Fornix-based trabeculectomy with mitomycin-C

BACKGROUND AND OBJECTIVE: Mitomycin-C (MMC) has been shown to improve the surgical success of trabeculectomy; however, the advantages of MMC have been evaluated almost entirely as an adjunct to limbal-based trabeculectomy. This study evaluated the efficacy and safety of fornix-based trabeculectomy with MMC for glaucomatous patients. PATIENTS AND METHODS: Between January 1993 and April 1995, 71 patients underwent fornix-based trabeculectomy with topical application of 0.4 mg/ml of MMC for 3 minutes. The conjunctiva-Tenon's capsule flap was spread over the limbus and sutured in order to create a visible crease with a water-tight closure. The mean follow-up time was 14.5 months. RESULTS: The mean intraocular pressure (IOP) before surgery was 32.4 +/- 9.7 mm Hg. The average postoperative IOP was 14.04 +/- 9.57 mm Hg. An IOP of 20 mm Hg or less was observed in 57 eyes (80%). Postoperatively, 37 eyes (52%) required no additional medical therapy. One month after surgery, only 2 patients had wound leakage with hypotony and choroidal detachment. Two eyes (3%) had suprachoroidal hemorrhage with loss of vision. A conjunctival "buttonhole" occurred in 2 eyes (3%), but only 1 persisted more than a month. CONCLUSIONS: Fornix-based trabeculectomy using intraoperative application of 0.4 mg/ml of MMC for 3 minutes was found to be as safe and effective as limbal-based trabeculectomy with MMC.     

The effects of physical fitness and feeling vigorous on self-rated health.

Objective: We prospectively studied the hypothesized beneficial effects of feeling vigorous and of objective physical fitness (gauged based on functional capacity) on subsequently assessed self-rated health (SRH), controlling for possible confounders known to be precursors of SRH and of our predictors. We also investigated the reverse-causation hypothesis that SRH predicts subsequent vigor and functional capacity. Design: Participants were apparently healthy employees (N = 779) who underwent a routine health check at two points of time, Time 1 (T1) and Time 2 (T2), about 18 months apart. We used regression analysis, predicting T2 SRH by T1 SRH, the control variables, and T1vigor and functional capacity. Main Outcome Measures: Vigor was assessed using the Shirom-Melamed Vigor Measure; objective physical fitness was indicated by functional capacity following a treadmill exercise, and self-rated health was measured by a single item. Results: As hypothesized, we found that the change in T2 SRH was positively predicted by T1 vigor, functional capacity, and their interactive term. Testing the reverse causation paths, we found that T1 SRH did not predict subsequent functional capacity and was a relatively weak predictor of subsequent vigor. Conclusion: The affective state of vigor and objectively assessed functional capacity interact to predict subsequent changes in self-rated health. Estimation of hydrodynamic nonuniformities in fluidized bed reactors using the potential-flow model for incompressible fluids

It is experimentally shown that the potential-flow model for nonviscous incompressible fluids can be used for estimating the hydrodynamic nonuniformities in fluidized bed reactors.     

The relationships among objective and subjective environmental stress levels and serum uric acid: The moderating effect of perceived control.

This study was designed to investigate the effects of objective and subjective stress levels and perceived control on serum uric acid (SUA). Respondents were 2,504 male and 1,176 female manufacturing employees. Objective job-related stress was assessed by the Ergonomic Stress Level (ESL) inventory (S. Melamed, D. Yekutieli, P. Froom, E. Kristal-Boneh, & J. Ribak, 1999). As expected, ESL was positively associated with SUA, but among male respondents only. The measure of subjective stress, environmental annoyance, was negatively related with SUA for male respondents, thus not confirming the hypothesis. Perceived control was not found to have a direct effect on SUA, after controlling for the ESL measure. For male respondents only, perceived control moderated the relationship between ESL and SUA: This relationship was negative for those reporting above-average control and positive for those reporting below-average control. Between "transparent room" and "sealed room:" professional confidentiality and therapeutic judgement

Patients and therapists are concerned with the complexities of protecting medical confidentiality. The traditional perception of protection of patient confidentiality is that the individual's interest in protecting his confidentiality may conflict with the public's need for information, especially in cases involving possible danger. In fulfilling his dual role of representing both the patient and the public, the therapist acts according to existing laws and regulations. We describe 3 cases in which refraining from providing the Army with information in accordance with the law was recognized in retrospect as having caused the patient suffering, and even danger. It is recommended that the therapist's judgement be considered in addition to other legal criteria for determining whether or not to protect patient confidentiality.     

Parkinson Psychosis Rating Scale: a practical instrument for grading psychosis in Parkinson''s disease

Three hospital sites evaluated the Bayer two-pad urine dipstick as a screening test for microalbuminuria. One pad estimates albumin concentrations between 10 and 150 mg/L, and the second estimates creatinine values between 300 and 3,000 mg/L. The Boehringer Mannheim (BMD) Micral dipstick was also compared and evaluated. The accuracy of the dipsticks was judged by comparison with cuvet-based immunonephelometry for albumin and to standard rate-Jaffe methods for creatinine; these assays were well standardized and controlled and were assumed to give accurate values. Precision of these methods and that of the dipsticks was determined by multiple assays of control materials. Visual or instrument (Clinitek 50 or 100) evaluation of the Bayer or visual checks of the BMD albumin dipstick pad with patients' urines gave clinically acceptable accuracy. The albumin/creatinine ratio from the Bayer dipsticks gave better accuracy for albumin excretion than the albumin pads alone from either manufacturer. This ratio should permit making a good estimate of the 24-hr albumin excretion in a randomly collected urine.     

"Objective and subjective work monotony: Effects on job satisfaction, psychological distress, and absenteeism in blue-collar workers": Correction.

The relation of objective work conditions (work underload, repetitive or varied work) and subjective monotony to job satisfaction, psychological distress, and sickness absence was examined in 1,278 male and female workers. Subjective monotony was moderately related to the objective work conditions. Hierarchical regression analyses showed that the effects on all outcomes were partially mediated by subjective monotony and were also directly related to repetitive work and work underload. Job satisfaction and psychological distress were mainly related to subjective monotony, whereas sickness absence was equally related to the work conditions and subjective monotony. The highest impact was observed for short-cycle repetitive work. Nerve growth factor induces activation of MAP-kinase and p90rsk in human B lymphocytes

A previous report from this laboratory demonstrated that human B lymphocytes expressed nerve growth factor (NGF) receptors on their surface. On the basis of NGF enhancement of B cell proliferation these receptors are presumed to be functional. We have now characterized one of the signaling pathways that NGF may utilize in the functional activation of B lymphocytes. Stimulation of three different human B-lymphoblastoid cell lines with NGF induced the tyrosine phosphorylation and activation of the p42erk-2 isoform of MAP-kinase (MAPK). In addition, NGF induced shifts in the mobility of p90 ribosomal S6 kinase (p90rsk) on immunoblots and increased p90rsk kinase activity in immunoprecipitates. NGF-induced shifts in p90rsk mobility displayed similar dose and time kinetics as NGF-induced MAPK activation. Activation of both MAPK and p90rsk occurred with doses of NGF as low as 400 pg/ml. Preincubation of NGF with anti-NGF Ab inhibited NGF-induced activation of MAPK and p90rsk. These results demonstrate that the interaction of NGF with its receptor on human B cells results in the stimulation of major components of the signaling pathway also initiated by NGF-receptor ligation in cells of neuronal origin.
