.95 +/- 5.15 vs TAT stroke- 8.34 +/- 2.41; F1 + 2 stroke+ 2.22 +/- 0.29 vs F1 + 2 stroke- 1.32 +/- 0.12). The presence of spontaneous echo contrast (SEC) within left atrium was also investigated in 32 AF patients by transesophageal echocardiography. RESULTS: TAT were significantly higher in subjects (n = 11) with SEC (TAT sec+ 37.5 +/- 13.41 vs TAT sec- 8.7 +/- 2.51, p = 0.008). CONCLUSIONS: Finally, when we grouped into 1) those with both AF and stroke, 2) AF alone, 3) stroke alone and 4) sinus rhythm without stroke, levels of F1 + 2 were higher (and marginally higher TAT) in patients with AF and stroke than in those without stroke, revealing that there is a true clotting activation state in these subjects.  相似文献

Crustacean and insect neuromuscular junctions typically include numerous small synapses, each of which usually contains one or more active zones, which possess voltage-sensitive calcium channels and are specialized for release of synaptic vesicles. Strength of transmission (the number of quantal units released per synapse by a nerve impulse) varies greatly among different endings of individual neurons, and from one neuron to another. Ultrastructural features of synapses account for some of the physiological differences at endings of individual neurons. The nerve terminals that release more neurotransmitter per impulse have a higher incidence of synapses with more than one active zone, and this is correlated with more calcium build-up during stimulation. However, comparison of synaptic structure in neurons with different physiological phenotypes indicates no major differences in structure that could account for their different levels of neurotransmitter release per impulse, and release per synapse differs among neurons despite similar calcium build-up in their terminals during stimulation. The evidence indicates differences in calcium sensitivity of the release process among neurons as an aspect of physiological specialization.  相似文献

OBJECTIVE: To investigate the efficacy of the rectus abdominis muscle flap (RAMF) technique for the closure and augmentation of small, non-compliant bladder exstrophies. PATIENTS AND METHODS: The RAMF technique was used in two girls and two boys (mean age at operation, 31.7 months, range 3-72) with bladder exstrophy. The clinical outcome and urodynamics were assessed during a follow-up of 29 months to 6 years (mean 49.2 months) and included imaging, cystoscopy, biochemical and microbiological studies. RESULTS: There were no urinary tract infections, metabolic problems or electrolyte disturbances and kidney function remained normal in all patients. Radiography confirmed intact function and anatomy of the urinary tract and cystoscopy showed complete coverage of the inner peritoneal layer of RAMF with uroepithelium. No stone formation or mucus production were detected. Currently, three patients void using clean intermittent catheterization through the native urethra and the fourth through an appendiceal Mitrofanoff valve. However, the bladder volume was insufficient at the late follow-up and three patients required bladder augmentation. CONCLUSIONS: The RAMF technique is a good alternative for closing bladder exstrophies and achieves an increase in bladder capacity; however, although there is a mild improvement in bladder capacity and compliance. RAMF should not be used as a bladder augmentation procedure. The technique is indicated in the closure of large bladder defects, bladder exstrophies with small, inelastic, non-compliant bladder remnants and failed primary closures.  相似文献

Using an enhanced multiple output domino logic (EMODL) implementation of a carry lookahead adder (CLA), sums of several consecutive bits can be built in one nFET tree with a single carry-in. Based on this result, a new sparse carry chain architecture is proposed for the CLA adder. We demonstrate the design approach using a 32-b adder, and show that only four carries are sufficient for generating all sums, with a consequent reduction in the number of stage delays. Using a 1.2-μm CMOS technology, we verify our simulation procedures by fabrication and measurement of a 2.7 ns critical path  相似文献

A series active power filter working as a sinusoidal current source, in-phase with the mains voltage, has been developed and tested. The amplitude of the fundamental current in the series filter is controlled through the error signal generated between the load voltage and a pre-established reference. The control allows an effective correction of power factor, harmonic distortion and load voltage regulation. Compared with previous methods of control developed for series active filters, this method is simpler to implement because it is only required to generate a sinusoidal current, in-phase with the mains voltage, the amplitude of which is controlled through the error in the load voltage. The proposed system has been studied analytically and tested using computer simulations and experiments. In the experiments, it has been verified that the filter keeps the line current almost sinusoidal and in-phase with the line voltage supply. It also responds very quickly under sudden changes in load conditions, reaching its steady-state in about two cycles of the fundamental  相似文献

In this paper, we examine, both experimentally and theoretically, the kinetics of formation and microstructure of product phases in thin film reactions, using the Nb/Al and Ti/Al systems as our prototypes. The results of calorimetry and microscopy studies are interpreted using simple kinetic and morphology models. In particular, the kinetic models employed here focus on the nucleation and growth components of the phase formation process and the morphology models provide a starting point for the classification of product grain structures. The influence of nonlinear gain and optical feedback on the dynamics of single-mode semiconductor lasers are numerically investigated based on the Lang and Kobayashi model. It is well known that the nonlinear gain tends to stabilize the dynamics, while the optical feedback tends to increase the instabilities. In this paper, we study the behavior of the attractors when the feedback level k and the gain saturation coefficient ε vary and show that the effects of these parameters are surprisingly opposite. For example, we find that the route to chaos that the external cavity modes follow for increasing k is reversed for increasing ε in an almost identical manner. When the feedback increases the modes follow the usual quasi-periodic route and turn into torus. If k continues to increase, the torus become chaotic attractors as the result of several period-doubling bifurcations or a third Hopf bifurcation. Further increase of k causes the chaotic attractors to lose stability, Contrarily, if the value of the parameter ε is increased, the attractors recover their stability and reverse the route becoming simple torus again. If ε is increased further, the torus reverse the quasi-periodic route and turn into stable modes again. We also find that on the contrary to k, the parameter ε enhances the stability region of an attractor. We show that the feedback level above which a limit cycle emerges from a stable mode, the feedback level above which a torus emerges from a limit cycle, and the feedback level above which a chaotic attractor loses stability are all increasing functions of ε  相似文献

Quenching of Er3+ ions by homogeneous energy-transfer upconversion in high-concentration erbium-doped silica glasses has been theoretically investigated, The results indicate that at Er3+ concentrations of 1.0-2.0·1026 m-3 or below, the kinetic limit of strong migration is not reached, and hence the widely accepted quadratic upconversion model is not generally valid. Nevertheless, the results offer an explanation of the experimental observations of quadratic upconversion. Furthermore, it has been shown that at a given population inversion, the quenching rate depends on the rate of exchange of the excited Er3+ ions by emission and absorption  相似文献
