Accuracy of diagnoses rendered using a live video telepathology network was assessed for permanent sections of surgical pathology specimens. To determine accuracy, telepathology diagnoses were compared with those obtained by directly viewing the glass slide using a standard microscope. A total of 294 cases were read via both telepathology and glass slide by attending pathologists at a tertiary care medical center. Overall accuracy was defined as exact concordance between diagnoses. Clinically insignificant differences in diagnoses were excluded to determine clinically significant accuracy. For the 285 cases with complete data, the overall accuracy for telepathology was 0.912 (95% confidence interval [CI], 0.872-0.941), whereas the overall accuracy for glass slide readings was 0.968 (95% CI, 0.939-0.985). This difference is statistically significant (p = 0.009). When focusing on clinically significant discrepancies, where the difference in diagnosis might affect therapeutic decisions, the video accuracy was only slightly less than the glass slide accuracy (0.965 [95% CI, 0.934-0.982] vs. 0.982 [95% CI, 0.957-0.994], respectively), but this difference is not statistically significant (p = 0.302). Most of the cases with clinically significant differences involved lesions with inherently high interobserver variation. Certainty of diagnosis did not differ between video and glass slide readings (p = 0.911), but there was an association between certainty of diagnosis and diagnostic accuracy for video (p = 0.003 for clinically significant accuracies). Based on these findings, we recommend when using this telepathology system that only preliminary diagnoses should be given in the following situations: for diagnostic areas with known high interobserver variability; when the consultant has any degree of uncertainty about the presence or absence of the lesion in question; and when there is insufficient experience using telepathology as a diagnostic medium.  相似文献

The use of fluorescence as an analytical technique has been growing over the last 20 years. A major factor in inhibiting more rapid growth has been the inability to make comparable fluorescence intensity measurements across laboratories. NIST recognizes the need to develop and provide primary fluorescence intensity standard (FIS) reference materials to the scientific and technical communities involved in these assays. The critical component of the effort will be the cooperation between the Federal laboratories, the manufacturers, and the technical personnel who will use the fluorescence intensity standards. We realize that the development and use of FIS will have to overcome many difficulties. However, as we outline in this article, the development of FIS is feasible.  相似文献

The phenomenon of thermally induced distortions in unsymmetrical laminates is well understood, and it may be shown that a square, unsymmetrical 0.90 laminate will tend to form two stable geometries with a snap-through phenomenon between them. This paper discusses laminates in which at each point the lay-up is symmetrical across the laminate mid-plane, but which still exhibit multiple stable geometries. The number of stable geometries can be controlled by the details of the lay-up from the minimum of two to, in principle, an unlimited number. In addition it will be shown how a similar process can be used to generate multiple stable stress states and geometries in unidirectional laminates. This paper represents a very preliminary experimental investigation of the design space available for such composite laminates. Possible applications of composites with multiple stable geometries are noted.  相似文献

The thermal conductivity of partially stabilized zirconia was measured over the temperature range 320–1273 K using the radial heat flow method. The data have an absolute uncertainty of about ±2% and repeat measurements showed no evidence of changes in the thermal conductivity at high temperatures. This also was true for the thermal diffusivity data, which were obtained in vacuum over the temperature range 300–1473 K. Both sets of thermal conductivity data pass through minima at high temperatures. Quantitative differences were observed in the temperatures and thermal conductivities of the two minima. The results were analyzed by assuming parallel conduction by phonons and photons, and the phonon component was identified by fitting lower-temperature data. Extrapolating this curve allowed identification of the photon contribution to the thermal conductivity at high temperatures. The photon contribution approached a T 3 function and was larger in the thermal conductivity specimens. The difference in the photon contributions correlates with changes in the optical properties of the samples produced during the high temperature measurments.  相似文献

Heterodimers of the 70 and 80 kDa Ku autoantigens (Ku70 and Ku80) activate the DNA-dependent protein kinase (DNA-PK). Mutations in any of the three subunits of this protein kinase (Ku70, Ku80 and DNA-PKcs) lead to sensitivity to ionizing radiation (IR) and to DNA double-strand breaks, and V(D)J recombination product formation defects. Here we show that the IR repair, DNA end binding and DNA-PK defects in Ku70-/- embryonic stem cells can be counteracted by introducing epitope-tagged wild-type Ku70 cDNA. Truncations and chimeras of Ku70 were used to identify the regions necessary for DNA end binding and IR repair. Site-specific mutational analysis revealed a core region of Ku70 responsible for DNA end binding and heterodimerization. The propensity for Ku70 to associate with Ku80 and to bind DNA correlates with the ability to activate DNA-PK, although two mutants showed that the roles of Ku70 in DNA-PK activation and IR repair are separate. Mutation of DNA-PK autophosphorylation sites and other structural motifs in Ku70 showed that these sites are not necessary for IR repair in vivo. These studies reveal Ku70 features required for double-strand break repair.  相似文献
