Environmental ecological modeling (EEM), which unifies models simulating transport of chemicals and exposure of humans to chemicals, was used to simulate long-term trends of female adult human blood lead levels (BLLs) and historical exposure to the atmospheric lead in four East Asian countries: Japan, Korea, China, and Vietnam. Anthropogenic lead emissions to the atmosphere in Vietnam were estimated from energy statistics to be 1931 t yr(-1). Calculated BLLs generally agreed with those observed in samples collected in these countries as the error factors were less than 2. The model results revealed that BLLs decreased significantly in Tokyo (by 58%) and Seoul (by 45%) in recent decades and confirmed the effects of efforts to reduce environmental lead in Japan and Korea. The model results also revealed that BLLs in Beijing did not decrease in this decade as much as in Tokyo and Seoul, despite the phasing out of leaded gasoline, and that the contribution from the atmospheric component was increasing (43% in 2009). Finally, we applied EEM to simulate BLLs of children in Hanoi. The probability of children having BLLs greater than 50 μg L(-1) was 7.5%, which was greater than those observed in developed countries.  相似文献

New donor–acceptor‐type copolymers comprised of benzobisthiazole (BBTz) as a weak donor rather than acceptor are proposed. This approach can simultaneously lead to deepening the HOMO and LUMO of the polymers with moderate energy offset against fullerene derivatives in bulk heterojunction organic photovoltaics. As a proof‐of‐concept, BBTz‐based random copolymers conjugated with typical electron acceptors: thienopyrroledione (TPD) and benzothiadiazole (BT) based on density functional theory calculations are synthesized. Laser‐flash and Xe‐flash time‐resolved microwave conductivity (TRMC) evaluations of polymer:[6,6]‐phenyl C61 butyric acid methyl ester (PCBM) blends are conducted to screen the feasibility of the copolymers, leading to optimization of processing conditions for photovoltaic device application. According to the TMRC results, alternating BBTz‐BT copolymers are designed, exhibiting extended photoabsorption up to ca. 750 nm, deep HOMO (–5.5 to –5.7 eV), good miscibility with PCBM, and inherent crystalline nature. Moreover, the maximized PCE of 3.8%, the top‐class among BBTz‐based polymers reported so far, is realized in an inverted cell using TiOx and MoOx as the buffer layers. This study opens up opportunities to create low‐bandgap polymers with deep HOMO, and shows how the device‐less TRMC evaluation is of help for decision‐making on judicious molecular design.  相似文献

Using a highly sensitive bioassay for TSH, in which human thyroid follicles incorporate 125I and release de novo synthesized thyroid hormone into the culture medium, the thyrotropic activities of various hCG preparations were studied. Under the culture conditions employed, bovine TSH (bTSH) was approximately 6- to 9-fold more active than human TSH (hTSH). Highly purified hCG prepared from urine of normal pregnant women (CR 127) had only a trivial thyrotropic activity equipotent to 0.00022 microU bTSH/U hCG or 0.0013 microU hTSH/U hCG (19.7 microU hTSH/mg hCG). Hybrid hCG (AB1ER) also elicited low thyrotropic activity (14.0 microU hTSH/mg), whereas crude hCG had moderate thyrotropic activity (0.041 hTSH microU/U hCG or 127 microU/mg protein). Deglycosylated hCG, a very weak LH/hCG receptor agonist, was the most potent agonist in thyroid follicles (588 microU hTSH/mg protein). hCGs purified from urine of patients with trophoblastic tumors had greater TSH-like activity (37-84 microU hTSH/mg protein) than purified hCG. Asialo-hCG purified from a patient with choriocarcinoma had very potent TSH-like activity (468 microU hTSH/mg). Submaximal doses of bTSH and hCG variants produced additive stimulation of thyroid function. Furthermore, the thyrotropic effect of hCG was inhibited by anti-TSH receptor antibody obtained from patients with myxedema. These in vitro findings suggest that although hCG is reported to exert potent cAMP-stimulating activity on rat thyroid-like cells (FRTL-5) and Chinese hamster ovary cells transfected with hTSH receptor complementary DNA (0.092-0.72 microU hTSH/U hCG), the thyrotropic activity induced by authentic hCG in human thyroid follicles is too weak to cause hyperthyroidism in normal pregnancy. However, hCG produced by some trophoblastic tumors, particularly asialo-hCG, has potent thyrotropic activity sufficient to cause clinically overt hyperthyroidism when produced excessively.  相似文献

To clarify the structure and function of the airways in Mycobacterium avium-intracellulare (MAI) infection, we performed pulmonary function tests and high-resolution computed tomography (HRCT) of the thorax in female patients 61 +/- 9 yr of age (n = 12) with pulmonary MAI infection without predisposing lung disease and compared their data with those of normal female volunteers 54 +/- 8 yr of age (n = 9). We calculated the E/I ratio, i.e., the average ratio of HRCT number at full expiration to that at full inspiration, as an index for the evaluation of air trapping distal to the small airways. Patients showed significant increases in residual volume and slope of phase III (DeltaN2) of the single-breath nitrogen test, and significant decreases in flow at 50 and 25% of FVC, suggesting hyperinflation and obstruction of the small airways. HRCT of patients revealed the small nodules and ectasis of bronchioles and small bronchi located mainly in segments (S) S2, S3, S4, and S5. The E/I ratio was significantly elevated in patients, and especially higher in the upper lung field than in the lower lung field, suggesting air trapping distal to the small airways. The difference of E/I ratio between the upper and lower field is probably related to the segmental distribution of CT abnormalities. These findings suggest that MAI infection can lead to air trapping distal to the small airways.  相似文献

BACKGROUND: Decreased muscle strength impedes elders' functional performance in daily activities such as gait. The mechanisms whereby increased strength improves gait are unknown. METHODS: A prospective, blinded, randomized trial of moderate intensity strength exercise was conducted and its impact was measured on functional mobility during gait in 132 functionally limited elders. Lower extremity strength was measured, including hip abductor, hip extensor, and knee extensor strength. Of the 132 subjects, 120 subjects (mean age, 75.1 yrs) completed 6 months of elastic band resistance training at least 3 times a week or served as no-exercise controls. RESULTS: Subjects increased their lower extremity strength in the exercise and control groups, by 17.6% and 7.3% (p < .01), respectively. Gait stability improved significantly more in the exercise group than in the control group (p < .05). Increases in forward gait velocity were not significantly different between groups. Peak mediolateral velocity and base of support improved in the exercise group, but not in the control group. Change in lower extremity strength correlated significantly but weakly with many of the gait variables. CONCLUSIONS: Gait stability, especially mediolateral steadiness, improved in the exercise group but not in the control group. These results show that even moderate strength gains benefit gait performance in elders and thus provide a sound basis for encouraging low-intensity strength training for elders with functional limitations.  相似文献

BACKGROUND: The autopsy is an educational experience that helps students correlate clinical findings with basic science issues. Because of the sensitive nature of the autopsy, students' attitudes should be considered prior to its design and implementation. METHOD: In 1992-1993, all 147 second-year students at the Medical College of Ohio completed a 26-item Likert questionnaire about their attitudes toward the autopsy. After participating in an elective autopsy experience, 26 students completed the same questionnaire plus 18 additional questions about the environment and behavior of the pathologist. Pre- and post-autopsy responses were factor analyzed using principal-components analysis with a varimax rotation. Student's t-tests were used to compare (1) the pre-autopsy factor scores of the elective participants and the rest of the class and (2) the pre- and post-autopsy scores of the participants. RESULTS: Factor analysis of the attitude questions identified seven factors. Comparison of the pre- and post-autopsy scores of the elective participants revealed statistically significant positive increases on four factors; three factors remained positive or neutral. CONCLUSION: It was concluded that the autopsy elective had a positive influence on the student's attitudes. In addition, the pathologist's behavior and the environment in which the autopsy occurred influenced the students. Twenty-two (85%) of the students indicated the autopsy should be mandatory for all students.  相似文献

Length of stay (LOS) predictions in acute pancreatitis could be used to stratify patients with severe acute pancreatitis, make treatment and resource allocation decisions, and for quality assurance. Artificial neural networks have been used to predict LOS in other conditions but not acute pancreatitis. The hypothesis of this study was that a neural network could predict LOS in patients with acute pancreatitis. The medical records of 195 patients admitted with acute pancreatitis were reviewed. A backpropagation neural network was developed to predict LOS >7 days. The network was trained on 156 randomly selected cases and tested on the remaining 39 cases. The neural network had the highest sensitivity (75%) for predicting LOS >7 days. Ranson criteria had the highest specificity (94%) for making this prediction. All methods incorrectly predicted LOS in two patients with severe acute pancreatitis who died early in their hospital course. An artificial neural network can predict LOS >7 days. The network and traditional prognostic indices were least accurate for predicting LOS in patients with severe acute pancreatitis who died early in their hospital course. The neural network has the advantage of making this prediction using admission data.  相似文献

Decreases in hepatic blood flow (HBF) have been reported in patients and in animal experiments during cardiopulmonary bypass (CPB). We examined changes in HBF and hepatic oxygen metabolism during CPB in 16 beagles anesthetized with fentanyl. Hepatic arterial blood flow (HABF) and portal venous blood flow (PVBF) were measured by using an electromagnetic flowmeter before and during normothermic and hypothermic CPB with 10 microg x kg(-1) x h(-1) (F-10 group; n = 8) or 50 microg x kg(-1) x h(-1) (F-50 group; n = 8) of fentanyl anesthesia. CPB was conducted with membrane oxygenation and a nonpulsatile pump flow of 2.4 L x m(-2) x min(-1). Hepatic oxygen delivery (HDO2) and consumption (HVO2) were calculated from HBF and oxygen content in arterial, portal venous, and hepatic venous blood. HABF did not change during normothermic CPB in the F-10 group, but it decreased significantly during hypothermic CPB in both groups, especially the F-50 group. During CPB, PVBF and total HBF decreased significantly in both groups-more so with the larger dose of fentanyl--whereas HDO2 decreased significantly because the arterial and portal venous blood oxygen levels decreased. The HVO2 was stable in the F-10 group but was significantly depressed during CPB in the F-50 group. Our results indicate that during hypothermic nonpulsatile CPB larger doses of fentanyl are associated with reduced HBF and impaired HDO2 and HVO2. Implications: Hepatic dysfunction after cardiopulmonary bypass (CPB) has been frequently reported and could be partly attributed to hepatic circulatory disturbance during CPB. We found that, in beagles, large doses of fentanyl were associated with greater decreases in hepatic blood flow and hepatic oxygen metabolism during hypothermic CPB than smaller doses of fentanyl.  相似文献

Development of neural circuits generating fetal oral-motor activity was characterized in an in vitro isolated brainstem block preparation. Rhythmical trigeminal activity (RTA) at E20-E21 resembled either the pattern or rhythm of neonatal RTA. Conversely, at E18-E19, RTA displayed a different pattern of discharge from neonatal RTA, and output was not regular but intermittent with another slow rhythm.  相似文献

An intermittent and cyclic regimen with All-Trans Retinoic Acid (ATRA) and intensive chemotherapy was conducted due to pharmacokinetic studies on ATRA for acute promyelocytic leukemia (APL) in children. We have treated 17 children with APL using ATRA for remission induction followed by an intermittent schedule of ATRA plus intensive chemotherapy (APL-ATRA protocol). There were 10 males and 7 females. The median age was 9.0 years old. The median baseline white blood cell count was 12.1 x 10(3)/microliter, hemoglobin 7.8 g/dl, platelet 4.5 x 10(4) microliters at diagnosis. Sixteen patients showed t(15; 17) translocation. RT-PCR analysis was available in 15 patients and showed PML/RAR alpha rearrangement in all patients. Overall, 13 or 17 newly diagnosed patients (88%) achieved complete remission and EFS was 67%. Compared to the control (same chemotherapy without ATRA regimen), remission induction and EFS were significantly increased. The toxicity of ATRA consisted of retinoic acid syndrome in 1 and pseudotumor cerebli in another. Other toxicities included headache, chelitis, gastrointestinal trouble and bone pain. These results suggest that intermittent and cyclic regimen with ATRA and intensive chemotherapy (APL-ATRA protocol) is highly effective for APL patients.  相似文献
