BACKGROUND: Data on cardiac and vascular structure in secondary hypertension are generally scarce, and no data on the interrelations between cardiac mass and structural characteristics of the vessel wall, both in large and in small resistance arteries, are presently available. OBJECTIVES: The aim of this study was to investigate the relation between structural changes in subcutaneous small arteries, left ventricular mass and wall thickness of the common carotid artery in patients with primary and secondary hypertension. METHODS: Seventy-four subjects were included in the study: 11 patients with pheochromocytoma, 14 with primary aldosteronism (PA), 19 with renovascular hypertension (RVH), 18 with essential hypertension (EH) and 12 normotensive (NT) control subjects. All subjects were submitted to a biopsy of subcutaneous fat. Morphologic characteristics of subcutaneous small resistance arteries (relaxed diameter <300 microm) were directly evaluated using a micromyographic technique. All subjects were submitted to calculation of left ventricular mass index (LVMI) and common carotid artery intima-media thickness (CCIMT), using ultrasound technique. RESULTS: The correlation coefficients between the media to lumen ratio in subcutaneous small arteries (M/L) and LVMI or between M/L and CCIMT were closer in RVH than in pheochromocytoma, EH or NT; in PA the correlation coefficients were slightly less close than those in RVH. An excess prevalence of carotid plaques in RVH was observed. CONCLUSIONS: A close relation between small resistance artery morphology and cardiac or carotid artery structure may be observed in those hypertensive patients in whom the renin-angiotensin-aldosterone system is activated. In constrast, in NT, EH and pheochromocytoma no significant correlation between M/L and LVMI or CCIMT was observed.  相似文献

Interest in measuring the quality of life (QL) in relation to health care has increased enormously in recent years. This is also true for end-stage renal failure where it is important not only to provide a better survival but also the quality of that survival. The aim of this study was to assess the relative influence of different kinds of treatment on end-stage renal disease after the patients' evaluation of their overall QL. We studied 167 patients receiving conservative treatment (45), haemodialysis (44), haemodialysis and erythropoieth (36), and continuous ambulatory peritoneal dialysis (42). The patients completed an original questionnaire consisting of 37 questions divided in five groups and generating 15 QL variables: personal data (name, gender, age, basic kidney disease); sociodemographic data influenced by the illness (family history, working ability, employment status); general health characteristics (fatigue, appetite, wound healing, sleep, resistance to cold); aspects of private life that are mostly influenced by the disease (social interaction, traveling, mood, sports, sexual life), and patients subjective assessment of their condition (self care and happiness). Patients on haemodialysis showed lower levels of QL than that on peritoneal dialysis related to fatigue (p < 0.01), working ability (p < 0.05), wound healing (p < 0.05), and appetite (p < 0.01) compared to the conservative treatment. Peritoneal dialysis had also a statistically significant positive influence on fatigue (p < 0.05) compared to conservative treatment. However, erythropoletin treatment showed better results with regard to traveling (p < 0.05), resistance to cold (p < 0.01), self care (p < 0.05) and mood (p < 0.05) compared to peritoneal dialysis, and working ability (p < 0.05), fatigue (p < 0.05) and mood (p < 0.05) compared to conservative treatment and haemodialysis.  相似文献

Solvent extraction of coal results in a coal-derived extract containing the solvent-soluble organic portion of coal free of almost all mineral matter. A residue is also produced and consists of most of the mineral matter. Although a procedure has been developed to manufacture ultra-pure coke precursors for value-added carbon products from coal-derived extracts, treatment/rc-usc of the residue is necessary to make the process economically attractive and environmentally friendly. In this study, a technique has been developed to beneficiate the residue by specific gravity fractionation. It is found that nearly 70 % of the residue can be separated into a fraction containing less than 3.5% ash for a typical WV bituminous coal using an organic liquid having a specific gravity of 1.5. This treatment enables the residue to serve as a low-ash boiler fuel or a suitable precursor for the manufacture of materials such as activated carbons. Consequently, the potential for coal-derived precursors leading to new and non-conventionai marketable carbon products can be realized easily.  相似文献

In the freshman year of Drexels E4 program, this common mathematical and scientific foundation is cultivated in the Mathematical and Scientific Foundations of Engineering I, II and III (MSFE I, MSFE II, MSFE III). In an integrated fashion, MSFE I presents the essential calculus, physics and engineering mechanics vital to the freshman engineering student. In the first two quarters, MSFE II presents chemistry with clearly defined engineering applications and significance: in the third quarter, MSFE II presents living systems with the same thrust. The influence of extra-cellular polymeric substances in microbiologically influenced corrosion Unexpected pitting failures in copper potable water installations have occurred in some countries (Germany, Sweden, Scotland, South-West England, Saudi-Arabia) in the mid-eighties. These failures could not be expected in a single case based on the chemical composition of the potable water. Further typical manifestation characteristics of this microbiologically influenced corrosion have been i.) an uptake of dissolved and undissolved copper species in an obsolete, non regular fashion, ii.) black layers of copper(II)-oxide on the inner tube surface, accompanied by posniakite and malachite in the pitting areas, and, iii.) the detection of extracellular polymeric substances (EPS). These EPS consist partly of polysaccharides. They could either be produced by microorganisms within the potable water installation or be transported in a dissolved or undissolved state into the installation. Consequently, they sediment on the inner surfaces of the copper tubes. The physico-chemical properties of some kinds of EPS, especially their membrane properties and their cation selectivity, play a key role in this corrosion process. This is shown exemplarily performing diffusion potential measurements and contact corrosion experiments.  相似文献
